COST PROPOSAL FOR SITE RESTORATION WORK PLAN

CONCORD CUSTOM CLEANERS #201

1413 S MONROE STREET, TALLAHASSEE, FLORIDA

DEP FACILITY ID NUMBER: ERIC_4867

PROPOSED TASK ASSIGNMENT: DC002A

Dear Mr. Grubbs:

Golder Associates USA Inc. (Golder) is providing the Florida Department of Environmental Protection (FDEP) with this cost proposal to develop a Site Restoration Work Plan for the above-referenced site. This proposal includes a description of the proposed scope of services, level of effort sheets for each subtask, and a cost estimate form that summarizes the costs of the proposed services. The proposed scope of services is described below.

SUBTASK 1 – PROJECT MANAGEMENT

To facilitate project quality and satisfactory completion of the proposed scope of work, all aspects of this project will be overseen by an experienced project manager. The project manager will be responsible for the schedule, budget, invoicing, project activities, and communications with FDEP and the field team. Project management will include coordination of field activities; resource allocation to confirm the work is conducted by qualified professionals in accordance with the project schedule; cost control and tracking of the budget status; and submittal of project invoices in accordance with FDEP requirements. The project manager will oversee the work so that it is conducted in a manner that is consistent with the requirements of the FDEP-approved task assignment.

SUBTASK 2 – SITE RESTORATION WORK PLAN

Golder completed a site visit with the FDEP and the property owner, Mr. David Barrett, on January 19, 2022. The purpose of the site visit was to complete an assessment of the general site layout and dimensions; identify existing monitoring well and remediation system infrastructure locations; and a discuss site restoration activities with the FDEP and the property owner.

Golder made the following observations during the site visit:

Mr. Ben Grubbs Proposal No. 202208346

Florida Department of Environmental Protection March 1, 2022

▪ There are nine monitoring wells onsite (CCCDW001, CCCMW001, CCCMW002, CCCMW003, CCCMW005, CCCMW006, CCCMW007, CCCMW018, and an unknown well located in the northwest corner of the site (near CCCMW006 and CCCMW007). The monitoring well locations are shown on the figure included in Attachment A. Note: the observed locations of monitoring wells CCCMW003 and CCCMW007 appear to differ slightly from the locations shown in Attachment A and are labeled accordingly.

▪ There are two horizontal vapor extraction wells (HEW-1 and HEW-2) and five nested pairs of vertical vapor extraction wells onsite (VEW-3S/3D, VEW-4S/4D, VEW-5S/5D, VEW-6S/6D, and VEW-7S/7D). The VEW-7S/7D nested pair is located inside the building. The locations are shown on the figure included in Attachment A. Construction details are shown on the figure included in Attachment B (from the November 2015 System Startup Report).

▪ The interior vapor extraction well conveyance piping trench line length (from the VEW-7S/7D nested pair to the south wall of the building) is approximately 27 linear feet. The width of the trench is approximately 1.5 feet.

The trench transects the interior excavation area. The subsurface conveyance piping daylights inside the building at the intersection with the south wall. The piping is then routed through the base of the wall to the exterior and into the subsurface trench line located along the south wall. The trench line and excavation area are shown on the figure included in Attachment A.

▪ The total exterior vapor extraction well conveyance piping trench line length is approximately 266 linear feet.

The trench lines are shown on the figure included in Attachment A.

▪ An FDEP-owned soil vapor extraction (SVE) system (Property ID 121306) is located within a fenced-in compound (approximately 59 linear feet of fencing) in the southeast corner of the site. The system trailer rear doors were open upon arrival to the site. Golder and the FDEP were able close and lock the rear doors (top latch only). One of the rear doors (right side) appears to be damaged, making it unable to be properly latched on the bottom. The fence gate was also unlocked upon arrival. The location of the SVE system is shown on the figure included in Attachment A.

▪ There are two 55-gallon carbon vessels located inside the SVE trailer.

▪ There are two 55-gallon steel drums (corroded) and one poly tank located inside the SVE system compound.

The 55-gallon drums appear to contain liquids.

▪ The aboveground piping associated with the SVE system manifold is located inside the SVE system compound.

▪ There are two wooden power poles located along the southern property boundary. The power poles are associated with the power drop to the SVE system from Monroe Street. A City of Tallahassee electrical meter and power disconnect are attached to one of the poles.

▪ The FDEP license plate (DEP05963) is located on the rear left door to the SVE trailer. The tires are on the trailer.

Golder submitted the above site visit observations and a draft list of proposed recommendations for site restoration to the FDEP and Mr. Barrett via e-mail on January 26, 2022. Mr. Barrett provided comments via e-mail regarding the submittal on January 26, 2022. The FDEP provided a response to Mr. Barrett’s comments via e-mail on February 1, 2022. Golder participated in a teleconference with the FDEP and Mr. Barrett on February 3, 2022 to discuss the site observations and the draft list of proposed recommendations. Following the teleconference, the

FDEP requested that Golder develop a Site Restoration Work Plan to detail the proposed recommendations for site restoration. The plan will include the following:

▪ A summary of previous site assessment and remediation activities completed at the site.

▪ A recommendation for a topographic survey of the existing site features by a licensed professional land surveyor that will be used to design the new asphalt cover.

▪ A recommendation for the evaluation of the structural integrity of the proposed in-place well abandonment methods for horizontal vapor extraction wells HEW-1 and HEW-2 by a structural engineer.

▪ The proposed site preparation activities, including obtaining an updated site access agreement, utility location services, erosion control measures, and documentation of existing site conditions.

▪ Proposed methods for transferring the SVE system from the site to the FDEP storage yard in Lakeland, Florida, including a discussion of the necessary FDEP chain-of-custody documentation.

▪ Proposed analyses for waste characterization of spent carbon located inside of the SVE trailer and liquids staged in 55-gallon drums located inside the SVE treatment compound.

▪ Proposed transportation and offsite disposal methods for the spent carbon and liquids, based on the waste characterization results.

▪ Proposed methods for the removal and disposal of existing asphalt and concrete trench lines surrounding the building. Note: the proposed asphalt removal and replacement area will include the area underneath the canopy along the north wall but will not include the concrete underneath the canopy along the west wall (drive-through area).

▪ Proposed methods for removal and disposal of concrete inside the building along conveyance piping trench line for vapor extraction wells VEW-7S/7D.

▪ Proposed methods for removal and disposal of existing aboveground and subsurface SVE conveyance piping;

SVE system fencing; SVE system electrical conduit and power lines, and wooden power poles associated with the power drop from Monroe Street to the SVE trailer.

▪ Proposed design for new asphalt cover outside of the building and concrete repair inside the building, including backfill and compaction methods and requirements prior to placement.

▪ A list of existing monitoring wells to be abandoned, including a recommendation for groundwater sampling prior to abandonment, and abandonment methods.

▪ Proposed abandonment methods for existing horizontal and vertical vapor extraction wells.

▪ Health and safety considerations, including recommended personal protective equipment and dust control measures.

As discussed with the FDEP, Golder will not solicit bids from contractors until the Site Restoration Work Plan has been approved by the FDEP and the property owner.
