Subject: Negotiation Summary
HW685_DC006B
This Task Assignment authorizes WSP to conduct Quarterly UIC and annual groundwater monitoring in accordance with the June 28, 2023, Scope of Work, Cost Estimate, and Bid Documentation. Since we could not task at the beginning of this fiscal year, the dates in the proposal were extended.
Subtask 1: Project Management An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.
Subtasks 2 - 5: Quarterly Ground Water Monitoring WSP will collect groundwater samples from MW001, MW003, and MW007R and analyze for total recoverable petroleum hydrocarbons (TRPH) using the FL-PRO Method. In addition, the groundwater sample collected from MW007R will be analyzed for dissolved gases (methane, ethane, and ethene) using Method RSK 174. A sample for chlorinated VOCs using EPA Method 8260 will be collected during the first quarter only because this data was not measured during the fourth quarter of the 2022 sampling event.
WSP will mobilize to the site on a quarterly basis and collect groundwater samples in accordance with the DEP-approved performance monitoring plan along with chlorinated VOCs.
Groundwater samples will be collected using low flow sampling methods. During the first three quarters groundwater samples will be collected for the UIC parameters from MW001, MW003, and MW007R for TRPH using the FL-PRO Method and methane, ethene, and ethane (MEE) for a sample collected from MW007R only (and chlorinated VOCs first quarter only). Groundwater samples collected during the fourth quarter will be analyzed for the UIC parameters from the three monitoring wells and chlorinated VOCs along with four additional wells MW004, MW008, MW009, and MW012 for chlorinated VOCs only. Investigation-derived waste (IDW) will be temporarily stored in a 55-gallon drum. WSP requested bids for IDW drum treatment and disposal from Erwin, US Ecology, and HEPACO. Erwin was selected as the lowest responsive and responsible bidder.
Subtask 3: Reporting WSP will submit an Annual Groundwater Monitoring to include a description of the sampling procedures, evaluation of the analytical results; comparison of current to historical data; tabulated summary of the current and historical analytical data; a site map showing the groundwater analytical results;; water level elevation information in tables and figures; laboratory analytical data; and conclusions, and recommendations.
WSP and the FDEP confirmed on August 21, 2023, that an Access Agreement with the current real property owner of the Touch of Class (ERIC_4521) site has been executed. The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost for this Task Assignment is $23,802.55 with 120 labor hours.
recommendations. Additionally, WSP will forward Automated Data Processing Tool (ADaPT) data deliverables provided by the laboratory for each groundwater sampling event.
The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment.
The final cost for this Task Assignment is $15,678.65 with 86 labor hours.

Scope of Work
General Concrete Scan: We understand the scope of work on this project is to scan for obstructions embedded in or just beneath a concrete slab. Per the details provided, our objective will be to detect and mark PT cables and/or conduit. The scan area should be laid out, marked, and cleared of obstructions prior to our arrival to avoid additional charges. Our findings will be marked directly onto the surface using marker when acceptable or other methods depending on the site conditions or as directed by the client. The depth of detected conduits and pipes will be marked when possible, but these measurements are only estimates. We do not recommend that you saw cut over the top of any marked conduits/pipes. GPR cannot determine the diameters of conduits/pipes so avoid all markings by at least 3” (or more if possible.)
Location Slab Type Square Footage Floor Slab on Grade 4 sq.ft.
Equipment Concrete Scanning GPR Antenna. This GPR Antenna is handheld and rolls over the surface. The device displays scan data on a screen, and the operator marks detected objects on the surface in realtime. The antenna needs a reasonably smooth, unobstructed surface for scanning and cannot scan within 2”4” of obstructions such as walls and metal tracks. Ideally, the client removes obstacles such as these before our work begins. The total effective scan depth can be as much as 18” or more with this antenna but can vary depending on the concrete conditions, composition, and other factors such as the spacing of the reinforcing. Depth accuracy depends on obtaining a precise depth calibration for the concrete. This device does not emit harmful radiation and can be safely operated while people are in close proximity. For more information, please visit Link
Electromagnetic Pipe Locator. This receiver can passively detect the signals from live AC power or radio signals traveling along some conductive utilities. Operators can connect a transmitter directly to accessible metallic pipes, risers, or tracer wires to generate a current at a specific frequency. The receiver can then detect the resulting signal along the pipe or tracer wire. Various factors may impact this device's effectiveness, including (but not limited to) access to the utility, conductivity, grounding, and interference from other utilities.
The depths provided should always be treated as estimates as their accuracy can be affected by multiple factors. General Terms & Conditions
This proposal is subject to the General Terms and Conditions for Services of Ground Penetrating Radar Systems, LLC (GPRS) posted at Link (the “Terms and Conditions”) and is hereby incorporated by reference into and made a part of this proposal. Customer acknowledges it has read and agrees to be bound by such Terms and Conditions. In the event of any conflict between the terms of this proposal and the Terms and Conditions, the Terms and Conditions will prevail. Customer also acknowledges that GPRS may, from time to time and at its discretion, modify the Terms and Conditions and Customer agrees to be bound by such Terms and Conditions as modified.
Customer agrees to meet and perform all requirements described in this document and has fully read and understands all items listed within this document including the proposalspecific Assumptions.
1.
It is the customer’s responsibility to prepare the site for scanning, including clearly identifying areas to be scanned, securing access to all areas required for scanning, and keeping these areas clear and free of obstructions. Delays caused by customer’s failure to do so may result in an increased price.
2.
GPRS does not conduct an investigation, analysis, or interpretation of soil composition, soil/concrete conditions, or geophysical, geological, engineering, or land surveying information. Customer acknowledges it understands that we are merely reporting retrieved data and that we do NOT provide geophysical, geological, engineering, or land surveying services. Customer should contact a professional in those fields if such services are needed.
3.
If any work to be performed is within a road or street, unless specifically included by GPRS within this proposal, it is the customer’s responsibility to provide adequate traffic control to allow GPRS’ personnel to safely and efficiently work in the road/street.
4.
If this proposal is not accepted within 90 days of June 20, 2023, the pricing may be subject to review.5.
If for some reason the technician arrives on site and the work is cancelled there will be a charge of $500 per requested technician.6.
If your project is in WV, SD, NM, or HI: State sales tax is not included in the total on this proposal, but will be included on the invoice.7.
Payment Terms are Net 30; or as specified if a current Master Service Agreement is in place.8.

Project-Specific Assumptions Standard fullday rates are based on an 8hour workday. Timeonsite in excess of 8hours will be billed at overtime rates.• This price assumes that we will be given access to perform the work during weekday business hours (6am5pm). Work performed outside of this is billed at time and a half.
All of our technicians have OSHA10 safety training or greater. Sitespecific safety training is not included in this quote. Please notify us if this project requires additional safety training.
Asbuilts and any other applicable drawings shall be made available to GPRS prior to the project.• The equipment listed in this proposal is the standard recommended equipment for your project. Depending on site conditions, your GPRS Project Manager may use all of the equipment listed or a subset thereof on your project. At their discretion, they may also employ additional tools not listed above to provide you with the best results possible.
A thorough utility search can only be completed if GPRS is given access to all utility structures, interior and exterior. This service is never a replacement for the use of the state One Call system (811).
If your project includes scanning an elevated concrete slab: Some slabs may not be completely penetrable from the top only and would need to be scanned from both top and bottom and should be considered an unforeseen circumstance that would incur additional time onsite and charges.
If your project includes scanning a wall or elevated concrete slab: Ladders, manlifts, or safe access for scanning wall or ceiling locations is the responsibility of the client.
If your project includes scanning a concrete slab with roofing material or other surface obstructions, these obstructions should be removed by the customer or customer's representative prior to our arrival. GPRS will need direct access to the surface of the concrete to obtain optimal results.
These rates assume that there are no certified payroll requirements. GPRS has not been notified of any PLA, DIR, or Certified Payroll requirements. If GPRS receives notice that any of these conditions exist, there will be additional costs.

RE: Utility Locating Services – Former Jay’s Cleaners in Jacksonville, Florida
Mr. Mongillo, GeoTek Services is pleased to respond to your request for Utility Locating services at the former Jay’s Cleaners located in Jacksonville, Florida. We will scan a 10’ x 10’ area around the one proposed bore location with the goal of locating underground utilities. We can only scan the easily accessible open areas for utilities. Note that we cannot scan beneath fixed objects such as landscaping, fencing, buildings. We will scan as close as reasonably possible to fixed objects such as walls, landscaping, fences, etc.
Our deliverable will be marking paint and/or flags indicating the location of potential underground utilities. It is the client’s responsibility to protect and preserve all marks.
We will use both Electromagnetic Induction instruments as well as Ground Penetrating Radar (GPR) to detect objects below the surface. Due to the nature of sub-surface detection and the physical laws governing radio wave propagation, some buried objects may not be detectable due to depth, obscuration, or a weak radar signature. GeoTek Services will use industry standard best practices to locate sub-surface objects, however some objects may not be detectable. Geotek Services does not guarantee that all underground objects can be found. GeoTek Services assumes no liability for any damages.
We request access to any as-built site plans of the scan areas prior to rendering services. Our fixed price quote for this effort is $750.00. No out-of-pocket expenses are expected for this project. This quote is valid for sixty days.

SUBTASKS 2 THROUGH 5 – QUARTERLY GROUNDWATER MONITORING
During the previous Interim Source Removal activities completed at the site which included the placement of food-grade and enriched emulsified vegetable oil, EOSPRO, manufactured by EOS Remediation, to the base of the excavation prior to backfilling to stimulate the enhanced bioremediation of any residual chlorinated solvents in the shallow zone of the surficial aquifer. To comply with the requirements of the DEP technology acceptance letter for EOSPRO and Underground Injection Control (UIC) monitoring requirements, WSP will continue to monitor UIC parameters on a quarterly basis.
The groundwater samples will be collected from MW001, MW003, and MW007R and analyzed for total recoverable petroleum hydrocarbons (TRPH) using the FL-PRO Method. The UIC and performance monitoring plan is presented in Table 1.
WSP will mobilize to the site on a quarterly basis (July and October 2023, and January and April 2024) and collect groundwater samples in accordance with the DEP-approved performance monitoring plan (Table 1) along with chlorinated VOCs. Prior to groundwater sampling during each event, the water level depth will be measured at each well to be sampled to determine purge well volumes. Groundwater samples will be collected from the wells in accordance with FDEP Standard Operating Procedures (SOPs) for Field Activities (DEP SOP-001/01), dated January 2017 (effective April 16, 2018). Prior to collecting the groundwater sample, each well will be purged and field parameters including temperature, pH, specific conductance, dissolved oxygen, turbidity, and oxidation-reduction potential will be monitored. Purge volumes and field parameters will be recorded on field data sheets, and the field monitoring equipment will be calibrated in accordance with the manufacturer’s recommended procedures.
