Arcadis U.S. Inc. will perform annual groundwater monitoring from select wells at the site. Groundwater monitoring will ensure that natural attenuation of contaminants continues to occur. Natural attenuation is expected to bring the site to unconditional closure within approximately three years. All activities will be conducted in accordance with the October 19, 2021, Work Plan submitted by Gus Suarez of Arcadis.

The final task assignment total is $5,021.78. Arcadis may invoice according to the attached schedule.

Specific negotiated subtasks are as follows:

Subtask 1: Project Management Project Management is automatically calculated as a percentage of total labor.

Subtask 2: Site Access Arcadis will present the new property owner with the appropriate FDEP site access forms before fieldwork commences.

Subtask 3: Annual Groundwater Monitoring Arcadis will sample five wells for drycleaning solvents based on recommendations from the previous monitoring report. IDW will be drummed and disposed by a licensed waste hauler per FDEP’s preference. An Annual Monitoring Report including recommendations for additional actions will be provided under this subtask.

Ms. Theresa Pepe Florida Department of Environmental Protection Waste Site Cleanup Section 2600 Blairstone Road, MS 4520 Tallahassee, FL 32399-2400

Date: October 19, 2021 Our Ref: 30019405 Subject: Natural Attenuation Monitoring Proposal Former Silver Hanger Cleaners 305 E Blanding Boulevard Orange Park, Florida FDEP Facility ID: 10/9501908 ERIC_4265 Contract No.: HW557

Dear Ms. Pepe, Arcadis U.S., Inc. (Arcadis) is pleased to submit to the Florida Department of Environmental Protection (FDEP) the following proposal to continue the Natural Attenuation Monitoring (NAM) Program at the Former Silver Hanger Cleaners (Site), located in Orange Park, Florida. This proposal includes background information, a description of the proposed scope of work (SOW), and an estimated cost and schedule for their completion.

Site Background The Site is located within the Tuesday Morning Shopping Plaza on Blanding Boulevard just north of Cleveland Avenue in Clay County. The Site was recently purchased by Khazaal Investment Group LLC. The Site consists of one unit within the shopping plaza and is currently operating as a Liquor Store.

On April 26, 2017, Arcadis submitted a Performance Based Cleanup (PBC) Proposal for the subject Site specifying the use of soil vapor extraction (SVE) technology followed by NAM Program to reduce the soil and groundwater impacts at the Site. On July 6, 2017, Arcadis submitted the Interim Source Removal Plan (ISRP) outlining the remedial strategy for the Site. The FDEP requested additional information on September 5, September 18, October 20, and November 6, 2017. Arcadis revised the ISRP and provided responses to FDEP comments on September 28, October 23, and October 30, 2018, November 6, 2017. The FDEP approved the report in a memorandum dated November 9, 2017.

Between March 5 and 28, 2018, Arcadis completed the construction of the SVE system as outlined in the approved ISRP. System startup activities were conducted at the Site on April 24, May 3, and May 4, 2018.

Following verification that SVE emissions were below required limits, Arcadis initiated continuous operations of the SVE system on May 8, 2018. The system operated until March 9, 2020. Officially, the FDEP approved Arcadis request to suspend the operation of the SVE system and the beginning of NAM in a memorandum dated May 28, 2020. On February 23, 2021, Arcadis completed the system decommissioning.

Florida Department of Environmental Protection October 19, 2021 www.arcadis.com 20211018.Silver Hanger Cleaners NAM Proposal

2/3

In July 2020, February 2021 and July 2021, Arcadis mobilized to the subject site to collect groundwater samples from selected monitoring wells. The results were summarized in the First through Third Semiannual NAM Reports dated August 12, 2020, March 4, 2021, and August 13, 2021. The groundwater analytical results revealed the presence of tetrachloroethene (PCE) concentration greater than the Groundwater Cleanup Target Level (GCTL), specified in Chapter 62-777, Florida Administrative Code (F.A.C.), in a sample collected from monitoring well MW005 during the three sampling events. In addition, PCE concentration greater than the GCTL was detected in the groundwater sample collected from monitoring well MW009 during the July 2021 event.

Based on the groundwater analytical results and as per the PBC agreement, Arcadis completed all appropriate tasks in the scope of work included in the Task Assignment (TA) DC035G. Therefore, Arcadis recommended closing out TA. In addition, Arcadis further recommended the continuation of NAM activities at the Site on a semiannual basis under a new TA. These recommendations were included in the Third Semiannual Report dated August 13, 2021. The most recent report was approved by the FDEP in an email dated August 26, 2021. Per FDEP request, the groundwater sampling frequency has been changed to annual events.

Scope Of Work This proposal includes Arcadis’ efforts to obtain a new site access agreement, complete one annual NAM event at the Site and submit one NAM report as summarized below.

Task 1: Project Management This task consists of project management activities and health and safety efforts in relation to the completion of Tasks 2 through 3.

Task 2: Site Access Agreement On September 28, 2020, Khazaal Investment Group LLC purchased the subject property, thus a new access agreement will be required. Arcadis will coordinate with the new owner to establish a current site access agreement for the property.

Task 3: Annual Natural Attenuation Monitoring

Arcadis will conduct one annual NAM event at the Site (e.g., July 2022). The event will consist of the collection of groundwater samples from five monitoring wells (MW001, MW003, MW005, MW007, and MW-009). Samples will be analyzed for volatile organic halocarbons (VOHs). Groundwater sampling activities will be performed in adherence to the FDEP Standard Operating Procedure (SOP), DEP-SOP-001/01, effective April 16, 2018.

Arcadis will prepare a NAM report summarizing the groundwater sampling results. The report will include appropriate tables, figures and recommendations.
