About this file

This document is a Task Assignment Change Order (TACO #2) issued by the State of Florida Department of Environmental Protection (FDEP) to APTIM Environmental & Infrastructure, LLC for environmental remediation services at the Blue Ribbon Cleaners (former) site located at 160 South U.S. Highway 17-92 in Longwood, Seminole County, Florida. The change order amends the existing Task Assignment (TA) DC009C under Contract HW689, dated December 21, 2021. The project is identified by ERIC_5462 and involves groundwater monitoring and remediation activities. The department's project manager is Alyssa Lamb (later Nicole Knutson), and the contractor's project manager is Eric Kramer.

The change order reduces the total task amount from $29,361.53 to $27,655.85, a decrease of $1,705.68, reflecting the removal of Industrial Waste Disposal (IDW) treatment and disposal costs from Subtask 5 and a reduction in sampling and reporting scope for Subtasks 6 and 7. Specifically, the IDW buckets generated during Quarter 2 sampling could not be located for disposal, and at FDEP's request, the sampling of the downgradient well MW009 was removed from Quarters 3 and 4, with reporting reduced to email summaries. The amended task cost is $27,655.85 on a fixed unit rate basis, with an extended completion date of August 28, 2026. Deliverables include quarterly PARM sampling reports with due dates ranging from November 28, 2025 to May 29, 2026, with invoicing structured upon FDEP approval of each subtask deliverable.

The following is the proposed scope of work for Task Assignment Change Order (TACO) DC009C TACO #2 to reconcile Subtask 5 and adjust the future scope of the Task Assignment

(TA).

Subtask - 1: Project Management This task includes project management activities associated with the completion of the following Subtasks, as calculated in accordance with Contract HW689.

Subtask - 2: Quarter 1 Monitoring Well Sampling – VOHs: Previously Reconciled Subtask - 3: Quarter 1/Annual Groundwater Reporting (VOHs) and Noticing Documents

Preparation: Previously Reconciliation Subtask - 4: Quarter 1 Additional PFAS Sampling and Reporting: Previously Reconciled

Subtask - 5: Quarter 2 Monitoring Well Sampling (VOHs) and Reporting:

The two (2) buckets generated during the quarter 2 sampling event could not be located by the subcontracted IDW transportation and disposal company requested to remove the IDW waste, therefore the drum disposal has been removed from the Subtask. The adjustment reconciles Subtask 5.

Subtask - 6: Quarter 3 Monitoring Well Sampling (VOHs) and Reporting:

At the request of the FDEP (see attached email), the quarterly 3 sampling scope has been reduced to remove the sampling of the downgradient well MW009. In addition, the reporting has also been reduced to the preparation and submittal of an email to include any recommendations for the site.

Subtask - 7: Quarter 4 Monitoring Well Sampling (VOHs) and Reporting:

The quarterly 4 sampling scope has also been reduced to remove the sampling of the downgradient well MW009 and change the reporting.

Subtask - 8: Additional Annual and SRCR Reporting: No Changes

No deliverable or cost to completion extensions should be necessary at this time.

APTIM proposes to perform this scope of work in accordance with the terms and conditions of the Contract between the Florida Department of Environmental Protection and APTIM dated December 21, 2021 (DEP Contract No. HW689). APTIM will begin the proposed scope of work upon issuance of the Task Assignment Notification.
