HW685 DC023B
This Task Assignment authorizes Golder to conduct groundwater monitoring activities in accordance with the September 28, 2022, Scope of Work, Cost Estimate, and Bid Documentation, and the follow up email on September 29, 2022 adjusting the project schedule.
Subtask 1: Project Management An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.
Subtask 2: Annual Groundwater Monitoring Golder will collect groundwater samples from monitoring wells MW004, MW511, MW513, MW514, MW612, MW015, MW016, MW021, MW024, MW026, MW028, MW606, MW607, MW014, MW019, MW022, MW025, MW027, MW029, MW608, MW005, MW013 and MW023. Prior to sampling, depth-to-water will be measured. The wells will then be purged, and field parameters will be recorded. The groundwater samples will be collected using low flow sampling methods and analyzed for chlorinated volatile organic compounds (VOCs) via EPA Method 8260. Samples from MW514, MW021, MW607, MW022, MW608, and MW023 will be analyzed for Total Iron via EPA Method 6010. Investigation-derived waste (IDW) will be temporarily stored in a 55-gallon drum. Golder requested bids for IDW drum treatment and disposal from Clark, Erwin, and Whatever Waste. Clark was selected as the lowest responsive and responsible bidder.
Subtask 3: Reporting Golder will submit the Annual Groundwater Monitoring Report deliverable to FDEP.
Golder and the FDEP confirmed on September 28, 2022, that an Access Agreement with the current real property owner of the Sages (ERIC_4596) site has been executed.
The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost for this Task Assignment is $17,848.36 with 106 labor hours.

RE: COST PROPOSAL FOR THE FORMER SAGES CLEANERS
2023 ANNUAL GROUNDWATER MONITORING
5800 FORT CAROLINE ROAD
JACKSONVILLE, DUVAL COUNTY, FLORIDA
DEP FACILITY ID NUMBER: ERIC_4596
PROPOSED TASK ASSIGNMENT: HW685_DC023B
Dear Ms. Shirah:
Golder Associates USA Inc. (Golder) is pleased to provide the Florida Department of Environmental Protection (DEP) with this cost proposal for the 2023 annual groundwater monitoring at the above-referenced site. This proposal includes a description of the proposed scope of services, level of effort sheets for each subtask, and a cost estimate form that summarizes the costs of the proposed services. The scope of work is based on the recommendations provided in the 2022 Annual Groundwater Monitoring Report. The proposed scope of services is described below.
SUBTASK 1 – PROJECT MANAGEMENT
To facilitate project quality and satisfactory completion of the proposed scope of work, all aspects of this project will be overseen by an experienced project manager. The project manager will be responsible for the schedule, budget, invoicing, project activities, and communications with DEP and the field team. Project management will include coordination of field activities; resource allocation to confirm the work is conducted by qualified professionals in accordance with the project schedule; cost control and tracking of the budget status; and submittal of project invoices in accordance with DEP Drycleaning Solvent Cleanup Program requirements. The project manager will oversee the work so that it is conducted in a manner that is consistent with the requirements of the DEP-approved task assignment.
SUBTASK 2 – ANNUAL GROUNDWATER MONITORING AND IDW TREATMENT
Golder will complete the 2022 annual groundwater monitoring event, as outlined in the attached Table 1. The 2023 performance monitoring plan was developed with consultation from the DEP and presented in the 2022 Annual Groundwater Monitoring Report. The plan includes collecting groundwater samples from select wells for chlorinated
Ms. Ginger Shirah Proposal No. 202208346
Florida Department of Environmental Protection September 28, 2022 volatile organic compound (VOC) analysis, as well as total iron as part of the continued implementation of the Underground Injection Control (UIC) monitoring. Figures 6 through 8 from the previous annual groundwater monitoring report show the well locations and chlorinated VOC results for the shallow, intermediate, and deep zones of the surficial aquifer
Prior to groundwater sampling, depth-to-water measurements will be collected using an electronic water level indicator at the 23 monitoring wells that will be sampled to determine groundwater depth. During the sampling event, the locks and well caps will be inspected for damage or degradation and repaired or replaced, as necessary.
Groundwater samples will be collected from the wells in accordance with DEP Standard Operating Procedures (SOPs) for Field Activities (DEP SOP-001/01). Prior to collecting the groundwater sample, each well will be purged and field parameters including temperature, pH, specific conductance, dissolved oxygen, turbidity, and oxidation reduction potential (ORP) will be monitored. Purge volumes and field parameters will be recorded on field data sheets, and the field monitoring equipment will be calibrated in accordance with the manufacturer’s recommended procedures.
Groundwater samples will be collected using low flow sampling methods. The groundwater samples will be placed in laboratory-supplied containers, labeled with a unique identification number attached to the outside of the sample container, preserved on ice in a laboratory-provided cooler at a maximum temperature of four degrees Celsius, and delivered to a Florida Department of Health and National Environmental Laboratory Accreditation Program certified fixed-base laboratory along with the completed chain-of-custody documentation to be analyzed for chlorinated VOCs using EPA Method 8260B. Six samples will be analyzed for iron using EPA Method 6010 to satisfy the UIC concerns. Upon receipt of the laboratory analytical results, Golder will evaluate the constituent concentrations to determine if off-site notification is required.
Golder will temporarily store the investigation derived waste (IDW) purge water generated from the groundwater sampling event in a 55-gallon steel drum(s). A waste characterization sample will be collected from the IDW drum(s) for analysis of total VOCs using EPA Method 8260. If the waste characterization sample is acceptable to the disposal subcontractor, the IDW drum(s) will be picked up and transported to a facility for proper disposal. A non-hazardous waste manifest will be provided by the disposal subcontractor upon delivery to the disposal facility.
Upon completion of Subtask 2, an e-mail deliverable will be submitted and include copies of the field data sheets, equipment calibration forms, field notes, laboratory analytical report, and laboratory chain-of-custody documentation.
SUBTASK 3 – REPORTING
The annual groundwater monitoring report will include a description of the sampling procedures, evaluation of the analytical results, comparison of current to historical data, tabulated summary of the current and historical analytical data, a site map showing the groundwater analytical results, water level elevation information in tables and figures, laboratory analytical data, conclusions, and recommendations. Golder will forward the Automated Data Processing Tool (ADaPT) data deliverables for the groundwater sampling event provided by the laboratory.
HEALTH AND SAFETY
Golder will update the existing site-specific Health and Safety Plan for the field work. The work will be completed in accordance with Golder’s safety policies and procedures. At a minimum, Golder will wear safety glasses, high visibility safety vests, and steel-toed boots. We are not going to provide annual Pricing for the Statewide Contract . After reviewing the scope with my partners and the variability with respect to disposal costing and ever changing fuel costs, we feel we can not bid this work given the current uncertainty in the market. We would be please to bid this work on a casa by case basis.
