Task Assignment HW687 HW007A

Terracon Proposal No. PHF227013

Dear Mr. Cox:

Terracon Consultants, Inc. (Terracon) appreciates the opportunity to submit this proposal to the Florida Department of Environmental Program (FDEP) Waste Cleanup Program to conduct Annual Groundwater Monitoring and Reporting for the Jay Lee Cleaners

(ERIC_5542) facility located at 119 Railroad Avenue in Chipley, Washington County, Florida.

Scope of Services

◼ Field preparation, mobilize to site, perform groundwater sampling and laboratory analysis of ten (10) shallow monitoring wells (MW002, MW003, MW004, MW006, MW008, MW009, MW010, MW014, MW015 and MW017) and four (4) Floridan

Aquifer monitoring wells (MW012, MW013, MW016 and MW024)

◼ Review data and prepare an Annual Groundwater Monitoring

Report including updated tables and groundwater flow and concentration exhibits.

See attached LOE for details mailto:Dean.Cox@FloridaDep.gov

Proposal for Annual Groundwater Monitoring and Preparation of Report

Jay Lee Cleaners, ERIC_5542 ■ Miami, FL

April 29, 2022 ■ Terracon Proposal No. PHF227013

Schedule Deliverable Annual Groundwater Monitoring Report within 20 business days following receipt of laboratory results.

Compensation See attached CEF for details.

BASIS FOR SCOPE OF SERVICES

Last sampling event on May 6 and 7 of 2020, Tetra Tech sampled only shallow monitoring wells MW003, MW004, MW007, MW009, MW015 and Floridan Aquifer monitoring wells

MW012, MW016, and MW024.

The critical issue for this site is the location of a potable/municipal well within ¼ mile of the site (municipal well #002, 174 feet deep). Also, of concern is the general increasing PCE concentrations observed in the vertical extent Floridan Aquifer well MW024.

Terracon recommends collecting samples and depths to water at ten (10) shallow monitoring wells (generally screened 9-29 feet bgs) and four (4) deep monitoring wells, (generally screened 50-70 feet bgs or 60-70 ft bgs). A brief justification for sampling the selected monitoring wells are described as follows:

Shallow Wells

▪ MW002, not sampled since 11/2007, PCE concentration of 200 ug/L

▪ MW003, continuation

▪ MW004, continuation

▪ MW006, not sampled since 2007, PCE concentrations of 84 ug/L

▪ MW008, not sampled since 2018, verify non detect PCE, only other sample result is 150 ug/L

▪ MW009, continuation, former source well

▪ MW010, not sampled since 2017, source well

▪ MW014, not sampled since 2017, only sampled twice, latest results 18 ug/L, midway downgradient well

▪ MW015, continuation, downgradient location, latest results below GCTL

▪ MW017, continuation, PCE concentration of 95.3 in 2020

Floridan Wells

▪ MW012, continuation, at source

▪ MW013, not sampled since 2009, midway downgradient well

Re: Non-Hazardous Drum Disposal – State of Florida

Mr. O’Neal:

The price for the transportation and disposal (T&D) of drums of non-hazardous soil, mud and water located in the State of Florida is listed below.

Non-Hazardous Chlorinated VOHs, Petroleum and Metals:

Description Rate Unit

T&D - Non-Haz Soil $ 100.00 Per Drum

T&D - Non-Haz Mud/Water $ 100.00 Per Drum

The prices listed are all inclusive without any additional fees or charges (fuel surcharge, low volume stop fee etc.) and are valid for all regions in the State of Florida except for the Florida

Keys. Pricing for sites located in the Florida Keys will be bid on a case-by-case basis dependent on the volume of drums and the location.
