DATE: 3/17/23
This TA authorizes Terracon’s costs for site assessment. The total cost of this TA is $296,831.54 and this entire cost is fixed unit rate. This TA was negotiated - the total cost is $71,051.89 or 19% less than originally proposed.
This site assessment TA includes the use of MiHPT to map a large PCE plume containing high concentrations and potential DNAPL. This technology is relatively expensive but it being authorized for this TA to evaluate Terracon’s assertion that it is more effective and cost efficient overall, with fewer site mobilizations to get the required data. Three bids were requested for each subcontracted task element except surveying. Site access was confirmed current.
Information on the contents of this TA package follows.
• Terracon provided Competitive Bid Worksheets (CBWs) for each subcontracted task element. The CBWs are located immediately following Terracon’s proposal letter.
• Subcontractor bid documentation follows the CBWs. This documentation appears in the order the subcontractors participate in site assessment, starting with LOE2 and progressing through LOE5. And this documentation is discussed in that order below.
o The third utility location bidder, GPRS, was non-responsive to Terracon’s request for bid. The SurvTech surveying bid included utility location, so they serve as a third utility location bidder. The cost of the utility portion of ServTech’s bid is removed to arrive at their surveying cost.
o The third coring bidder, Florida State Concrete Coring, was non-responsive as documented via the email record.
o The third drilling bidder, Drill Pro, was non-responsive as documented via the email to Todd Hodgson. Drilling bids are provided via the Terracon Drilling Spec Sheet – Terracon completes the job specs and the bidder makes the associated cost entries and returns the sheet as their bid. The low-bid/chosen drilling subcontractor’s costs are split between ST2 and ST3 according to which drilling task elements apply to each subtask.
o The third MiHPT bidder, Dakota, was nonresponsive as documented via the email record. The Cascade MiHPT bid includes drilling. Both bidders included drilling in their initial bid request. Terracon was directed by the WSCS PM to sub the drilling because their handling cost was less than the driller’s premiums for subcontracting. Columbia resubmitted a bid without drilling but Cascade did not, so Terracon subtracted the amount Cascade charged for drilling to arrive at their MiPHT bid. MiPHT costs are summarized in the Competitive Bid Worksheet. (Cascade’s bid also includes mobilization to the wrong destination which may have increased the mob costs they quoted, but a reduction [or elimination] of this cost would not bring their total below that of Columbia’s.)
Note - the Soil Investigation section of Terracon proposal letter should refer to 6 soil boring locations (rather than 3) in addition to soil samples to be collected from 18 MAGS locations. The CEF-LOE correctly accounts for 6 additional soil boring locations.

Proposal for Site Assessment Workplan Implementation Miller Machinery & Supply Company #2, ERIC_4733 ■ Tampa, FL March 9, 2023 ■ Terracon Proposal No. 4227492
SCOPE OF SERVICES
Subtask 1 – Project Management
Dan Nedvidek, P.G. will serve as the project manager for this site. Donna Cline, P.E., will serve as the Authorized Project Reviewer.
Subtask 2 - Soil Sampling, Interior Well Installation, Modified Active Gas Sampling (MAGS), and Assessment/Sampling of Existing Monitoring Wells
Terracon will prepare health and safety plan. Pre-Task Planning will also be performed to prepare for field investigations.
Utility Locate Prior to conducting the any subsurface activities, Terracon will subcontract GeoTek to provide a subsurface geophysical survey to identify potential subsurface utilities or obstructions. A 10-foot by 10-foot box will be white lined around each boring location with utilities marked within that boundary, to allow for alternative location in case of hand auger obstruction. This scope of work is estimated to take two (2) full days to complete.
Due to the presence of reinforced concrete across much of the project site, Terracon will subcontract concrete coring services for the MAGS well installation to True Line Coring and Cutting. It is anticipated that this scope of work will take two days to complete. Concrete coring will happen concurrently with interior and on-site utility clearance of boring locations to minimize mobilization charges to FDEP.
Soil Investigation Terracon will collect soil samples via hand-auger and DPT from 3 proposed soil boring locations (only) and from the 18 proposed MAGS locations as described in SAWP (Terracon 2022). Terracon will collect soil samples from 0.5, 2, 4, 6, 8 and 10 feet (ft) below ground surface (BGS), or to the top of the water table, via hand auger and/or from the DPT soil samples at each soil boring location.
Terracon has contracted with Action Environmental to perform the soil boring and MAGS well installation. One shallow well will be installed in the interior of the facility near the suspected source zone. Terracon personnel will field screen the soils with an OVA-PID, document the lithology observed at each of these soil boring locations, and will record the Global Positioning System (GPS) coordinates of completed borings. Based on the number of proposed boring locations and depth intervals, Terracon will collect up to 138 soil samples for laboratory analysis of Volatile Organic Halocarbons (VOH) and five samples for Volatile Organic Aromatics (VOA) via EPA Method 8260. The approximate sampling locations, depth intervals, matrices, analytes and laboratory methods, rationale, and screening criteria are summarized in the SAWP (Terracon, 2022). The soil borings will be tremie- grouted with neat cement / bentonite mixture. MAGS sampling points will be constructed of 2” PVC and screened per FDEP requirements, and as detailed in the SAWP. Surface finishes will consist of a flush-mounted PVC cleanout ports with screw-type lids.
Modified Active Gas Survey (MAGS) The MAGS test will be performed over a period of 3.5 days. One air sample will be collected via Summa cannister at each MAGS location and analyzed for TO-15.
Assessment/Sampling of Existing Monitoring Wells Terracon will assess and sample (if possible) up to 12 existing on-site and off-site monitoring wells to determine if additional wells will be required at a later date. Each assessment will consist of a visual inspection of the well structure, collection of total depth and depth to water measurements, and 30 minutes of well development. Following the development task, Terracon will wait no less than 24 hours
March 9, 2023 ■ Terracon Proposal No. 4227492 before returning to the site to collect groundwater samples from wells deemed viable. Samples will be analyzed for VOH/VOA via EPA Method 8260.
Subtask 3 – Membrane Interface Probe (MiHPT) Assessment, Confirmatory Soil and Groundwater Sampling
MiHPT Assessment MiHPT assessment data will be collected from ground surface to 30 feet BGS at approximately 70 on-site and off-site locations. Terrracon has selected Columbia Technologies/Action Environmental to collect MiHPT data and provide DPT services. Terracon personnel will remain on-site for the duration of the MiHPT data collection to provide real time interpretation of the data to ensure efficient data collection decisions are made. Data will be reviewed in real time and a summary posted to the Client Portal the following morning. A senior geologist will remotely review the data and verify that quality control metrics are within range for all data. Three-dimensional modeling will be updated throughout the project ensure the conceptual site model reflects current site conditions. In addition to VOC data, the MiHPT also provides hydraulic profiling information that can be used to roughly correlate probe pressures to changes in lithology. This scope of work is estimated to take approximately 15 days to complete.
DPT Groundwater Profile Assessment and Soil Sampling Groundwater profiling and soil sampling will be conducted at approximately 10 MiHPT locations to confirm the presence of VOCs and correlate hydraulic profiling data to observed lithology. Terracon anticipates collecting three samples from each location. Generally, samples are taken from areas of high, medium, and low MIP response to better define model boundaries. Samples will be analyzed for VOH/VOA via EPA Method 8260. Additional details are summarized in the SAWP (Terracon, 2022).
Subtask 4 –Boundary and Property Survey
Terracon will subcontract PLS services from SurvTech for site boundaries, structures, wells, soil borings, groundwater profiling locations, and MiHPT borings located on-site and offsite.
Subtask 5 –Investigation Derived Waste
Decontamination activities will take place at a designated staging and laydown area. The soil and water IDW will be containerized in 55-gallon drums and staged on-Site at a designated location. It is estimated that 8 drums of IDW will be generated in Subtasks 2 and 3 and the transportation and disposal costs for the drums are included under Subtask 5.
Terracon has selected Erwin Remediation, Inc. (Erwin) for IDW transportation and disposal. Terracon will provide oversight of the IDW pickup, and sign waste manifests as an agent of FDEP.
Subtask 6 –Site Assessment Report
Terracon will prepare a Site Assessment Report that will detail and summarize the groundwater sampling activities, field observations, and results of soil and groundwater analysis and will include data tables and exhibits of groundwater flow and soil and groundwater concentrations. Conclusions and recommendations will include a summary of results and recommendations for future sampling or remedial activities. Recommendations regarding the number and location of additional monitoring wells
March 9, 2023 ■ Terracon Proposal No. 4227492 will be discussed in this report as necessary. Installation of additional monitoring wells will be covered under a separate Task Assignment.
SCHEDULE
It is estimated that the field activities outlined in this proposal will be completed over an approximately 30-day period. Terracon will strive to conduct activities concurrently when personnel scheduling and safety allow. Daily updates of activities will be uploaded within 24 hours of completion on the Compass website to allow for FDEP review of site data. Terracon anticipates the deliverable of site assessment activities within 30 business days of receipt of analytical results.
CLOSING REMARKS
Terracon services will be conducted in accordance with the terms and conditions contained within DEP Waste Cleanup Program (WCP) Contract No. HW687, executed January 5, 2022. It is understood that a Task Assignment will be issued to us to authorize us to perform this work.

RE: Utility Locating Services – 80 Bore Locations, 1920 Tampa East Blvd.
Mr. Nedvidek, GeoTek Services is pleased to respond to your request for Utility Locating services at 1920 Tampa East Blvd. located in Tampa, Florida. We will scan up to a 10’ x 10’ area around the 80 proposed bore locations with the goal of locating underground utilities.
We can only scan the easily accessible open areas for utilities. Note that we cannot scan beneath fixed objects such as landscaping, fencing, buildings. We will scan as close as reasonably possible to fixed objects such as walls, landscaping, fences, etc.
Our deliverable will be crayon, marking paint or flags indicating the location of potential underground utilities. It is the client’s responsibility to protect and preserve all marks.
We will use both Electromagnetic Induction instruments as well as Ground Penetrating Radar (GPR) to detect objects below the surface. Due to the nature of sub-surface detection and the physical laws governing radio wave propagation, some buried objects may not be detectable due to depth, obscuration, or a weak radar signature. GeoTek Services will use industry standard best practices to locate sub-surface objects, however some objects may not be detectable. Geotek Services does not guarantee that all underground objects can be found. GeoTek Services assumes no liability for any damages.
We request access to any as-built site plans of the scan areas prior to rendering services. Our fixed price quote for this effort is $2,950.00. No out-of-pocket expenses are expected for this project. This quote is valid for sixty days.

Proposal
This document shall serve as a proposal for work to be performed by GeoView, Inc. (GeoView) for Terracon.
Description of Site
The project site is referred to as the Miller Machinery and Supply Company No. 2 and is located at 1920 Tampa East Boulevard in Tampa, Florida. The purpose of the geophysical survey will be to determine the location of underground utilities within a 5 to 10 feet radius of 80 proposed borings. The location of the study area is provided on Figure 1.
Scope of Work
The geophysical survey will be conducted using ground penetrating radar (GPR) and conventional electromagnetic (EM) pipe and cable locators. The GPR survey will be conducted using either a Mala and/or a GSSI GPR system. The appropriate GPR antenna configuration and time range and processing parameters will be determined in the field by the project manager.
The EM survey will be performed using a Subsite 95R and/or a Radiodetection RD (8000, 8100 or 8200) system. As appropriate, the EM survey will be conducted using both passive and active modes. The passive mode is used to find cables and conduits carrying live power (50-60 Hz) and communication (29-33Hz) signals. For the active mode survey, a current is transmitted directly or broadcast into using electromagnetic induction a metal pipe or conduit and the path of the pipe/conduit is identified by following the signal with a portable receiver.
The active mode is used to trace out cast iron pipes, metal water and sewer lines, utilities that have trace wires, and inactive power and communication lines.
Presentation of Results
The location and estimated depths of underground utilities or any other identified structures shall be indicated on the ground surface using spray paint or other methods as specified by Terracon. No report shall be prepared.
Compensation
It is estimated the scanning will take 2.5 days to complete. Cost to complete the survey will be 2 to 3 days at $1200 per day (not to exceed $3600). This price is inclusive of all charges associated with the project. Unless otherwise agreed upon, GeoView shall be compensated for all services within 30 days of invoice date.
GeoView shall be paid in full regardless of whether the results of the geophysical survey are what Terracon anticipated. If it is determined during the survey that the geophysical survey will not achieve the objectives of the project, Terracon will immediately be notified. If a decision is made to discontinue the survey, only charges for time and materials costs to that point will be submitted.
Requirements of Client
Terracon will provide a scaled map and digital file, if available, of the project site showing the boundaries of the project site, areas of specific concern and pertinent landmarks. Terracon will also supply all available information regarding the presence and history of underground utilities at the project site (see limitations for further discussion). Unless otherwise requested, Terracon will be responsible for the coordination of site access, traffic control, clearing of onsite obstructions or any other logistical consideration necessary to conduct the survey.
General Description of Each of The Geophysical Methods
Ground Penetrating Radar (GPR) The GPR survey will be conducted using a Mala or GSSI GPR system. The survey will be conducted with either a 500, 400 or 250 mega-Hertz antenna. The appropriate antenna will be determined in the field by the project manager. GPR will be able to provide both the position and estimated depth of both metallic and non-metallic underground utilities.
The ability of GPR to resolve a particular utility will be controlled by the following factors: 1) Depth of penetration of the GPR signal, 2) Depth of burial and diameter of the underground utility and 3) Electrical contract between the underground utility and the surrounding soils and 4) The complexity of the utility layout at the site. Due to one or a combination of these factors the GPR may not be able to identify all the utilities at the site. The diameter or composition of utilities cannot be accurately determined using GPR, though can be estimated. Estimates of burial depth are typically accurate to within +/- 20 percent, more accurate depths are possible if invert elevations for some on-site utilities are known.
Conventional Electromagnetic Pipe and Cable Locators
A Radiodetection System is used to locate underground utilities using conventional means. The system consists of a dual-function receiver and transmitter. The receiver can be operated in two modes: active and passive. In the passive mode the receiver detects the presence of underground utilities that are energized by a 60-Hertz or other known frequency. These utilities can either be those actually carrying electrical power or those utilities that are both metallic and sufficiently close to the electrical lines to have an electrical field induced within them.
In the active mode, the transmitter deliberately induces an electrical current into the utility. The frequency of this field can be varied from 512 Hertz to 200 kilohertz. The electrical field is induced using a transmitter which is either directly attached to the utility, placed on or above the utility or attached to an induction coil which is placed around the utility. Depths of underground utilities can be determined when the EM equipment is being used in the active mode. Depths are typically accurate to within 10-15 percent of the total utility depth.
Limitations
GeoView shall only be liable for the successful locating of underground utilities or any other under ground structures whose presence is both known and indicated on site maps to be provided prior to the performance of the geophysical survey. Every attempt will be made to locate both known and unknown utilities within the survey area. However, GeoView will only be liable for utilities whose presence is known prior to the survey.
The objective of the geophysical survey is to determine the presence and configuration of underground utilities. However, the geophysical response of these features may be highly complex depending upon on the physical characteristics of the underground utilities, the electrical properties of the surrounding soils, the depth of burial of the utilities, and the proximity of the utilities to each other.
GeoView shall conduct the geophysical survey using the most “up-to-date” geophysical equipment in a manner consistent with the level of care and skill ordinarily exercised by members of the geophysical profession practicing in the same locality under similar conditions. GeoView’s utility locating personnel are all certified GPR and conventional pipe and cable locator operators with extensive in-house training.
When Terracon is working around indicated utilities, a minimum of 2 feet clearance from either side of the marked utility will be maintained.
