Arcadis U.S. Inc. will perform annual groundwater monitoring from select wells at the site. Groundwater monitoring will ensure that natural attenuation of contaminants continues to occur. Additionally, select wells will be sampled for PFAS compounds. All activities will be conducted in accordance with the January 9, 2025, Work Plan submitted by Mike O’Sullivan of Arcadis.

The final task assignment total is $18,358.91. Arcadis may invoice according to the attached schedule.

Specific negotiated subtasks are as follows:

Subtask 1: Project Management Project Management is automatically calculated as a percentage of total labor.

Subtask 2: PFAS sampling FDEP obtained a signed site access form from the current property owner in June 2022.

Arcadis will conduct expanded PFAS sampling. They will sample nine wells for PFAS compounds using FDEP’s laboratory in Tallahassee. The wells have been chosen based on prior PFAS results and FDEP’s new guidance/flow chart.

Subtask 3: Annual Groundwater Monitoring Arcadis will sample twelve wells for drycleaning solvents based on recommendations from the previous monitoring report. Additionally, they will attempt to locate missing monitor well MW001. IDW will be removed by a licensed waste hauler.

Date: January 9, 2025

Subject: Proposal for Annual Groundwater Monitoring at Butler Cleaners, 1268 South McDuff Avenue, Jacksonville, Florida, FDEP Facility ID No. ERIC_4573.

Dear Ms. Pepe, Arcadis U.S., Inc. (Arcadis) is pleased to submit this level of effort (LOE) and cost proposal for the 2024 annual groundwater monitoring activities at the Butler Cleaners located at 1268 South McDuff Avenue, Jacksonville, Florida (FDEP Facility Identification Number ERIC_4576; “the Site”). This proposal includes one groundwater monitoring event for PFAS and one groundwater monitoring event for VOHs with an email summarizing the PFAS results, and one letter report summarizing field activities and results of the VOH monitoring. The FDEP cost estimating spreadsheets (Attachment A), and subcontractor quote (Attachment B) are attached.

SCOPE OF WORK

Subtask 1 - Project Management Subtask 1 is reserved for project management as specified by FDEP and is calculated automatically in the cost estimating form (Attachment A).

Subtask 2 - Groundwater Monitoring (PFAS) Arcadis proposes to mobilize to the Site to conduct an expanded annual groundwater monitoring event to delineate PFAS to the PGCTL and determining whether there are contributing offsite sources of PFAS. Based on the results from the 2023 monitoring event Arcadis recommends additional PFAS sampling of monitoring wells MW151, MW154, and MW194. Arcadis also recommends PFAS sampling of shallow monitoring wells MW303, MW003, and MW130; and intermediate monitoring wells MW124, MW302, and MW304.

Monitoring well locations are shown on the attached figures. Groundwater samples will be collected in accordance with FDEP SOPs and submitted to FDEP’s Laboratory for PFAS analysis. Monitoring well purge water will be contained in 55-gallon drums, sampled for characterization, and disposed of properly.

Subtask 3 - Groundwater Monitoring (VOHs) Arcadis proposes to mobilize to the Site and collect groundwater samples from shallow monitoring wells MW001, MW003, MW151, MW305, MW307, and MW317; and intermediate monitoring wells MW124, MW154, MW194, MW302, MW304, and MW308. Monitoring wells will be sampled in accordance with FDEP SOPs.

Monitoring well locations are shown on the attached figures. Samples will be submitted to a certified laboratory for analysis by EPA Method 8260C or equivalent (VOHs only). Monitoring well purge water will be contained in

FDEP – Butler Cleaners AGM Proposal Jan 2025 www.arcadis.com _Butler Cleaners AGM proposal_dec24

55-gallon drums, sampled for characterization, and disposed of properly. Arcadis proposes additional time to search for monitoring well MW001 using a metal detector because it could not be located during the last monitoring event. If MW001 can be located Arcadis proposes to collect a groundwater sample to be analyzed for VOHs. Monitoring well locations are shown on the attached Figures.

Subtask 4 - Reporting (PFAS and VOHs) Arcadis will incorporate the data collected during the groundwater sampling event into a letter report summarizing the field activities. The report will include laboratory analytical data, figures presenting groundwater analytical data, a summary of the data, conclusions, and recommendations for future work at the Site.

SCHEDULE

Arrangements for groundwater sampling activities will be conducted upon receipt of a Task Assignment (TA) from the FDEP. Arcadis will schedule the PFAS sampling event with the FDEP Lab.

ESTIMATED COST

A Cost Estimate Form and Level of Effort forms (CEF/LOE) that present estimated project costs are included as Attachment A.

Please contact me if you have any questions regarding this proposal.

Sincerely, Arcadis U.S., Inc.

Mr. O’Sullivan:

The price for the transportation and disposal (T&D) of one (1) 55-gallon drum of non-hazardous water impacted with drycleaning solvents at your site located in Jacksonville, FL will be $125.00 per drum/total.

Please ensure that all drum lids are properly secured and that drums are on a level surface like the surrounding area (i.e., not on a pallet, etc.). A single False Mobilization fee will apply if our driver is not able to locate a drum on site or if we are unable to access the property due to a locked gate, etc.

This quote is valid for 90 days.
