Proposed Scope of Work Clothes Doctor Dry Cleaners 13915 US Hwy 98 Bypass, Dade City, FL ERIC_5246 (Former Facility No. 519502401) Remedial System Decommissioning Task Assignment DC(TBD)A APTIM Environmental & Infrastructure, LLC June 15, 2023

Following review of the recommendations provided in APTIM’s Remedial Action Status Quarterly Report – September to November 2021 dated February 2022, the FDEP approved the removal of the SVE system and remedial system infrastructure at the site on March 10, 2022.

APTIM has prepared the following Scope of Work and LOE to remove the aboveground portions of the Clothes Doctor Cleaners remediation system. The proposed scope of work includes removal of the SVE remediation system components existing aboveground and decommissioning the soil vapor extraction (SVE) system housed in a shed. APTIM proposes the SVE remedial be transferred to the FDEP Lakeland Storage Yard for later use if needed on another State site. In general, the scope of work includes:

1) Remove electrical power attached to the remedial system, meter can and pole (subcontracted to licensed electrical contractor).

2) Detach piping from system remedial system, excavate around stubups next to trailer, cap pipes and fill excavation to match existing surface grade materials to match surrounding elevation

3) Remove manholes at VE-1 through VE08, excavate around pipe and cut below grade. Tremie fill SVE wells with grout, concrete around pipes to match existing grade. Replace floor tile with spare tiles, as applicable

4) Subcontract the removal of vapor GAC for offsite disposal to a licensed facility

5) Disconnect hurricane ties and remove system trailer from work area for transport to

FDEP’s Lakeland storage yard

6) General cleanup miscellaneous trash and sweep area

7) Perform a final walk though and complete any necessary touchups

8) Document final site conditions using photo documentation and prepare report summarizing field activities

All construction debris will be removed from the site and transported to an approved waste collection and/or waste recycling facility for proper disposal. The SVE system removal activities will be documented in a Remediation System Decommissioning letter report, which will include photo documentation of the equipment and site conditions before and after removal of the system, and a revised Property Reporting Form to designate location of equipment.

The deliverables for this task assignment will include:

1. System Decommissioning by August 18, 2023)

2. Completion of Remediation System Decommissioning Letter Report and revised

Property Reporting Form (August 31, 2023)

3. Task Assignment expiration (October 20, 2023)

APTIM proposes to perform this scope of work in accordance with the terms and conditions of the contract No. HW689 executed on December 21, 2021 between the Florida Department of Environmental Protection and APTIM Environmental & Infrastructure, LLC. APTIM will begin the proposed scope of work upon written authorization.

According to the Pasco County Property Appraisers office records, the site continues to be owned by Realsouth Investments LLC, which was previously owned by Claude Sanderson. It is noted that the access agreement was signed by Claude Sanderson, now deceased (January 31, 2022).

According to the Florida Division of Corporations, Realsouth Investments LLC is currently listed with a Registered Agent of Sheila Sanderson (also deceased on February 12, 2022). However, other authorized persons for the LLC are listed as Julie Ann Eynon, listed at 8884 Random Road, Fort Worth, Texas 76179 and Pamela Annette Beasley, located at PO Box 1322, Dade City, Florida 33526 (believed to be the daughters of Claude and Sheila Sanderson). Since the 2017 FDEP Permission to Enter Property was not signed by a current listed officer or authorized person for Realsouth Investments LLC, an updated access agreement needs to be requested and approved.

If you have any questions, please contact APTIM for clarification.
