Proposed Scope of Work Gators One Hour Cleaners 737 NE 36th Avenue, Ocala, Florida 34470 Annual Sampling Task Assignment HW689 DC007B ERIC_4908, (Former Facility ID #429500902) Aptim Environmental & Infrastructure, LLC 12/11/25

The following proposed scope of work for Task Assignment DC007B is for the collection and sampling of nine passive diffusion bag (PDB) samplers. In addition, groundwater samples will be collected from three monitoring wells that reported PFAS compounds exceeding the Provisional Groundwater Cleanup Target Levels (PGCTLs) from the June 2024 sampling event. The scope of work proposed in this task assignment will also update the groundwater quality data with respect to PFAS compounds.

Subtask 1-Project Management Project management hours are calculated based on the labor hours anticipated in the scope of work.

Subtask - 2: Monitoring Well Sampling (VOH) The proposed scope of work includes the collection of depth-to-groundwater measurements from the existing 24 monitor wells (MW001 through MW024). Samples will be collected from the PDB’s harvested from monitoring wells MW001, MW005, MW010, MW012, MW013, MW016, MW021, MW022 and MW011. These bags were deployed on June 27,2024. The water samples from the nine existing PDB’s will be submitted for analysis by EPA Method 8260 for volatile organic halocarbons (VOHs).

This scope of work includes installation of replacement passive diffusion bag (PDB) samplers into the nine wells that contain PDBs. The PDBs will be obtained from ALS Environmental, a licensed manufacturer of passive diffusion samplers. The selected PDB samplers for 2” diameter monitoring wells MW001, MW011 MW012, MW013, MW016, MW021 and MW022 are 24” long, 1 ¼” in diameter and are manufactured from 4 Mil-thick, low-density polyethylene. For the 1” diameter monitoring wells MW005, the selected PDB samplers will be 48” long and ¾” in diameter. The ¾ inch diameter PDB sampler deployed into 1-inch diameter monitoring well MW010 will be 36 inches long to ensure full submersion of the PDB due to the shallow depth of this well. The samplers will be ordered pre-filled with certified, laboratory-grade, analyte-free, deionized water. The nine PDBs will be deployed to the following depths:

1. MW001, (2” well) total depth of approx. 31 ft bls (placement from 29 to 31 ft bls),

2. MW005, (1” well) total depth of approx. 31.5 ft bls (placement from 27.5 to 31.5 ft bls),

3. MW010, (1” well) total depth of approx. 28 ft bls (placement from 25 to 28 ft bls),

4. MW012, (2” well) total depth of approx. 99 ft bls (placement from 97 to 99 ft bls),

5. MW013, (2” well) total depth of approx. 62 ft bls (placement from 60 to 62 ft bls),

6. MW016, (2” well) total depth of approx. 56.5 ft bls (placement from 54.5 to 56.5 ft bls),

7. MW021, (2” well) total depth of approx. 61 ft bls (placement from 59 to 61 ft bls),

8. MW022, (2” well) total depth of approx. 60 ft bls (placement from 54 to 56 ft bls).

9. MW011, (2” well) total depth of approx. 147 ft bls (placement from 141 to 143 ft bls).

The excess sample and decontamination fluids from the sampling event will be placed into a drum for storage onsite and future pickup and disposal. Erwin Remediation was selected to provide IDW transportation and disposal, based on bids recently procured. An IDW sample will be analyzed by EPA Method 8260 for VOHs (only) to characterize the waste. Note that PFAS constituents have previously been reported in groundwater at the site exceeding the PGCTL and therefore the disposal rate is increased. Please see the attached IDW bid summary documents.

Subtask - 3: Annual Groundwater Reporting (VOHs) At the conclusion of the groundwater sampling event and upon receipt of the analytical reports from the fixed laboratory and the FDEP State Laboratory, a Groundwater Monitoring Report documenting the sampling activities and results will be prepared and include conclusions and recommendations for future site activities.

Subtask - 4: PFAS Sampling and Reporting During the collection of VOC groundwater samples, additional monitoring for PFAS will be performed with the collection of PFAS samples from monitoring wells MW010, MW012, and MW013, which are the only wells previously sampled exceeding the PGCTLs for PFAS constituents. These wells will be sampled conventionally. One field regent blank utilizing laboratory provided PFAS-free DI water is also proposed for collection and analysis.

Groundwater samples from the three monitoring wells proposed to be sampled for PFAS will be collected in accordance with the Perfluorinated Compounds Field Sampling Protocols. The PFAS samples will be shipped to the Florida DEP Laboratory in Tallahassee, Florida, and analyzed for the presence of perfluorinated substances, by Method DEP SOP: LC-001-3, including PFOA and PFOS. The additional purge and decon water will be placed in a DOT approved drum and further characterized by the collection of an IDW sample PFAS (State Laboratory).

Description Due Date

Collection of Samples and Deployment of PDBs 3/6/26

Submit Annual Report 4/24/26

Task Expiration Date 6/26/26

APTIM proposes to perform this scope of work in accordance with the terms and conditions of the contract executed between APTIM Environmental & Infrastructure, LLC and Florida Department of Environmental Protection No. HW689 signed December 21, 2021. A Site Access Agreement with the property owner was executed in May 2024. APTIM will begin the proposed scope of work upon written authorization.

As discussed this is for the Haz Waste program so every T&D project will have chlorinated compounds. We do not do any petroleum cleanup/monitoring in the Haz Waste program although some sites could have petroleum hydrocarbons along with the chlorinated compounds. So please confirm the costs I input for you on the attached sheet and reply if you agree, thanks.

RE: IDW Drum Transportation & Disposal North, Central, West Coast, East Coast, & South Florida Lewis Proposal No. 40908LA

Dear Mr. Kramer, Lewis Environmental, LLC (Lewis) is pleased to offer APTIM Orlando (APTIM) this proposal to provide waste transportation and disposal services across the state of Florida. This proposal is based on the information provided by the client and from performing similar work throughout the Southeast region. The following is offered for your review and consideration:

Scope of Work:

1. Prior to mobilization, Lewis will prepare waste profiles for review and signature by the authorized agent for generator. Lewis will then submit signed profiles to the designated disposal facilities for final review and approval.

2. Upon disposal facility approval and scheduling of the waste removal event with the client.

Lewis will mobilize the following crew and equipment: one driver, one rack truck, and any accessories required for this scope of work.

3. Lewis will inspect and label all proposed waste materials onsite for DOT Compliance.

4. Lewis will load all permitted waste streams onto the rack truck for offsite shipment.

5. At shipment time, Lewis will provide waste manifests for review and signature by an onsite agent for the generator.

6. Lewis will then transport the waste drums to the designated licensed facilities for disposal.

7. Upon receipt from the facilities, Lewis will provide APTIM with the final copies of all waste disposal documentation.

Pricing Schedule:

Description of Services Cost Transportation for the North Zone $1,285.00 / Trip Transportation for the Central Zone $860.00 / Trip Transportation for the West Zone $819.00 / Trip Transportation for the East Zone $1,040.00 / Trip Transportation for the South Zone $1,433.00 / Trip Profile for New Sites, Non-PFAS $100.00 / Each 55 Gallon Drum Disposal Non-PFAS $123.00 / Drum Profile for New Sites, PFAS Impacted $150.00 / Each 55 Gallon Drum Disposal PFAS Impacted $475.00 / Drum

Assumptions & Clarifications:

Lewis assumes all work will be performed on normal business days Monday through

Friday (non-holidays).

Lewis understands client will have all proposed waste containers DOT compliant prior to

Lewis mobilization to the site. This includes non-leaking or non-bulging, lids, bungs and rings in place and fastened, and no holes or major dents. If any of the containers are not DOT shippable, Lewis will provide the missing items or overpack the drummed waste into 85-gallon overpack steel containers at an additional cost.

Clear and adequate access to the site will be available on scheduled workdays, Lewis assumes access within 50 feet of the work area, including adequate parking for company vehicles.

Lewis assumes all surface debris/obstructions will be removed from the work area prior to arrival. Failure to do so may cause delays in work and additional expenses.

Lewis assumes sanitary facilities will be made available to Lewis personnel.

The cost to perform sampling and analytical testing is not included in this proposal.

Lewis assumes that the customer will provide the Florida Department of Environmental

Protection hazardous waste site number, known as an EPA ID number for profiling and manifesting of the PFAS impacted waste.

Lewis assumes that all activities involving completion of the scope of work can be performed in level D Personal protective equipment (PPE).

The cost and terms of this proposal are valid for no more than a one-year period. Lewis reserves the right to adjust pricing based on fluctuating disposal costs.

Lewis assumes that any project delays resulting due to unforeseen site conditions or the above assumptions/clarifications will be billed at Lewis time and material rates.

Lewis appreciates the opportunity to provide you with a cost estimate for the work described above. If you have any questions regarding our proposal at any time, please feel free to contact the undersigned at your convenience. Thank you for your consideration.

Sincerely,
