TO: Negotiation File Peerless Dry Cleaners

FACID NO. 13-9501189; ERIC_4379

Miami, Miami-Dade County FROM: Theresa Pepe, Project Manager SUBJECT: Task Assignment DC017B (HW680) DATE: September 21, 2022

Per FDEP’s direction, Arcadis U.S. Inc. turned off the remedial system and the site has progressed to annual monitoring. Therefore, Arcadis will perform decommissioning and site restoration activities including transferring the system to FDEP’s storage yard.

All activities will be conducted in accordance with the September 15, 2022, Work Plan submitted by Doug McGlone of Arcadis.

The final task assignment total is $12,572.57. Arcadis may invoice according to the attached schedule.

Specific negotiated subtasks are as follows:

Subtask 1: Project Management Project Management is automatically calculated as a percentage of total labor.

Subtask 2: Decommissioning An access agreement with the current real property owner was signed in December 2012. Arcadis and FDEP confirmed that no changes in property ownership have occurred since that dat.

Arcadis will decommission and remove the remedial system from the site. SVE wells will be capped and properly abandoned. Hurricane tie downs and electrical conduits will be removed. Vegetation clearing and grading/patching of parking lot asphalt will be performed as needed. The used carbon vessels will be properly disposed.

EESI was chosen as the subcontractor to perform the work. They are the second lowest bidder but can perform the work in one day as opposed to three days for the lowest bidder.

Subtask 3: Reporting Arcadis will submit a written report as the final deliverable. This report will include a photographic log, Equipment Chain of Custody forms and associated property documentation as required by FDEP.

Hours and costs are comparable to past task assignments of similar scope.
