May 25, 2022 Proposal No. 202208346
Mr. Ben Grubbs Florida Department of Environment Protection Division of Waste Management Drycleaning Solvent Cleanup Program 2600 Blair Stone Road, MS 4520 Tallahassee, FL 32399-2400
RE: PROPOSAL FOR WELL INSTALLATION AND BIENNIAL GROUNDWATER MONITORING
CLASSIC CLEANERS OF BELLEVIEW
10841 SE HIGHWAY 441, BELLEVIEW, MARION RIVER COUNTY, FLORIDA
FACILITY ID NUMBER: ERIC_4906
PROPOSED TASK ASSIGNMENT: HW685_DC035A
Dear Mr. Grubbs:
Golder Associates USA Inc. (Golder) is pleased to provide the Florida Department of Environmental Protection (DEP) with this proposal for conducting well installation and the biennial groundwater monitoring activities at the Classic Cleaners of Belleview site, located at the above referenced address. This proposal includes a description of the proposed scope of services, level of effort sheets for each subtask, and a cost estimate form that summarizes the costs of the proposed services. The scope of work is based on the recommendations accepted by the DEP in the August 5, 2020, Memorandum submitted to E&E. The proposed scope of services is described below.
SUBTASK 1 – PROJECT MANAGEMENT
To facilitate project quality and satisfactory completion of the proposed scope of work, all aspects of this project will be overseen by an experienced project manager. The project manager will be responsible for the schedule, budget, invoicing, project activities, and communications with DEP. Project management will include coordination of field activities; resource allocation to confirm the work is conducted by qualified professionals in accordance with the project schedule; cost control and tracking of the budget status; and submission of project invoices in accordance with DEP Drycleaning Solvent Cleanup Program requirements. The project manager will be responsible seeing that the work is conducted in a manner that is consistent with the requirements of the DEP approved task assignment.
SUBTASK 2 – MONITORING WELL INSTALLATION
Three monitoring wells MW004, MW005, and MW006 were observed to be “destroyed” during the June 2020 site visit where no depth-to-water could be measured. The DEP recommended that MW004 and MW005 be replaced if they could not be rehabilitated prior to the next sampling event. Golder will install two replacement monitoring
Mr. Ben Grubbs Proposal No. 202208346
Florida Department of Environment Protection May 25, 2022 wells near the MW004 and MW005 locations. The attached Figure 3 shows the approximate monitoring well locations.
Golder will oversee use of a drill rig to install two shallow zone surficial aquifer wells. The proposed monitoring wells installed at the site will be constructed of 2-inch diameter, Schedule 40 PVC casing with 0.01-inch diameter slot size Schedule 40 PVC screen. The two shallow zone wells will be constructed with 10-feet of screen installed at approximately 47 to 57 feet bgs. The well casings and screens will be connected using flush-threaded joints.
The annular space between the borehole wall and each well screen will be completed with a 20/30 grade silica sand filter pack from the bottom of the borehole to approximately 2 feet above the top of the well screen. A minimum 2-foot thick, fine (30/65) grade silica sand seal will be placed above the sand filter pack and the remainder of the annular space will be completed with a Portland Cement Type II bentonite grout seal.
The wells will be installed with flush-mounted surface completion, a 2-foot by 2-foot sloped concrete pad and an 8-inch diameter bolt-down steel cover. Each of the well’s risers will be cut just below final grade and capped with water-tight expandable locking plugs.
To reduce sample turbidity and establish a good hydraulic connection with the aquifer, Golder will develop both wells the following day until purged water remains visibly clear and free of suspended particulate matter. Golder estimates two days to complete utility locates and well installation activities.
Golder will temporarily store the investigation derived waste (IDW) soil cuttings and purge water generated from well development in a 55-gallon steel drum(s). A waste characterization sample will be collected from the IDW drum(s) for analysis of total volatile organic compounds (VOCs) using EPA Method 8260 during Subtask 3 (groundwater monitoring). If the waste characterization sample is nonhazardous and acceptable to the disposal subcontractor, the IDW drum(s) will be picked up and transported to a facility for proper disposal. A non-hazardous waste manifest will be provided by the disposal subcontractor upon delivery to the disposal facility.
Upon completion of Subtask 2, an e-mail deliverable will be submitted and include copies of the field well installation forms, field notes, and well permits.
SUBTASK 3 – BIENNIAL GROUNDWATER MONITORING
Golder will collect groundwater samples from 10 shallow zone monitoring wells included in the groundwater monitoring program, MW001, MW002, MW003, MW004R, MW005R, MW007, MW008, MW010, MW0013, and MW015 for chlorinated VOCs. The potentiometric map of the shallow surficial aquifer (Figure 2) and chlorinated VOC detection map (Figure 3) from the previous groundwater monitoring report are attached for reference.
Prior to groundwater sampling, depth-to-water measurements will be collected using an electronic water level indicator to determine groundwater depth. During the sampling event, the locks and well caps will be inspected for damage or degradation and repaired or replaced, as necessary.
Groundwater samples will be collected from the wells in accordance with DEP Standard Operating Procedures (SOPs) for Field Activities (DEP SOP-001/01), dated January 2017 (effective April 16, 2018). Prior to collecting the groundwater sample, each well will be purged and field parameters including temperature, pH, specific conductance, dissolved oxygen, turbidity, and oxidation reduction potential (ORP) will be monitored. Purge volumes and field parameters will be recorded on field data sheets, and the field monitoring equipment will be calibrated in accordance with the manufacturer’s recommended procedures.
Groundwater samples will be collected using low flow sampling methods. The groundwater samples will be placed in laboratory-supplied containers, labeled with a unique identification number attached to the outside of the sample container, preserved on ice in a laboratory-provided cooler at a maximum temperature of four degrees Celsius, and delivered to a Florida Department of Health and National Environmental Laboratory Accreditation Program certified fixed-base laboratory along with the completed chain-of-custody documentation to be analyzed for chlorinated VOCs using EPA Method 8260B. Upon receipt of the laboratory analytical results, Golder will evaluate the constituent concentrations to determine if off-site notification is required.
Golder will temporarily store the IDW purge water generated from the groundwater sampling event in a 55-gallon steel drum(s) and sample the IDW for waste characterization and follow the same procedures as described in Subtask 2 for storage, characterization, and disposal.
Upon completion of Subtask 3, an e-mail deliverable will be submitted and include copies of the field data sheets, equipment calibration forms, field notes, laboratory analytical report, nonhazardous waste disposal, and laboratory chain-of-custody documentation.
SUBTASK 4 – REPORTING
The biennial groundwater monitoring report will include a description of the sampling procedures, evaluation of the analytical results, comparison of current to historical data, tabulated summary of the current and historical analytical data, a site map showing the groundwater analytical results, water level elevation information in tables and figures, laboratory analytical data, conclusions, and recommendations. Golder will also update the DEP GIS tables with the new water level data, field parameter measurements, and VOC data; run the DEP data checker on the completed tables; and submit the updated tables and error reports under this subtask. Additionally, Golder will forward the Automated Data Processing Tool (ADaPT) data deliverables for the groundwater sampling event provided by the laboratory.
HEALTH AND SAFETY
Golder will prepare a site-specific Health and Safety Plan for the field work. The work will be completed in accordance with Golder’s safety policies and procedures. At a minimum, Golder will wear safety glasses, high visibility safety vests, and steel-toed boots. A safety job briefing will be conducted prior to the start of work activities, and anytime the scope of work or site conditions change.
COST AND SCHEDULE
The estimated cost to complete the proposed scope of work is $30,291.62. Golder anticipates the well installation will be completed late July/early August 2022 and the groundwater monitoring event will take place approximately 2 weeks later. The LOE/CEF sheets and the subcontractor bids are included in Attachment A. The following table summarizes the proposed costs and deliverable schedule.

Ralph, I have this on schedule, but we recently had to increase our drum rate by $10.00.
Unfortunately the cost of fuel and disposal has forced our hand on this.

We are not going to provide annual Pricing for the Statewide Contract . After reviewing the scope with my partners and the variability with respect to disposal costing and ever changing fuel costs, we feel we can not bid this work given the current uncertainty in the market. We would be please to bid this work on a casa by case basis.

GPRS appreciates the opportunity to provide this proposal. I encourage you to visit our website (www.gprsinc.com) and contact any of the numerous references listed. Our insurance certificate and W-9 can also be downloaded here. Please feel free to contact me if you have any questions, or if you need additional information.
SCOPE OF WORK
Soil Borings We understand the scope to be to search for underground utilities within a radius of approximately 10’ around (2) monitoring wells, MW004R and MW005R. We will attempt to trace any utilities for which there are structures visible from the work area. Utilities will be marked on the surface using paint, flags, or other appropriate means. The client will be responsible to provide drawings or notify GPRS of any utilities known to be entering the work area for which there are no apparent surface features or structures that are visible from the work area. The areas should be laid out, marked, and cleared of obstructions prior to our arrival in order to avoid additional charges. Please visit www.simspec.org for an overview of our industry-leading best practices that will be applied to this project.
EQUIPMENT
Underground Scanning GPR Antenna. The antenna frequencies range from 250 MHz-450 MHz is mounted in a stroller frame which rolls over the surface. The surface needs to be reasonably smooth and unobstructed in order to obtain readable scans. Obstructions such as curbs, landscaping, and vegetation will limit the feasibility of GPR. The data is displayed on a screen and marked in the field in real time.
The total depth achieved can be as much as 8’ or more with this antenna but can vary widely depending on the types of materials being scanned through. Some soil types such as clay may limit maximum depths to 3’ or less. As depth increases, targets must be larger in order to be detected and non-metallic targets can be especially difficult to locate. Depths provided should always be treated as estimates as their accuracy can be affected by multiple factors. For more information, please visit: Link
Electromagnetic Pipe Locator. The EM locator can passively detect the signals from live AC power or radio signals travelling along some conductive utilities. It can also be used in conjunction with a transmitter to connect directly to accessible, metallic pipes, risers, or tracer wires. A current is sent through the pipe or tracer wire at a specific frequency and the resulting signal can then be detected by the receiver. A utility’s ability to be located depends on a variety of factors including access to the utility, conductivity, grounding, interference from other utilities, and many others. Depths provided should always be treated as estimates as their accuracy can be affected by multiple factors. For more information, please visit: Link
Traceable Rodder. The rodder has a copper wire encased in fiberglass. The line is pushed through accessible pipes before placing a current on the wire and the signal is then traced from the surface. The maximum traceable depth is 10’ depending on the soil conditions and the maximum distance is 200’. The line can be pushed through a pipe with direct access such as a sewer line at a cleanout or a storm drain catch basin. It may not be able to be pushed through deeper pipes within manholes. Electrical conduits will not be accessed by GPRS. The signal cannot be located through metallic pipes. For more information, please visit: Link
GPS. This handheld GPS unit offers accuracy down to 4 inches, however, the accuracy achieved will depend on the satellite environment at the time of collection and should not be considered to be survey-grade. Features can be collected as points, lines, or areas and then exported as a KML/KMZ or overlaid on a CAD drawing.
