Proposed Scope of Work Crystal Cleaners 2316 South Dixie Highway (US-1) West Palm Beach, Palm Beach County, FL 33401 Site Assessment and Reporting Task Assignment DC017B TACO #1 ERIC_5171 (Former Facility No. 509502101) Aptim Environmental & Infrastructure, LLC October 16, 2023

The following is the proposed scope of work for DC017B Task Assignment Change Order (TACO) #1 for the former Crystal Cleaners site located in West Palm Beach, Palm Beach County, Florida. The proposed scope of work under Task Assignment DC017B TACO #1 is to resolve Work Change Directive (WCD) #1 previously approved for the site.

Subtask 3 Soil & GW Field Activities

WCD #1 was approved to increase the drilling costs, as requested by the subcontractor, Groundwater Protection. The subcontractor submitted an email indicating that one additional night of hotel and per diem for the crew was requested for the proposed field work. In addition, the subcontractor requested funds for permitting, which were inadvertently not included in the original quotation. The total WCD #1 request was approved for $385.00. Based on the final invoice for the drilling, WCD #1 will be resolved for a total of $180.40, the costs in excess of the originally quoted scope of work and subcontractor approved budget. The subcontractor email requesting the additional funds and the invoice for the drilling services are attached as backup. A copy of executed WCD #1 is also attached.

Currently the site is on track for completion of the SAR within the approved timeline, however, the FDEP has requested additional testing due to vapor concentrations encountered during the previous MAGS testing. These costs will be incorporated into a separate TACO and any scheduling changes will be requested at that time.

APTIM proposes to perform this scope of work in accordance with the terms and conditions of the contract No. HW689executed between Florida Department of Environmental Protection and APTIM Environmental & Infrastructure, LLC.
