About this file

This document comprises Task Assignment Change Orders (TACO) issued by the State of Florida, Department of Environmental Protection (FDEP) to Arcadis U.S. Inc. for environmental remediation work at the Butler Cleaners facility (ERIC_4573) located in Jacksonville, Duval County, Florida. The first change order (DC006A-1, Contract Number HW680) was executed on June 13, 2022, and the second change order (DC006A-2) was executed on August 12, 2022. Theresa Pepe served as the FDEP Project Manager, while Gus Suarez represented Arcadis U.S. Inc. as the contractor's project manager. William E. Burns Jr., as Secretary or Designee, authorized the change orders on behalf of the State of Florida.

Both change orders maintained a fixed task assignment value of $8,088.49 with no cost adjustments. The first change order extended the completion date by 60 days from July 15, 2022, to September 13, 2022, due to delays in obtaining signed site access agreements from the property owner. The second change order added an additional 30-day extension to the completion date, extending it from September 13, 2022, to October 13, 2022, following receipt of updated and signed site access forms. The deliverable for both change orders consisted of an Annual Monitoring Report, Investigative Derived Waste (IDW) manifest, and Project Management documentation, with invoicing contingent upon FDEP approval. The total cost remained $8,088.49, with no additional funding information required.

ARC DC006A Butler Report - Monitoring due 8/9/2022 ARC DC007A Herman Jackson Report - Monitoring due 8/16/2022

We have resolved the Access Agreements for Butler but will need additional time to conduct the groundwater sampling and complete the report. We still have one more property that we need to complete an Access Agreement prior to scheduling the groundwater sampling event at Herman Jackson.
