HW561 DC009A

This Task Assignment authorizes Golder to conduct groundwater monitoring activities in accordance with the March 18, 2022, Scope of Work, Cost Estimate, and Bid Documentation.

Subtask 1: Project Management An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.

Subtask 2: Annual Groundwater Monitoring and IDW Treatment Golder will collect groundwater samples from monitoring wells MW001 – MW007, MW012 – MW022, MW024 – MW030, IW001, IW002, IW004, IW007, IW008, IW012, IW022, and IW023. Prior to sampling, depth-to-water will be measured. The wells will then be purged, and field parameters will be recorded. The groundwater samples will be collected using low flow sampling methods and analyzed for chlorinated volatile organic compounds (VOCs). Twelve samples will also be analyzed for total organic carbon (TOC), and eight samples will also be analyzed for dissolved gases. Investigation-derived waste (IDW) will be temporarily stored in a 55-gallon drum. Golder requested bids for IDW drum treatment and disposal from Clark, Erwin, Whatever Waste, and M & D Industrial Services. Erwin was selected as the lowest responsive and responsible bidder.

Subtask 4: Reporting Golder will submit the Annual Groundwater Monitoring Report and ADaPT and GIS deliverables to FDEP.

Golder and the FDEP confirmed on March 18, 2022, that an Access Agreement with the current real property owner of the Lake Lucina Shopping Center (ERIC_4610) site has been executed.

The FDEP Project Manager agrees to the scope of work and final cost associated with this Task Assignment. The final cost for this Task Assignment is $20,836.03 with 111 labor hours.

ANNUAL GROUNDWATER MONITORING

6054 MERRILL ROAD

JACKSONVILLE, DUVAL COUNTY, FLORIDA

DEP FACILITY ID NUMBER: ERIC_4610

PROPOSED TASK ASSIGNMENT: HW685_DC009A

Dear Mr. Grubbs:

Golder Associates USA Inc. (Golder) is pleased to provide the Florida Department of Environmental Protection (DEP) with this cost proposal for annual groundwater monitoring at the above-referenced site. This proposal includes a description of the proposed scope of services, level of effort sheets for each subtask, and a cost estimate form that summarizes the costs of the proposed services. The proposed scope of services is described below.

SUBTASK 1 – PROJECT MANAGEMENT

To facilitate project quality and satisfactory completion of the proposed scope of work, all aspects of this project will be overseen by an experienced project manager. The project manager will be responsible for the schedule, budget, invoicing, project activities, and communications with DEP and the field team. Project management will include coordination of field activities; resource allocation to confirm the work is conducted by qualified professionals in accordance with the project schedule; cost control and tracking of the budget status; and submittal of project invoices in accordance with DEP Drycleaning Solvent Cleanup Program requirements. The project manager will oversee the work so that it is conducted in a manner that is consistent with the requirements of the DEP-approved task assignment.

SUBTASK 2 – ANNUAL GROUNDWATER MONITORING AND IDW TREATMENT

Golder will collect groundwater samples from 22 monitoring/injection wells screened in the shallow zone of the surficial aquifer (MW-001, MW-003, MW-017, MW-018, MW 019, MW-020, MW-021, MW-022, MW-024, MW-025, MW-027, MW-028, MW-029, MW-030, IW001, IW002, IW004, IW007, IW008, IW012, IW022, and IW023);

seven downgradient offsite wells screened in the intermediate zone of the surficial aquifer (MW-002, MW-004, MW-005, MW-012, MW-014, MW-015, and MW-026); and four downgradient offsite wells screened in the deep

Mr. Ben Grubbs Proposal No. 202208346

Florida Department of Environmental Protection March 18, 2022 zone of the surficial aquifer (MW-006, MW-007, MW-013, and MW-016). The following monitoring wells will be used as temporary point-of-compliance (TPOC) monitoring wells: MW-013 (deep), MW-014 (intermediate), MW-016 (deep), MW-022 (shallow), and MW 024 (shallow). Figures 5 through 8 from the previous groundwater monitoring report are attached for reference.

Prior to groundwater sampling, depth-to-water measurements will be collected using an electronic water level indicator to determine groundwater depth. During the sampling event, the locks and well caps will be inspected for damage or degradation and repaired or replaced, as necessary.

Groundwater samples will be collected from the wells in accordance with DEP Standard Operating Procedures (SOPs) for Field Activities (DEP SOP-001/01). Prior to collecting the groundwater sample, each well will be purged and field parameters including temperature, pH, specific conductance, dissolved oxygen, turbidity, and oxidation reduction potential (ORP) will be monitored. Purge volumes and field parameters will be recorded on field data sheets, and the field monitoring equipment will be calibrated in accordance with the manufacturer’s recommended procedures.

Groundwater samples will be collected using low flow sampling methods. The groundwater samples will be placed in laboratory-supplied containers, labeled with a unique identification number attached to the outside of the sample container, preserved on ice in a laboratory-provided cooler at a maximum temperature of four degrees Celsius, and delivered to a Florida Department of Health and National Environmental Laboratory Accreditation Program certified fixed-base laboratory along with the completed chain-of-custody documentation to be analyzed in accordance with the monitoring plan shown in Table 1. In addition, twelve samples will be collected for Total Organic Carbon (Method SM5310B) and eight samples will be collected for Dissolved Gases (Method RSK175) analyses at the monitoring wells identified on Table 1. Upon receipt of the laboratory analytical results, Golder will evaluate the constituent concentrations to determine if off-site notification is required.

Golder will temporarily store the investigation derived waste (IDW) purge water generated from the groundwater sampling event in a 55-gallon steel drum(s). A waste characterization sample will be collected from the IDW drum(s) for analysis of total VOCs using EPA Method 8260. If the waste characterization sample is acceptable to the disposal subcontractor, the IDW drum(s) will be picked up and transported to a facility for proper disposal. A non-hazardous waste manifest will be provided by the disposal subcontractor upon delivery to the disposal facility.

Upon completion of Subtask 2, an e-mail deliverable will be submitted and include copies of the field data sheets, equipment calibration forms, field notes, laboratory analytical report, and laboratory chain-of-custody documentation.

SUBTASK 3 – REPORTING

The annual groundwater monitoring report will include a description of the sampling procedures, evaluation of the analytical results, comparison of current to historical data, tabulated summary of the current and historical analytical data, a site map showing the groundwater analytical results, water level elevation information in tables and figures, laboratory analytical data, conclusions, and recommendations. Golder will also update the DEP GIS tables with the new water level data, field parameter measurements, and VOC data; run the DEP data checker on the completed tables; and submit the updated tables and error reports under this subtask. Additionally, Golder will forward the Automated Data Processing Tool (ADaPT) data deliverables for the groundwater sampling event provided by the laboratory.

HEALTH AND SAFETY

Golder will update the existing site-specific Health and Safety Plan for the field work. The work will be completed in accordance with Golder’s safety policies and procedures. At a minimum, Golder will wear safety glasses, high visibility safety vests, and steel-toed boots. A safety job briefing will be conducted prior to the start of work activities, and anytime the scope of work or site conditions change.

COST AND SCHEDULE

The estimated cost to complete the proposed scope of work is $20,197.46. Golder anticipates the monitoring event will take place in late April or middle May 2022. The LOE/CEF sheets are included in Attachment A. Disposal subcontractor IDW quotes are included in Attachment B. The following table summarizes the proposed costs and deliverable schedule.

We are not going to provide annual Pricing for the Statewide Contract . After reviewing the scope with my partners and the variability with respect to disposal costing and ever changing fuel costs, we feel we can not bid this work given the current uncertainty in the market. We would be please to bid this work on a casa by case basis.
