Mr. Ben Grubbs Florida Department of Environment Protection Division of Waste Management Drycleaning Solvent Cleanup Program 2600 Blair Stone Road, MS 4520 Tallahassee, FL 32399-2400

RE: PROPOSAL FOR ANNUAL GROUNDWATER MONITORING

AR-PAT’S DRY CLEANING INC.

SEBASTIAN, INDIAN RIVER COUNTY, FLORIDA

FACILITY ID NUMBER: ERIC_4805

PROPOSED TASK ASSIGNMENT: HW685_DC008A

Dear Mr. Grubbs:

Golder Associates USA Inc. (Golder) is pleased to provide the Florida Department of Environmental Protection (DEP) with this proposal for conducting annual groundwater monitoring activities at the AR-Pat’s Dry Cleaners site, located at 720 Harrison Street, in Sebastian, Indian River County, Florida. This proposal includes a description of the proposed scope of services, level of effort sheets for each subtask, and a cost estimate form that summarizes the costs of the proposed services. The scope of work is based on the recommendations accepted by the DEP in the May 25, 2021 electronic mail correspondence to Golder. The proposed scope of services is described below.

SUBTASK 1 – PROJECT MANAGEMENT

To facilitate project quality and satisfactory completion of the proposed scope of work, all aspects of this project will be overseen by an experienced project manager. The project manager will be responsible for the schedule, budget, invoicing, project activities, and communications with DEP. Project management will include coordination of field activities; resource allocation to confirm the work is conducted by qualified professionals in accordance with the project schedule; cost control and tracking of the budget status; and submission of project invoices in accordance with DEP Drycleaning Solvent Cleanup Program requirements. The project manager will be responsible seeing that the work is conducted in a manner that is consistent with the requirements of the DEP approved task assignment.

SUBTASK 2 – MONITORING WELL INSTALLATION

To address the vertical delineation of groundwater impacts in the vicinity of two impacted offsite (City of Sebastian property) shallow surficial aquifer monitoring wells, Golder will install two microwells located adjacent to existing wells MW011S and MW016S. The groundwater flow direction has been in an eastern direction towards the Intracoastal Waterway. The attached Figure 4 shows the approximate monitoring well locations.

Mr. Ben Grubbs Proposal No. 202208346

Florida Department of Environment Protection March 16, 2022

Golder will oversee use of a DPT rig to install two intermediate zone surficial aquifer wells. The proposed monitoring wells installed at the site will be pre-packed screen microwells. The microwells will be constructed of 1-inch diameter, Schedule 40 PVC casing with pre-packed 0.01-inch diameter slot size Schedule 40 PVC screen. The two intermediate zone wells will be constructed with 5-feet of screen installed at approximately 30 to 35 feet bgs.

The well casings and screens will be connected using flush-threaded joints. The annular space between the borehole wall and each well screen will be completed with a 20/30 grade silica sand filter pack from the bottom of the borehole to approximately two feet above the top of the well screen. A minimum 2-foot thick, fine (30/65) grade silica sand seal will be placed above the sand filter pack and the remainder of the annular space will be completed with a Portland Cement Type II bentonite grout seal.

The wells will be installed with flush-mounted surface completion, a 2-foot by 2-foot sloped concrete pad and an 8-inch diameter bolt-down steel cover. Each of the well’s risers will be cut just below final grade and capped with water-tight expandable locking plugs.

To reduce sample turbidity and establish a good hydraulic connection with the aquifer, Golder will develop both wells the following day until purged water remains visibly clear and free of suspended particulate matter. Golder estimates two days to complete utility locates and well installation activities.

Golder will temporarily store the investigation derived waste (IDW) purge water generated from the groundwater sampling event in a 55-gallon steel drum(s). A waste characterization sample will be collected from the IDW drum(s) for analysis of total VOCs using EPA Method 8260 during Subtask 3 (groundwater monitoring). If the waste characterization sample is nonhazardous and acceptable to the disposal subcontractor, the IDW drum(s) will be picked up and transported to a facility for proper disposal. A non-hazardous waste manifest will be provided by the disposal subcontractor upon delivery to the disposal facility.

Upon completion of Subtask 2, an e-mail deliverable will be submitted and include copies of the field well installation forms, field notes, and well permits.

SUBTASK 3 – ANNUAL GROUNDWATER MONITORING

Golder will collect groundwater samples from 20 existing shallow zone monitoring wells included in the groundwater monitoring program, MW001S, MW002S, MW003S, MW004S, MW005S, MW006S, MW007S, MW008S, MW009S, MW011S, MW0012S, MW015S, MW016S, MW017S, MW018S, MW019, MW020S, MW021S, MW022S, and MW023S and the two newly installed intermediate zone monitoring wells MW008I and MW009I, along with a lift station sample for chlorinated VOCs. The original six intermediate zone monitoring wells will not be sampled during this field event. The following monitoring wells MW022S and MW023S will be used as temporary point-of-compliance (TPOC) monitoring wells. The potentiometric map of the shallow surficial aquifer (Figure 2) and chlorinated VOC detection map (Figure 4) from the previous groundwater monitoring report are attached for reference.

Prior to groundwater sampling, depth-to-water measurements will be collected using an electronic water level indicator to determine groundwater depth. During the sampling event, the locks and well caps will be inspected for damage or degradation and repaired or replaced, as necessary.

Groundwater samples will be collected from the wells in accordance with DEP Standard Operating Procedures (SOPs) for Field Activities (DEP SOP-001/01), dated January 2017 (effective April 16, 2018). Prior to collecting the groundwater sample, each well will be purged and field parameters including temperature, pH, specific conductance, dissolved oxygen, turbidity, and oxidation reduction potential (ORP) will be monitored. In addition, groundwater samples from these wells will be analyzed in the field for the following MNA field parameters: hydrogen sulfide, carbon dioxide, and ferrous iron. Purge volumes and field parameters will be recorded on field data sheets, and the field monitoring equipment will be calibrated in accordance with the manufacturer’s recommended procedures.

Groundwater samples will be collected using low flow sampling methods. The groundwater samples will be placed in laboratory-supplied containers, labeled with a unique identification number attached to the outside of the sample container, preserved on ice in a laboratory-provided cooler at a maximum temperature of four degrees Celsius, and delivered to a Florida Department of Health and National Environmental Laboratory Accreditation Program certified fixed-base laboratory along with the completed chain-of-custody documentation to be analyzed for chlorinated VOCs using EPA Method 8260B. Upon receipt of the laboratory analytical results, Golder will evaluate the constituent concentrations to determine if off-site notification is required.

Golder will temporarily store the IDW purge water generated from the groundwater sampling event in a 55-gallon steel drum(s) and follow the same procedures as described in Subtask 2 for storage, characterization, and disposal.

Upon completion of Subtask 3, an e-mail deliverable will be submitted and include copies of the field data sheets, equipment calibration forms, field notes, laboratory analytical report, and laboratory chain-of-custody documentation.

SUBTASK 4 – REPORTING

The annual groundwater monitoring report will include a description of the sampling procedures, evaluation of the analytical results, comparison of current to historical data, tabulated summary of the current and historical analytical data, a site map showing the groundwater analytical results, water level elevation information in tables and figures, laboratory analytical data, conclusions, and recommendations. Golder will also update the DEP GIS tables with the new water level data, field parameter measurements, and VOC data; run the DEP data checker on the completed tables; and submit the updated tables and error reports under this subtask. Additionally, Golder will forward the Automated Data Processing Tool (ADaPT) data deliverables for the groundwater sampling event provided by the laboratory.

HEALTH AND SAFETY

Golder will update the existing site-specific Health and Safety Plan for the field work. The work will be completed in accordance with Golder’s safety policies and procedures. At a minimum, Golder will wear safety glasses, high visibility safety vests, and steel-toed boots. A safety job briefing will be conducted prior to the start of work activities, and anytime the scope of work or site conditions change.

COST AND SCHEDULE

The estimated cost to complete the proposed scope of work is $26,352.25. Golder anticipates the well installation will be completed late May/early June 2022 and the groundwater monitoring event will take place approximately 2 weeks later. The LOE/CEF sheets and the subcontractor bids are included in Attachment A. The following table summarizes the proposed costs and deliverable schedule.

We are not going to provide annual Pricing for the Statewide Contract . After reviewing the scope with my partners and the variability with respect to disposal costing and ever changing fuel costs, we feel we can not bid this work given the current uncertainty in the market. We would be please to bid this work on a casa by case basis.

ProCheck Inspections appreciates the opportunity to provide this proposal. Feel free to contact me at any time if you have any questions, or if you need additional information.

SCOPE OF WORK

We understand the scope to be to search for underground utilities as identified by the customer within (x2)

10’x10’ areas prior to conducting soil borings. We will attempt to trace any utilities for which there are structures visible from the work area, and mark said utilities with paint and flags where possible. The client will be responsible to provide drawings or notify ProCheck Inspections of any utilities known to be entering the work area for which there are no apparent surface features or structures that are visible from the work area. The areas should be laid out, marked, and cleared of obstructions prior to our arrival.

EQUIPMENT

• 350 MHz Hyperstacking GPR Antenna. The antenna is mounted in a stroller frame which rolls over the surface. The surface needs to be reasonably smooth and unobstructed in order to obtain readable scans. Obstructions such as curbs, landscaping, standing water, and vegetation will limit the feasibility of GPR. The data is displayed on a screen and marked in the field in real time. GPR works by sending pulses of energy into a material and recording the strength and the time required for the return of the reflected signal. Reflections are produced when the energy pulses enter a material with different electrical properties from the material it left. The strength of the reflection is determined by the contrast in signal speed between the two materials. The total depth achieved can be as much as 8’ or more with this antenna but can vary widely depending on the conductivity of the materials. The patented hyperstacking antenna can improve depth penetration and resolution when compared with a traditional GPR antenna.

• Electromagnetic Pipe Locator. The EM locator can detect the electromagnetic fields from live power or radio frequency signals. It can also be used in conjunction with a transmitter to connect directly to accessible, metallic pipes, risers, or tracer wires. A current is sent through the pipe or tracer wire at a specific frequency and the resulting EM field can then be detected by the receiver. The receiver is moved over the surface without contacting the ground, so it is not affected by terrain. Depths provided should always be treated as estimates as their accuracy can be affected by multiple factors. Depths achieved can be as much as 20’ depending on the type of signal being traced or methods used.

TERMS & CONDITIONS

1. ProCheck Inspections provides Ground Penetrating Radar (GPR) services in underground applications, including assisting in location services in accordance with ASCE Standard 38-02, Quality Level “B”. GPR is a great tool for detecting objects underground, however, site restrictions and environmental conditions can limit the accuracy and effective depth of GPR. For underground scanning, depth is completely dependent on the composition of soils in the area being surveyed. Customer fully understands that for every foot in depth penetration with the GPR equipment, the pipe/utility must be at least 1” in diameter to be located. For example, at 4 feet in depth, the pipe/utility must be 4” or larger to be detected. Some types of pipes are very difficult to locate, such as clay or concrete pipes, transite piping, and empty PVC type pipes. Given these factors, ProCheck Inspections CANNOT guarantee it will be able to locate ALL utilities on site.

2. Our goal is to provide you with the best answers possible to your questions regarding what lies below the surface, and where it is located. Customer acknowledges that they understand the fact that our answers are based upon the quality of the data retrieved and what ProCheck Inspections believes lies below the surface. The decision to proceed with boring or excavations of any manner, type, or method, it under the sole discretion of the customer and customer assumes full liability for aforementioned processes.

3. ProCheck Inspections will not accept liability for an inaccurate interpretation or any other reason, and customer agrees to release and indemnify ProCheck Inspections and its owners and agents from all losses and damages from all alleged negligence and/or contract claims by customer or any third party. These terms and conditions supersede any other previous terms and conditions either oral or written.

4. Payment is due upon receipt of invoice unless otherwise specified in writing.

5. If for some reason ProCheck Inspections arrives on site and the work is cancelled without prior notification, there will be a charge of

$300.00.

RE: Utility Locating Services – 720 Harrison Street, Sebastian, Florida

Mr. Mongillo, GeoTek Services is pleased to respond to your request for Utility Locating services at 720 Harrison Street located in Sebastian, Florida. We will scan a 10’ x 10’ area around the two locations shown in your PDF drawing with the goal of locating underground utilities. We can only scan the easily accessible open areas for utilities.

Note that we cannot scan beneath fixed objects such as landscaping, fencing, buildings.

We will scan as close as reasonably possible to fixed objects such as walls, landscaping, fences, etc. Our deliverable will be marking paint and/or flags indicating the location of potential underground utilities. It is the client’s responsibility to protect and preserve all marks.

We will use both Electromagnetic Induction instruments as well as Ground Penetrating Radar (GPR) to detect objects below the surface. Due to the nature of sub-surface detection and the physical laws governing radio wave propagation, some buried objects may not be detectable due to depth, obscuration, or a weak radar signature. GeoTek Services will use industry standard best practices to locate sub-surface objects, however some objects may not be detectable. Geotek Services does not guarantee that all underground objects can be found. GeoTek Services assumes no liability for any damages.

We request access to any as-built site plans of the scan areas prior to rendering services. Our fixed price quote for this effort is $750.00. No out-of-pocket expenses are expected for this project. This quote is valid for sixty days.

GPRS appreciates the opportunity to provide this proposal. I encourage you to visit our website (www.gprsinc.com) and contact any of the numerous references listed. Our insurance certificate and W-9 can also be downloaded here. Please feel free to contact me if you have any questions, or if you need additional information.

SCOPE OF WORK

We understand the scope to be to search for underground utilities within the vicinity of (5) proposed boring locations. At each location, we will scan a 10’ x 10’ area around the proposed boring. We will attempt to trace any utilities for which there are structures visible from the work area, while also identifying any additional items we may encounter that are not tied to a visible surface feature. Utilities will be marked on the surface using paint, flags, or other appropriate means. The client will be responsible to provide drawings or notify GPRS of any utilities known to be entering the work area for which there are no apparent surface features or structures that are visible from the work area. The areas should be laid out, marked, and cleared of obstructions prior to our arrival in order to avoid additional charges. Please visit www.simspec.org for an overview of our industry-leading best practices that will be applied to this project.

EQUIPMENT

• Underground Scanning GPR Antenna. The antenna frequencies range from 250 MHz-450 MHz is mounted in a stroller frame which rolls over the surface. The surface needs to be reasonably smooth and unobstructed in order to obtain readable scans. Obstructions such as curbs, landscaping, and vegetation will limit the feasibility of GPR. The data is displayed on a screen and marked in the field in real time. The total depth achieved can be as much as 8’ or more with this antenna but can vary widely depending on the types of materials being scanned through. Some soil types such as clay may limit maximum depths to 3’ or less. As depth increases, targets must be larger in order to be detected and non-metallic targets can be especially difficult to locate. Depths provided should always be treated as estimates as their accuracy can be affected by multiple factors. For more information, please visit: Link

• Electromagnetic Pipe Locator. The EM locator can passively detect the signals from live AC power or radio signals travelling along some conductive utilities. It can also be used in conjunction with a transmitter to connect directly to accessible, metallic pipes, risers, or tracer wires. A current is sent through the pipe or tracer wire at a specific frequency and the resulting signal can then be detected by the receiver. A utility’s ability to be located depends on a variety of factors including access to the utility, conductivity, grounding, interference from other utilities, and many others. Depths provided should always be treated as estimates as their accuracy can be affected by multiple factors. For more information, please visit: Link

• GPS. This handheld GPS unit offers accuracy down to 4 inches, however, the accuracy achieved will depend on the satellite environment at the time of collection and should not be considered to be survey-grade.
