RE: Utility Locating Services – Jet Cleaners Site located in Jacksonville, Florida
Ms. Kamakazi, GeoTek Services is pleased to respond to your request for Utility Locating services at the Jet Cleaners Site located at 1345 Gandy Street in Jacksonville, Florida. We will scan a (10’ x 10’) area around ten proposed bore locations and the trenching areas shown in your PDF drawing with the goal of locating underground utilities. We can only scan the easily accessible open areas for utilities. Note that we cannot scan beneath fixed objects such as landscaping, fencing, buildings. We will scan as close as reasonably possible to fixed objects such as walls, landscaping, fences, etc. Our deliverable will be marking paint and/or flags indicating the location of potential underground utilities. It is the client’s responsibility to protect and preserve all marks.
We will use both Electromagnetic Induction instruments as well as Ground Penetrating Radar (GPR) to detect objects below the surface. Due to the nature of sub-surface detection and the physical laws governing radio wave propagation, some buried objects may not be detectable due to depth, obscuration, or a weak radar signature. GeoTek Services will use industry standard best practices to locate sub-surface objects, however some objects may not be detectable. Geotek Services does not guarantee that all underground objects can be found. GeoTek Services assumes no liability for any damages.
We request access to any as-built site plans of the scan areas prior to rendering services. Our fixed price quote for this effort is $690.00. No out-of-pocket expenses are expected for this project. This quote is valid for sixty days.

LMS will include all labor, material, equipment, subcontractors, and any other cost, and install a remedial system as indicated on Figure 10 in accordance with the specifications. The 7 legs of the conveyance pipe (5 SVE and 2 spares) to the exterior manifold and then connect the 2-inch header to the SVE system. Also include materials and labor to assemble a 7 leg exterior manifold.
This item also includes contracting the services of an Electrician. PRM will contract the services of an Electrical Contractor to provide the electrical service connections. Assumes the power provider is able to provide access to the existing transformer located adjacent to the Remedial System. Electrician to provide necessary meter can and disconnects for power hookup. LMS/Electrician will trench from transformer to treatment system, provide PVC conduit and wires, provide disconnect at the remedial system, and connect to single point on the Remedial System estimated to be approximately 30 feet as per the electrical load requirements of the SVE system and the SVE system Process and Instrumentation Diagram provided in Figure 11. Electrical Contractor to provide startup assistance (minimum 4 hours).
This item does not include providing the 55-gallon Drum of Vapor Phase Carbon (VP-55) with applicable hoses and fittings, which will be provided by WSP.
WSP to provide additional 55-gallon drums to store excess soil generated during well installation and trenching activities. WSP is responsible for the sample, transport, and disposal.
It is understood that WSP will be providing a FDEP owned remedial systemk. Both the system and the VGACs are assumed to be on-site prior to LMS arrival.
Site Restoration and Demobilization LMS will include all labor, material, equipment, subcontractors and any other cost to perform all site restoration, all other costs to remove contractor resources and materials that are not to be included within the final product and demobilization of all personnel, equipment, materials and any other incidental costs.
Assumptions and Exclusions
The excavation activities will occur between the hours of 7:00 AM or after 6:00 PM, Monday through Friday, unless obtaining prior written approval from WSP and OWNER. The costs provided are in accordance with this request and anything beyond would be considered beyond the scope of work.
Assumes no utilities that would interfere with the installation. If excessive underground utilities are found that impede progress on-site, LMS will discuss changes with the client on-site. Cost per day/per hour of $3,404.97/$340.49 may apply.
WSP is responsible for providing a 3rd party utility location. LMS will maintain an active 811 ticket through Sunshine State One Call of Florida.
All chlorinated-solvent contaminated soils to be stage on visqueen plastic and ultimately stored in 55-gallon drums. Any sampling and disposal required will be performed by others.
LMS assumes that the trenches material is free from large rocks or boulders that cannot be removed with the equipment on-site (mini ex) within a reasonable amount of time.
Project sign-off by others.
No compaction testing is included.
Existing asphalt and concrete to be removed are assumed to be approximately 6 inches thick and can be cut with a 14-inch walk-behind concrete saw.
PRM is not responsible for any charges from the power company to bring power to the new services.
No performance bonds are included in this proposal.
LMS will maintain a low-dust construction site and has included dust suppression measures (i.e. cutting by wet methods if water is available). Assumed water available on the site.
LMS will discuss any deviations from the provided Construction Drawings prior to performance of the work which could result in a change in scope and/or cost.
It is LMS’s understanding that a General Contractor’s license nor City of Jacksonville permitting is not required for this type of work.
LMS acknowledges that the quantities of work to be performed change and the locations of the SVE treatment points are subject to change based on site conditions.
