FACID NO. 61-9500991; ERIC_5504

Live Oak, Suwannee County FROM: Theresa Pepe, Project Manager SUBJECT: Task Assignment DC008B (HW680) DATE: March 19, 2024

Arcadis U.S. Inc. will perform annual groundwater monitoring from select wells at the site. This monitoring event also includes PFAS delineation sampling. Groundwater monitoring will ensure that natural attenuation of drycleaning contaminants continues to occur. All activities will be conducted in accordance with the March 14, 2024, Work Plan submitted by Mike O’Sullivan of Arcadis.

The original task assignment did not include costs for PFAS delineation and was revised. The final task assignment total including PFAS sampling is $17,916.56. Arcadis may invoice according to the attached schedule.

Specific negotiated subtasks are as follows:

Subtask 1: Project Management Project Management is automatically calculated as a percentage of total labor.

Subtask 2: PFAS Sampling An access agreement with the property owner was signed in April 2023. Arcadis will update site access if required before conducting fieldwork at the site. Additionally, Arcadis will search for several missing wells at the site and negotiate with property owners as necessary to recover the missing wells.

Arcadis will sample seven wells for PFAS compounds in an attempt to delineate previously detected PFAS at the site. Expanded PFAS monitoring has been requested by the Waste Site Cleanup Section’s geologist. FDEP’s laboratory in Tallahassee will be utilized for the PFAS analyticals. IDW will be drummed onsite and then transported by a licensed waste hauler, Erwin Remediation.

Kathy’s Cleaners, ERIC_5504

Subtask 3: Reporting Arcadis will submit an electronic summary report as the final deliverable. Per FDEP’s request, the deliverable will include one figure, field notes and the certified lab report.

Subtask 4: Annual Groundwater Sampling This subtask provides costs to sample twelve monitor wells for drycleaning solvents.

Water levels will be measured to determine the direction of groundwater flow. IDW will be drummed and disposed by a licensed waste hauler as noted above. Costs for expendables were removed from this subtask since they are covered under subtask 2.

Subtask 5: Reporting Arcadis will submit a written report as the final deliverable including recommendations for additional actions as needed.

Subject: Proposal for Annual Groundwater Monitoring at Kathy’s Cleaners, 202 West Howard Street, Live Oak, Florida, FDEP Facility Identification Number

ERIC_5504.

Dear Ms. Pepe, Arcadis U.S., Inc. (Arcadis) is pleased to submit this level of effort (LOE) and cost proposal for the 2024 annual groundwater monitoring activities at the Kathy’s Cleaners Facility located at 202 West Howard Street, Live Oak, Florida, (FDEP Facility Identification Number ERIC_5504; “the Site”). This proposal includes one groundwater monitoring event for PFAS and VOHs with an email summarizing the PFAS results, and one letter report summarizing field activities and results. The FDEP cost estimating spreadsheets (Attachment A) and subcontractor quote (Attachment B) are attached.

SCOPE OF WORK

Subtask 1 - Project Management Subtask 1 is reserved for project management as specified by FDEP and is calculated automatically in the cost estimating form (Attachment A).

Subtask 2 - Groundwater Monitoring (PFAS) Arcadis proposes to mobilize to the Site to collect groundwater samples from seven monitoring wells (MW001, MW002, MW006, MW010, MW012, MW013, and MW015) in accordance with FDEP SOPs. Monitoring well locations are shown on the attached figures. Samples will be submitted to FDEP’s Laboratory for PFAS analysis. Monitoring well purge water will be contained in 55-gallon drums, sampled for characterization, and disposed of properly.

Subtask 3 – Reporting (PFAS) Arcadis will submit a brief email summarizing the groundwater monitoring results. The email will include a summary of the analytical results, a figure showing PFOA, PFOS, and combined concentrations for each well sampled, field logs, and lab report. The email will also include recommendations for future PFAS sampling.

Subtask 4 - Groundwater Monitoring (VOHs) During the same mobilization as Subtask 2 Arcadis proposes to collect groundwater samples from seven shallow monitoring wells screened 60-70 feet below ground surface (ft. bgs; MW001, MW002, MW006, MW008, MW009, MW012, and MW015), four intermediate monitoring wells screened 115-120 ft. bgs (MW005, MW010, FDEP – Kathy’s Cleaner AGM Proposal 2024 www.arcadis.com Kathys Cleaner GW Mon proposal_Mar2024

MW011, and MW013), and one deep monitoring well screened 175-180 ft. bgs (MW014). Monitoring wells will be sampled in accordance with FDEP SOPs. Monitoring well locations are shown on the attached figures.

Samples will be submitted to a certified laboratory for analysis by EPA Method 8260C or equivalent (VOHs only). Monitoring well purge water will be contained in 55-gallon drums, sampled for characterization, and disposed of properly.

Arcadis proposes additional time to search for the monitoring wells MW002, MW006 and MW010 using a metal detector because they could not be located during the last monitoring event. Arcadis also proposes to communicate with the off-site property owner to determine what can be done to uncover and access MW002 which appears to have been paved over. Monitoring well locations are shown on Figures 1 and 2.

Subtask 5 - Reporting (VOHs) Arcadis will incorporate the data collected during the groundwater sampling event into a letter report summarizing the field activities. The report will include laboratory analytical data, figures presenting groundwater analytical data, a summary of the data, conclusions, and recommendations for future work at the Site.

SCHEDULE

Arrangements for groundwater sampling activities will be conducted upon receipt of a Task Assignment (TA) from the FDEP.

ESTIMATED COST

A Cost Estimate Form and Level of Effort forms (CEF/LOE) that present estimated project costs are included as

Attachment A.

Please contact me if you have any questions regarding this proposal.
