FACID NO. 06-9502029; ERIC_4160

Fort Lauderdale, Broward County FROM: Theresa Pepe, Project Manager SUBJECT: Task Assignment DC014B (HW680) DATE: October 26, 2022

Arcadis U.S. Inc. will install and sample four monitor wells to various depths to help delineate the groundwater plume. All activities will be conducted in accordance with the August 31, 2022, Work Plan submitted by Doug McGlone of Arcadis.

The final task assignment total is $37,828.24. Arcadis may invoice according to the attached schedule.

Specific negotiated subtasks are as follows:

Subtask 1: Project Management Project Management is automatically calculated as a percentage of total labor.

Subtask 2: Well Installation and sampling Arcadis will obtain site access agreements for off-site properties before work commences. A site access form for the source property was obtained in October 2015.

No changes in property ownership have occurred since that date.

Arcadis will install three off-site monitor wells to delineate the downgradient plume and one onsite well to refine the area proposed for remediation.

Earth Tech Drilling was chosen as the drilling subcontractor based on their low bid of three bids solicited. IDW will be drummed and disposed by a licensed waste hauler.

Subtask 3: Reporting A Supplemental Assessment Report including recommendations for additional actions will be provided under this subtask.

Proposal for Additional Assessment Dry Cleaning Depot 3470 West Broward Blvd, Ft. Lauderdale, FL FDEP Facility ID No. 06-9502029

Dear Theresa:

As requested, Arcadis U.S., Inc. (Arcadis) is pleased to provide the Florida Department of Environmental Protection (FDEP) this proposal for monitor well installation and sampling and preparation of report of findings with recommendations as to whether the monitor well network is adequate.

PROPOSED SCOPE OF WORK

Task 1: Project Management

This task includes project management activities associated with completion of Tasks 2 through 3.

Task 2: Offsite Access, Well Installations and Sampling/Analysis

Arcadis proposes to secure an offsite access agreement with church located on north side of West Broward Blvd for the installation of three monitor wells as shown in the included Figure 4C. Four new monitor wells are proposed (three on offsite church property and one onsite, south of existing Well MW021D. These wells are proposed to delineate the VOH plume to the north and to aid in defining potential injection footprint onsite for plume remediation. Each new well will be constructed with 2-inch diameter pvc. The onsite new well will be screened 95- 100 ft bg. The offsite wells will include one screened 65-70 ft bg, one screened

75-80 ft bg, and one screened 95-100 ft bg. The top of casing elevations of the newly installed wells will be surveyed and referenced to the existing well network. The four newly installed wells will be sampled for the presence of VOHs by EPA 8260.

Task 3: Supplemental Groundwater Assessment Report

A brief letter report summarizing well installations and survey elevations, and groundwater monitoring methods and results including a table summarizing the groundwater analytical results and figures illustrating the extent and magnitude of the groundwater impacts and copies of sample lab reports will be provided to the FDEP, along with recommendations for future site activities. The report will be signed and sealed by a Florida registered professional geologist.

SCHEDULE

Arcadis will begin implementing the proposed tasks upon receipt of a Task Assignment (TA) from FDEP.

We anticipate performing the well installations and groundwater sampling within 30 days of receiving a signed Site Access Agreement for the offsite property to the northwest. We anticipate submittal of the letter report within 45 days of completion of field work activities.
