About this file

This document comprises Task Assignment Change Order (TACO) #3 and related TACO #2 for Contract HW689 between the State of Florida Department of Environmental Protection (FDEP) and APTIM Environmental & Infrastructure, LLC. The contract pertains to environmental assessment and remediation services at the Spring Lake Cleaners Inc. (Former) facility located at 1120 Havendale Boulevard NW in Winter Haven, Polk County, Florida. The project number is ERIC_5381 (Former Facility No. 539502466), with Gabriel Sostre serving as the FDEP Project Manager and Eric Kramer as the contractor's project manager.

TACO #3 reduces the total task assignment cost from $128,577.17 to $106,551.26, representing a decrease of $22,025.93 or 17.13%, while maintaining the completion date of April 17, 2026. The scope reduction removes the installation of additional monitoring wells and associated subcontracted drilling, oversight labor, equipment, vehicles, and expendables from Subtask 4, based on FDEP approval to collect additional sampling without well installation. The revised deliverables and invoice schedule include: Subtask 2 (MAGS and Soil Sampling, $43,112.83), Subtask 3 (DPT Field Notes, $34,947.68), Subtasks 4, 5, & 1 (Site Assessment Report, $27,075.94), and Subtask 6 (PFAS Email Deliverable, $1,414.81), totaling $106,551.26. TACO #2 had previously increased costs to $128,577.17 with an extended completion date from January 16, 2026 to April 17, 2026 (91 additional days), adding professional services for additional groundwater and PFAS sampling activities.

Proposed Scope of Work Spring Lake Cleaners Inc. (Former) Spring Lake Plaza 1120 Havendale Boulevard NW Winter Haven, Polk County, FL 33881 Site Assessment and Reporting Task Assignment DC029A TACO #3 ERIC_5381 (Former Facility No. 539502466) Aptim Environmental & Infrastructure, LLC November 21, 2025

The following Task Assignment Change Order (TACO) #3 has been prepared to update the current TA based on a review of the preliminary findings by the FDEP and the associated recommendations. Based on the preliminary findings, it has been recommended and approved by the FDEP that the existing wells and piezometers be resampled and additional MAGS testing be performed. Since the additional MAGS testing is specifically related to the design of an interim source removal (ISR) document, this testing will be addressed in the task assignment for the development of the ISR Plan. Therefore, TACO #3 was prepared to remove elements of Subtask 4 that will not be performed, based on the recommendations.

Subtask 1-Project Management Project management hours are calculated based on the labor hours in the scope of work.

Subtask 2-Meeting & MAGS/Soil Sampling: No Changes Subtask 3-DPT & GW Sampling Field Activities: No Changes

Subtask 4-Well Installations:

Based on a review of the initial analysis for the vapor, soil and groundwater reported to date, the FDEP approved the collection of additional sampling, but removed the installation of additional monitoring wells at this time. Therefore, the Subtask removes the subcontracted drilling and the associated oversight (labor), equipment, vehicles and expendables. Since the total well count will be reduced, the analytical for groundwater samples have been reduced to the existing well count. Additionally, the IDW disposal costs have been reduced, based on an updated estimated drum count which will require transportation and disposal.

Subtask 5-SAR Reporting: No Changes Subtask 6-Additional PFAS Sampling: No Changes

No deliverable or cost to completion extensions should be necessary at this time.

APTIM proposes to perform this scope of work in accordance with the terms and conditions of the contract executed between APTIM Environmental & Infrastructure, LLC and Florida Department of Environmental Protection No. HW689. APTIM will begin the proposed scope of work upon issuance of the Task Assignment Notification.
