About this file

This document is a Task Assignment Change Order (TACO #4) issued by the State of Florida Department of Environmental Protection (FDEP) to APTIM Environmental & Infrastructure, LLC for environmental assessment services at the former Deluxe Cleaners & Laundry site located at 1622 West Kennedy Boulevard, Tampa, Hillsborough County, Florida (FDEP Site ID ERIC_4696). The change order amends a previously executed Task Assignment (DC018B) under Indefinite Delivery Vehicle Contract HW689. The contracting parties include FDEP (represented by Project Manager Ben Grubbs) and APTIM (represented by Project Manager Eric Kramer).

TACO #4 authorizes an increase in subcontractor costs totaling $1,887.63, resulting in a revised total task assignment value of $149,620.61 (increased from $147,732.98), with 686 labor hours. The cost increase is specifically associated with Subtask 4 (Macrocore and DPT Groundwater Sampling) and relates to soliciting new Temporary Traffic Control Plan (TTCP) bids after the original subcontractor ceased Tampa-area operations. The selected replacement TTCP provider is Prime Traffic Control at $11,717.78. The task completion date has been extended from December 3, 2025, to May 29, 2026 (177 additional days). Invoicing follows a milestone-based schedule tied to FDEP approval of deliverables including analytical results, field notes, well construction logs, and the site assessment report. The revised completion date accommodates well installation work scheduled for January-February 2026 and final reporting for March 2026.

Proposed Scope of Work Deluxe Cleaners & Laundry 1622 West Kennedy Boulevard. Tampa, Hillsborough County, FL 33606 Site Assessment and Reporting Task Assignment DC018B TACO #4 ERIC_4696 (Former Facility No. 299500411) Aptim Environmental & Infrastructure, LLC November 13, 2025

Deluxe Cleaners DC018B Task Assignment Change Order (TACO) #4 was created to address the ongoing issues associated with the necessary Temporary Traffic Control Plans (TTCPs), previously referred to as Maintenance of Traffic (MOT), for the various offsite areas where drilling activities will be performed. Following approval of the previous TACO, the subcontractor selected to provide the MOT informed us that they stopped servicing the Tampa area. Therefore, additional bids were requested by the FDEP.

Subtask 1-Project Management:

This task includes project management activities associated with the completion of the remaining Subtasks, as calculated in accordance with Contract HW689.

Subtask 2-Preliminary Site Activities and MAGS Testing: No Changes Subtask 3-Soil and Groundwater DPT Sampling Activities: No Changes Subtask 4-Well Installations:

APTIM has previously obtained four permits for the installation of wells within several right-of-ways (ROWs) surrounding the former Deluxe Cleaners.

1) City of Tampa permit number ROW-25-0000105 has been developed to allow for the installation of wells and the collection of soil samples in the alley between the Starbucks and the Dunkin’.

2) City of Tampa permit ROW-25-000013 was developed to allow for the installation of a shallow and deep well in a parallel parking location on the west side of South Dakota Avenue.

3) City of Tampa permit ROW-25-0000104 was created to allow for the installation of a shallow and deep well within a parallel parking lane on the west side of South Rome Avenue

4) Finally, FDOT permit 2024-K-796-00174 was created to allow for the installation of a shallow well within the northern Right-of-Way of State Road 60 (West Kennedy Boulevard).

TTCPs were developed for each permit, as follows:

1) The drilling and soil sampling within the alley between the Starbucks and the Dunkin’ will require a TTCP to allow the alley to be shut down to through traffic while the work is being completed. Since the road becomes one-way on the residential portion, flaggers will be required to control traffic into and out of this portion of the alley so the residences can leave and enter the area without incident.

2) The South Dakota work will require a TTCP to close of the parking lane and barricade the parking spaces from access.

3) The TTCP for on South Rome Avenue will also require closure of the parking lane and the placement of barricades to block access to the parking spaces.

4) The required TTCP for the well proposed in the northern Right-of-Way of State Road 60 will require a sidewalk closure (Standard Index 102-660).

5) Diagrams of the required TTCP are provided in the attached bid documents.

Due to the previous provider withdrawing services in Tampa, additional bids were requested (see attached MOT Bid Summary). Subtask 4 changes the TTCP provider to Prime Traffic Control so the work can move forward.

Subtask 5-Repoting: No Changes Subtask 6-WCD#3: No Changes Subtask 7-Permitting and MOT Development: No Changes

Subtasks 2, 3, 6 and 7 have been completed and the results and deliverables reported. Having completed efforts to initiate access to the offsite ROW locations for drilling and upon approval of TACO #4 the wells will be scheduled for installation in January (2026) and sampled soon thereafter. The results of the sampling will be reported to the FDEP upon receipt to determine if additional assessment activities should be performed, or if reporting (SAR) should be completed. The well installation extension for Subtasks 4 and 6 to February 25, 2026, and reporting will be extended to March 25, 2026 (Subtask 5 and 1). The task completion date should be extended to May 29, 2026.

APTIM proposes to perform this scope of work in accordance with the terms and conditions of the contract executed between APTIM Environmental & Infrastructure, LLC and Florida Department of Environmental Protection No. HW689. APTIM will begin the proposed scope of work upon issuance of the Task Assignment Notification.
