About this file

This document is a Task Assignment Change Order (TACO #1) entered into between the State of Florida Department of Environmental Protection (FDEP) and Terracon Consultants, Inc. for site assessment activities at the 2402 Market Street Project (Site ID: ERIC_18247) in Jacksonville, Florida. The change order modifies the original Task Assignment (TA BF04A) under Contract HW687, with Megan Johnson serving as the FDEP Project Manager and Ralph Mongillo Jr., P.G. as Terracon's Project Manager.

The change order settles Work Change Directives (WCD) #1 and #2 issued in September and October 2025, increasing the task value from $50,052.06 to $50,227.18, representing a total cost increase of $175.12. Key modifications include subcontractor costs for FDOT signage and cones setup/pickup ($1,235.85 increase in Subtask 3), City of Jacksonville permit fees for revocable permits and right-of-way access, and SPLP barium analysis ($45.00 plus $13.00 handling fee in Subtask 2). Changes in field staffing resulted in minor cost adjustments, and two 0.5-foot soil samples were removed from the scope, offsetting some expenses. The task completion date was extended from November 7, 2025 to February 27, 2026, allowing 112 additional days for completion. Invoice schedules remain tied to departmental approval of deliverables across five subtasks: project management, QAPP/work plan updates, field preparation, groundwater sampling, and the final site assessment report due by December 12, 2025.

SUBJECT: Negotiation Summary – SRP28 Task Assignment (TA) Change Order #1 – TA BF04A CO1 2402 Market Street Project - Site ID #: ERIC_18247 Site Assessment Activities

This Task Assignment Change Order (TACO) settles Work Change Directives (WCD) #1 and #2, issued on September 12, 2025, and October 10, 2025, respectively. These WCD authorized a subcontractor to set up and pick up Florida Department of Transportation Index 602 signage and cones, a City of Jacksonville permit fee, and sample analysis for SPLP extraction for Barium.

Specific changes are detailed below:

Subtask 1: Project Management Under this subtask there was a decrease in project management hours due to a change in field staff under Subtask 2. This change resulted in a cost actual decrease of ($16.35).

Subtask 2: Update QAPP/Work Plan and HASP The changes under this subtask include Avery Bliss performing the field work in place of Tyler Covert, a decrease in two 0.5-foot soil samples located inside of the building beneath the concrete floor slab, the additional costs for the right of way and revocable permits authorized in WCD #1, and the SPLP barium analysis authorized in WCD #2. These changes resulted in an overall cost actual decrease of ($1,044.38).

Subtask 3: Field Preparation and Off-Site Access Under this subtask there were additional costs authorized under WCD #1 for the well installation traffic signage and cones. This change resulted in a cost actual increase of $1,235.85.

Subtask 4: Field Work There are no scope or cost changes associated with this Subtask.

Subtask 5: Site Assessment Report Addendum There are no scope or cost changes associated with this Subtask.

Negotiation Summary – Task Assignment Change Order #1 TA BF04A CO1 2402 Market Street Project – Site ID # ERIC_18247 October 15, 2025

Summary The Department’s SRP Contract Manager and Terracon’s Contract Manager have both agreed to the above changes. All changes noted above represent a total overall cost increase of $175.12, as shown in the Final/Cost Actual LOE. Due to a rounding error in Excel, the Delta LOE states an overall cost increase of $175.11, however this was addressed in the Cost Actual LOE. The changes were noted in the TACO Request received from Terracon on October 13, 2025. The final cost associated with this TA increased from $50,052.06 to $50,227.18. Documentation of the changes is included in the attached TACO Request, the Delta LOE spreadsheet and the Cost Actual LOE spreadsheet, and the TACO form for BF04A CO1 prepared by the Department.

EXTERNAL MESSAGE

This email originated outside of DEP. Please use caution when opening attachments, clicking links, or responding to this email.

Megan and Darrin, Terracon Consultants, Inc. (Terracon) is pleased to submit TACO#1 for Subtasks 2 and 3 including WCD#1 and WCD #2 to the Florida Department of Environmental Program (FDEP) Waste Cleanup Section for the 2402 Market Street Property located at 2402 Market Street in Jacksonville, Florida. Field work was completed on September 29 and 30 and October 6, 2025.

The Subtask 2 deliverable was previously submitted t the FDEP. Several changes of note are presented below:

Subtask 2

Avery Bliss performed field work in place of Tyler Covert.
