Document Summary
This Task Assignment Change Order (TACO #1) amends the original Task Assignment DC024C under Contract HW689 between the State of Florida Department of Environmental Protection (FDEP) and APTIM Environmental & Infrastructure, LLC. The change order addresses cost increases resulting from personnel scheduling conflicts that required team members to mobilize from a Winter Garden office location rather than the originally planned Tampa office.
The change order increases the total task value from $100,556.56 to $103,952.56—an increase of $3,396.00—to cover additional travel expenses including hotel accommodations and per diem for affected personnel. The contract scope involves site assessment activities at the former Southgate Cleaners facility (ERIC_5228) located at 5317 U.S. Highway 19, New Port Richey, Pasco County, Florida, including modified active gas sampling, soil sampling, direct push technology groundwater sampling, and well installations. The task completion date has been extended from April 11, 2025, to July 18, 2025 (98 days total), with the final deliverable due date extended from February 11, 2025, to May 15, 2025. The amended price is structured as a fixed unit rate, with invoicing tied to FDEP approval of specific deliverables: Field Notes Package & Analytical Results ($68,009.02), Boring Log & Field Notes ($15,506.49), and Site Assessment Report & Project Management ($20,437.05).

SUBJECT: Negotiation Summary
HW689 DC024C CO1
DATE: March 20, 2025
This Change Order reconciles WCD #1, which increases travel expenses for the site assessment work at Southgate Cleaners. Due to scheduling conflicts, several team members from outside of the nearby Tampa office have been assigned to this project. For those personnel, travel expenses such as lodging and per diem have been added to the proposed cost.
Total task amount has increased by $3,396.00. The original price was $100,556.56 and the amended price is $103,952.56. APTIM’s time, labor, and subcontractor costs for these activities have been reviewed and are appropriate. The level-of-effort and cost estimate are reasonable for the scope of services. Final deliverable due date has been extended from February 11, 2025 to May 15, 2025.
Task completion date has been extended from April 11, 2025 to July 10, 2025.

Proposed Scope of Work Southgate Cleaners (Former) 5317 U.S. Highway 19 (Currently 5325 U.S. Hwy 19) New Port Richey, Pasco County, FL 34652 Site Assessment and Reporting Task Assignment DC024C TACO #1 ERIC_5228 (Former Facility No. 519500354) Aptim Environmental & Infrastructure, LLC March 20, 2025
The following proposed scope of work for DC024C TACO #1 is to resolve WCD #1, approved for the former Southgate Cleaners site located in New Port Richey, Pasco County, Florida. WCD #1 was approved to allow for additional costs after several personnel changes were required during the performance of assessment activities, which will require travel expenses because some personnel were mobilizing from a different office than the proposed nearby Tampa office.
Subtask 1-Project Management
Project management hours are calculated based on the labor hours proposed in the scope of work.
Subtask 2-Preliminary Site Activities and Modified Active Gas Sampling Changes in personnel required additional travel expenses including hotel and per diem as shown on the attached Subtask 2 Level of Effort (LOE) spreadsheet.
Subtask 3-Soil Sampling & GW DPT Sampling Field Activities Changes in personnel for the soil sampling and DPT groundwater sampling required additional per diem and hotel travel expenses, as shown on the attached Subtask 3 LOE spreadsheet.
Subtask 4-Well Installations Changes in personnel for the well installation oversight will likely additional per diem and hotel travel expenses as shown on the attached LOE for Subtask 4.
WCD #1 was approved for a total of $3,400 and is being resolved for a total of $3,396.00.
Field activities for Subtask 2 and the soil sampling and drilling in Subtask 3 have been performed, and boring logs and preliminary reporting have been completed. Based on the findings, the single well installation requested by the FDEP, based on the findings, is scheduled to be installed on Friday, March 21st and sampled early next week, additional time is required to review the findings of the groundwater analytical and determine if additional work will be required. The FDEP requested that a single well be installed and sampled at the area previously demonstrating a TCE detection to determine if the assessment can be competed with a single well. Currently the Site Assessment Reporting was due February 14, 2025. A 90-day extension for the completion of the SAR is requested, to May 15, 2025, which should allow for time for the FDEP to review the results and determine the best path forward. The current TA Completion Date of April 11, 2025, will be extended to July 10, 2025.
APTIM proposes to perform this scope of work in accordance with the terms and conditions of the contract executed between APTIM Environmental & Infrastructure, LLC and Florida Department of Environmental Protection No. HW689. APTIM will begin the proposed scope of work upon issuance of the Task Assignment Notification.
