FACID NO. 16-9502441; ERIC_4587

Jacksonville, Duval County FROM: Theresa Pepe, Project Manager SUBJECT: Task Assignment DC007A (HW680) DATE: March 7, 2022

Arcadis U.S. Inc. will perform annual groundwater monitoring from select wells at the site. Groundwater monitoring will ensure that natural attenuation of contaminants continues to occur. Mobilization will be combined with the nearby Butler Cleaners as a cost saving measure. All activities will be conducted in accordance with the March 3, 2022, Work Plan submitted by Mike O’Sullivan of Arcadis.

The final task assignment total is $10,424.64. Arcadis may invoice according to the attached schedule.

Specific negotiated subtasks are as follows:

Subtask 1: Project Management Project Management is automatically calculated as a percentage of total labor.

Subtask 2: Annual Groundwater Monitoring Arcadis will update site access before fieldwork commences.

Arcadis will sample thirteen wells for drycleaning solvents based on recommendations from the previous monitoring report. IDW will be drummed and disposed by a licensed waste hauler per FDEP’s preference. Prior to disposal, one IDW sample will be collected and if determined to be hazardous, Arcadis will submit a change order to cover the additional costs. An Annual Monitoring Report including recommendations for additional actions will be provided under this subtask.

Hours and costs are comparable to past task assignments of similar scope.

Subject: Proposal for Annual Groundwater Monitoring at Herman Jackson

Cleaners, 1981 San Marco Boulevard, Jacksonville, Florida; FDEP Facility

Identification Number ERIC_4587.

Dear Ms. Pepe, Arcadis U.S., Inc. (Arcadis) is pleased to submit this level of effort (LOE) and cost proposal for the annual groundwater monitoring activities at Herman Jackson Cleaners, 1981 San Marco Boulevard, Jacksonville, Florida

(FDEP Facility Identification Number ERIC_4587; “the Site”; Figure 1). This proposal includes one groundwater monitoring event, and one letter report summarizing field activities. The FDEP cost estimating spreadsheets

(Attachment A) and subcontractor quote (Attachment B) are attached.

SCOPE OF WORK

Subtask 1 - Project Management

Subtask 1 is reserved for project management as specified by FDEP and is calculated automatically in the cost estimating form (Attachment A).

Subtask 2 - Groundwater Monitoring (2022)

Prior to collecting groundwater samples Arcadis will update the Access Agreements for multiple properties including the City of Jacksonville to access monitoring wells. Arcadis proposes to collect groundwater samples from 8 monitoring and 5 injection wells (MW002, MW004, MW010, MW011, MW020, MW022, MW025, MW027, IW-1, IW-3S, IW-3D, IW-4S, and IW-4D) in accordance with FDEP SOPs. Monitoring well locations are shown on attached figure. Samples will be submitted to a certified laboratory for analysis by EPA Method 8260C or equivalent (VOHs only). One QA/QC sample (trip blank) and duplicate sample (MW002) will be collected and submitted to the laboratory. Monitoring well purge water will be contained in 55-gallon drums, sampled for characterization, and disposed of properly. Monitoring well locations are shown on Figure 1.

Arcadis will incorporate the data collected during the groundwater sampling event into a letter report summarizing the field activities. Based on the concentrations of target analytes in injection wells IW-3D, IW-4S, and IW-4D, FDEP – Herman Jackson Cleaners Proposal 2022 www.arcadis.com _Herman Jackson AGM proposal_Mar2022

Arcadis will evaluate the need to conduct an ethyl lactate injection event. The report will include laboratory analytical data, figures presenting groundwater analytical data, and recommendations for the Site.

SCHEDULE

Arrangements for groundwater sampling activities will be conducted upon receipt of a Task Assignment (TA) from the FDEP.

ESTIMATED COST

A Cost Estimate Form and Level of Effort forms (CEF/LOE) that present estimated project costs are included as

Attachment A.

Please contact me if you have any questions regarding this proposal.
