About this file

This is a Facility Maintenance Plan (FMP) template spreadsheet containing detailed maintenance requirements and schedules for fuel facilities at McAlester Army Ammunition Plant (AAP). The template includes sections for administrative information, general facility description, communication plans, safety requirements, recurring maintenance schedules, and component tracking for fuel system assets like storage tanks, pipelines, dispensers, and related equipment.

The template provides standardized formats for documenting maintenance activities across multiple facilities including the Top Load Facility, Service Station #1, and Service Station #2. Key components tracked include above-ground storage tanks, fuel dispensers, piping systems, valves, pumps, filters, and safety equipment. For each component, the template captures details like make/model, capacity, maintenance frequencies (daily/weekly/monthly/annual), UFC reference codes, special considerations, and system availability impacts. The document serves as a framework for contractors to develop site-specific maintenance plans but does not contain actual maintenance requirements or schedules.

Instructions:

This table shows the minimum information required. Additional details should be included based on site specific requirements. Additional POCs may be required.

General Facility Description Facility Description

Purpose:

This Facility Maintenance Plan provides guidance for Contractor personnel in the operation, inspection, and maintenance of permanently installed fuel facilities operated under the control of the Quality Assurance Evaluators. Federal, state, and local environmental laws and regulations shall be complied with if this plan is in conflict with them.

Scope:

The instructions prescribed by this plan outline general fuels maintenance information associated with fuel storage and dispensing systems and provides the minimum periodic contractor maintenance and inspection criteria requirements for the systems. This plan is to be used in conjunction with other existing directives governing fuel system operations and maintenance requirements. Conotractor will properly maintain and repair federally funded petroleum systems and facilities in order to provide compliance with applicable code, criteria, and federal, state, local and/or host nation regulations.

Fuels Facilities:

Organizational tanks, and associated dispensing equipment are maintained and operated by the owning organization. Fuel system maintenance contractors shall perform all maintenance and associated procurement actions on installed fuel systems.

Each installation under the Southwest Division contract are military land-based facilities who receive, store, distribute, and/or dispense liquid fuels.

Standard fuel system equipment includes:

a) Above Ground Sorage Stanks (AST), Underground Storage Tanks (UST)

b) Filter/separator and other filtration device maintenance access, particularly element removal.

c) Manual valves

d) Meter Registers, and Flow Meters.

e) Control Valves.

f) Pumps

g) Tank Monitoring Equipment and Instruments

h) Alarms

Instructions:

1. Input all data as indicated on the sheet.

2. Additional documents referred to in the Special Considerations column shall be included as an appendix to this FMP.

3. The contractor shall generate a Component Unique Identifier (CUID) for each component utilizing the Component Acronym Sheet within this Work Book. The unique identifier must use the following:

Three letter Site Code - Facility # - Component Acronym - Sequential number Example: AAD-00063A-RCVL-0001

If component acronym is not listed, the contractor shall request an acronym be established through the HNC Project Manager and Project Engineer via email. Contractors may utilize other means of tracking/labeling assets such as QR codes in addition to the required CUID however, tracking/labeling must correlate with the CUID. This CUID shall be the standard for all DLA capitalized equipment. Base Asset ID if used by site shall be cross referenced with CUID appropriately.

4. Utilize the UFC Section Reference Work Category as noted on the Component Acronym Sheet.

5. Enter any applicable manufacturer requirements not stated in the UFC. References are acceptable with attachments.

6. Enter any applicable operator support or government owned equipment required to complete the RM task.

7. Enter impact on system availability by performing the RM task. This could include shutdown, lockout, or similar impacts.

8. Enter contract information for any other contract, contractor, or government personnel performing the applicable task. This is applicable to other Centrally Managed Programs, SRM, MILCON, or Public Works/Civil Engineering projects.

9. Print Hard Copies as 11x17.
