About this file
This document is a Task Assignment Change Order (TACO #3) executed between the State of Florida Department of Environmental Protection (FDEP) and APTIM Environmental & Infrastructure, LLC (APTIM). The change order pertains to environmental remediation work at the Former Brantley Cleaners site (ERIC_5454) located in Altamonte Springs, Seminole County, Florida, under Contract HW689. The TACO was executed on June 16, 2025, with authorization from FDEP Project Manager Nicole Knutson and APTIM Project Manager Eric Kramer.
TACO #3 reconciles Work Change Directive (WCD #2), which authorized APTIM to oversee modifications to the remedial system following a failure of the knockout water storage tank and associated piping. The total task amount increased from $225,165.95 to $231,625.08, representing a cost increase of $6,459.13. The amended compensation consists of a fixed unit rate of $212,425.08 for labor and services and a cost reimbursement component of $19,200.00 for utilities, with no changes to the task completion date of February 27, 2026. The scope includes project management (Subtask 1), annual groundwater sampling (Subtask 2), remedial system replacement documentation (Subtask 3), semi-annual operations and maintenance reporting (Subtasks 4 and 6), utility costs for quarters 1-4 (Subtasks 5 and 7), and system repairs (Subtask 8/WCD #2). All work is to be performed under the terms of Contract HW689 executed December 21, 2021, with final deliverables due by December 12, 2025.

HW689 DC016C-3
DATE: June 13, 2025
This Change Order reconciles WCD #2, which authorized APTIM to oversee modifications to the remedial system. The APTIM engineer on site reported a failure of the knockout water storage tank and piping. As a result, APTIM removed the water storage tank and prepared it for transport to the manufacturing facility. After the tank was modified, APTIM engineers provided oversight as the system components were reinstalled.
Total task amount has increased from $225,165.95 to $231,625.08. The total increase in cost is $6,459.13. APTIM’s time, labor, and subcontractor costs for these activities have been reviewed and are appropriate. The level-of-effort and cost estimate are reasonable for the scope of services. Final deliverable due date remains December 12, 2025. Task completion date remains February 27, 2026.

Upon not being able to establish telemetry contact with the remedial system at the former Brantley Cleaners APTIM sent an engineer to determine the cause. Engineer reported failure of the knockout water storage tank and associated piping.
Fabrication contractor will remove tank for transport back to their manufacturing facility to repair broken welds and piping. In addition, as a prevention for future issues a vent value and two level probe will be retrofitted and programmed into the PLC. APTIM engineers to review modification to system, oversee onsite work on remedial system for removal and re-installation of system components.
