About this file
This document is a Task Assignment Change Order (TACO) #1 issued by the State of Florida Department of Environmental Protection (FDEP) to APTIM Environmental & Infrastructure, LLC for environmental assessment services at Care Cleaners of Jacksonville, Inc., located at 8011-1 Merrill Road in Jacksonville, Duval County, Florida. The task assignment number is DC030A, with FDEP Project Manager Nicole Knutson and contractor Project Manager Eric Kramer serving as the primary contacts. The change order reconciles three previously approved Work Change Directives (WCDs #1, #2, and #3) related to unforeseen field conditions and additional analytical work required during the initial site assessment phase.
The amended total task value increased from $102,782.76 to $106,876.86, representing an increase of $4,094.10. The scope of work encompasses project management, MAGS vapor well installation and soil sampling, direct push technology groundwater sampling, monitoring well installations, site assessment reporting, PFAS groundwater sampling, and equipment inspection services. Deliverables include field notes packages with boring logs and analytical reports (due 07/31/2025 and 08/29/2025) and a comprehensive Site Assessment Report with PFAS sampling results (due 11/07/2025). Invoice payments are structured upon FDEP approval of each deliverable, with amounts of $43,584.08, $25,586.42, and $37,706.36 corresponding to the three major deliverable milestones. The task completion date remains January 16, 2026, and all costs are on a fixed-price basis under contract HW689.

FROM: Nicole Knutson, Project Manager
SUBJECT: Negotiation Summary
HW689 DC030A-1
DATE: August 15, 2025
This Change Order reconciles WCDs 1, 2, and 3 for Care Cleaners of Jacksonville. WCD #1 was approved for additional hours to complete the MAGS testing and soil sampling during the Subtask 2 field activities at the Care Cleaners. Subtask 2 also resolves WCD #2 which was approved for the SPLP extraction and analysis of three (3) soil samples based on the total analytical results, which exceeded the Soil Cleanup Target Level (SCTL) for leachability. WCD #3 was approved for an additional site visit to oversee a drycleaning technician with OEM Laundry troubleshoot issues associated with equipment malfunctions, which the operator claims is a result of the assessment activities within the store.
Total task amount has increased from $102,782.76 to $106,876.86. The difference between the revised cost and the original cost is $4,094.10. APTIM’s time, labor, and subcontractor costs for these activities have been reviewed and are appropriate. The level-of-effort and cost estimate are reasonable for the scope of services. Final deliverable due date remains November 7, 2025. Task completion date remains January 16, 2025.
