About this file

This document consists of two Task Assignment Change Orders (TACOs) executed between the State of Florida Department of Environmental Protection (FDEP) and APTIM Environmental & Infrastructure, LLC for remedial system operation and maintenance services at the former Carmon's Cleaners facility located at 7980 North Tamiami Trail in Sarasota, Florida. The contract number is HW689, with project designation ERIC_4903. FDEP's project managers are listed as Alyssa Lamb and Nicole Knutson, while APTIM's project manager is Eric Kramer. The first change order (TACO #1) was executed on June 17, 2025, and the second change order (TACO #2) was executed on February 3, 2026.

TACO #1 reconciles the authorization for APTIM to update the telecommunication component of the remedial system, increasing the task value from $54,495.97 to $58,948.78 with a completion date extended from January 16, 2026 to April 30, 2026 (104 total days). TACO #2 adds three months of operation and maintenance services during remedial system shutdown procedures and adjusts costs for reduced groundwater sampling due to one inaccessible monitoring well, bringing the total task value to $67,224.31 with a new completion date of July 17, 2026 (78 days from the prior completion date). Services include quarterly operations and maintenance reporting, groundwater sampling, vapor sampling and analysis, system shutdown evaluation, utility costs ($4,434.00 total), and professional project management. The invoicing schedule spans from May 30, 2025 through April 24, 2026, with payments contingent upon FDEP approval of submitted reports and receipts of paid utility invoices.

Proposed Scope of Work Former Carmon’s Cleaners 7980 North Tamiami Trail, Sarasota, FL Remedial System Operation & Maintenance Task Assignment DC005D TACO #2 ERIC_4903 (Former Facility ID No. 419700833) Aptim Environmental & Infrastructure, LLC (APTIM) January 21, 2026

Aptim Environmental & Infrastructure, LLC (APTIM) submits this Task Assignment Change Order (TACO) to add three months of O&M services during remedial system shut down procedures.

Subtask 2: No scope or budget changes are proposed for this subtask.

Subtask 3: No scope or budget changes are proposed for this subtask.

Subtask 4: No scope or budget changes are proposed for this subtask.

Subtask 5: No scope or budget changes are proposed for this subtask.

Subtask 6: No scope or budget changes are proposed for this subtask.

Subtask 7: APTIM proposes to add three months to this subtask to allow the remedial system to operate while implementing the shutdown procedure. During the first month period APTIM will mobilize to the site to collect vapor samples from VE01, VE02, VE03 and the combined vapor influent to evaluate if shutdown of the remedial system is appropriate. The system will remain in operation and monthly O&M services will be performed at the site. If remedial system shutdown is appropriate, then APTIM will travel to the site and perform an O&M service and shut down the system for one month. After a one-month duration the system will be restarted and vapor samples collected after one pore volume is extracted from each vapor extraction well. After vapor sample collection the system will remain off. APTIM will compile the vapor analytical data and submit a letter report to provide tables, graphs and recommendations on the operation of the remedial system operation.

Subtask 8: No scope or budget changes are proposed for this subtask.

Subtask 9: Annual Groundwater Sampling Monitoring well MW-1 is located within the building and could not be sampled during the recent groundwater sampling event. The former closed restaurant, in the space of the former drycleaner, was under renovations upon arrival to the site and the well was not accessible. The site was revisited a few days later, after access was granted to attempt to collect the interior sample, however the well was found to be grouted closed (tile grout). Attempts to grind out the grout and remove the manhole lid were unsuccessful. It is apparent that the well manhole lid will need to be broken to access the well. Without a replacement lid available, the repair will need to be delayed to avoid leaving an open manhole in the kitchen of the restaurant. Since the well could not be sampled, the costs associated with the analysis is being removed from the

Subtask. Additionally, a poly drum is being used for storage of IDW, therefore the subtask was changed to adjust the drum costs.

Subtask 10: No scope or budget changes are proposed for this subtask.

A schedule of the proposed O&M, groundwater sampling, and reporting activities follows:

Vapor sample collection 2/15/26

Confirmation vapor sample collection 3/27/26

Letter report submittal 4/24/26

TA completion Due Date 6/19/26

APTIM proposes to perform this scope of work in accordance with the terms and conditions of the Contract No. HW689 that was executed December 21, 2021 between Florida Department of Environmental Protection and APTIM Environmental & Infrastructure, LLC. If you have any questions, please contact us for clarification.
