Subject: Proposal for Annual Groundwater Monitoring at Butler Cleaners, 1268

South McDuff Avenue, Jacksonville, Florida, FDEP Facility ID No. ERIC_4573.

Dear Ms. Pepe, Arcadis U.S., Inc. (Arcadis) is pleased to submit this level of effort (LOE) and cost proposal for the annual groundwater monitoring activities at the Butler Cleaners located at 1268 South McDuff Avenue, Jacksonville, Florida (FDEP Facility Identification Number ERIC_4576; “the Site”; Figure 1). This proposal includes one groundwater monitoring event, and one letter report summarizing field activities. The FDEP cost estimating spreadsheets (Attachment A) and subcontractor quote (Attachment B) are attached.

This proposed annual groundwater monitoring will aid in the continued evaluation of site conditions relative to the proposed Remedial Action Plan (RAP) Modification for enhanced bioremediation. Based on the 2020 and 2021 groundwater monitoring results and discussions with FDEP, the implementation of the RAP Modification was postponed until additional assessment and groundwater monitoring results source area results are conducted.

SCOPE OF WORK

Subtask 1 - Project Management

Subtask 1 is reserved for project management as specified by FDEP and is calculated automatically in the cost estimating form (Attachment A).

Subtask 2 - Groundwater Monitoring (2022)

Arcadis will conduct one annual groundwater monitoring event to collect groundwater samples from shallow monitoring wells MW151, MW303, MW305, MW307, MW309, and MW311; intermediate monitoring wells MW124, MW153, MW154, MW194, MW302, MW304, and MW318; and one equipment blank. Fourteen samples will be collected in accordance with the FDEP SOPs and submitted to a certified laboratory for analysis by EPA Method

8260C or equivalent (VOHs only). Additionally, one sample (MW164) will be analyzed for manganese to evaluate compliance with UIC variance requirements. Arcadis proposes to collect groundwater elevation measurements in the same wells that are being sampled. Monitoring well purge water will be contained in 55-gallon drums, sampled for characterization, and disposed of properly. Monitoring well locations are shown on Figure 1.

FDEP – Butler Cleaners Proposal 2022 www.arcadis.com _Butler AGM proposal_Feb2022

Arcadis will incorporate the data collected during the groundwater sampling event into a letter report summarizing the field activities. The report will include laboratory analytical data, figures presenting groundwater analytical data, and recommendations for the Site.

Based on the groundwater results, Arcadis recommends evaluating the need to implement the FDEP-approved

Remedial Action Plan (RAP) Modification, dated October 19, 2010, for enhanced dechlorination through substrate addition as FDEP priorities and funding allow. Prior to implementation of the RAP Modification, Arcadis recommends conducting a baseline sampling event to evaluate the distribution of chlorinated ethenes at the Site, particularly in the vicinity of the MW150-series monitoring wells, for consideration in the implementation of the

RAP Modification. Also, the RAP Modification should be reviewed prior to implementation relative to advances in remediation technologies to verify that the selected approach is consistent with these advances.

SCHEDULE

Arrangements for monitoring well replacement and groundwater sampling activities will be conducted upon receipt of a Task Assignment (TA) from the FDEP.

ESTIMATED COST

A Cost Estimate Form and Level of Effort forms (CEF/LOE) that present estimated project costs are included as

Attachment A.

Please contact me if you have any questions regarding this proposal.

Sincerely, Arcadis U.S., Inc.
