Proposed Scope of Work Altamonte Springs Dry Cleaners Biennial Groundwater Sampling Proposal Task Assignment DC(TBD)A ERIC_5459 (Former FAC ID No. 599500541) APTIM Environmental & Infrastructure, LLC (APTIM) 12/8/25

The following proposed scope of work for the former Altamonte Springs Dry Cleaners is to perform biennial groundwater sampling. This scope of work includes the collection of groundwater samples from four monitoring wells for analysis by EPA Method 8260 for volatile organic halocarbons (VOHs) as recommended in the previous Biennial Groundwater Monitoring Report dated November 9, 2023. No PFAS sampling is proposed as previous sampling results for these constituents reported no exceedances of the PGCTLs, and a Publix Supermarket is the plazas anchor store.

Subtask - 1: Project Management This task includes project management activities associated with the completion of

Tasks 2 through 5, as calculated in accordance with Contract HW689.

Subtask - 2: Monitoring Well Sampling APTIM proposes groundwater samples be collected from four monitoring wells. Prior to groundwater sample collection, depth to water measurement will be collected from all wells that can be located and accessed. Groundwater samples from monitoring wells

MW009, MW017 and MW019 will be collected from the deployed Passive Diffusion Bags (PDBs). In addition, a groundwater sample will be collected from deep monitoring well

MW013 using conventional sampling techniques. The groundwater sample from MW013 will be collected in accordance with the FDEP Standard Operating Procedures for Field

Activities utilizing a submersible pump and a field-calibrated YSI multi-parameter instrument.

The purge and decon water from sampling activities will be placed in a DOT approved drum, characterized by the collection of an IDW sample for VOHs only (EPA Method

8260), and stored onsite for future transport and disposal. APTIM recommends Erwin Remediation to perform the IDW transportation and disposal based on three bids received. Note that although PFAS constituents were not reported above the PGCTLs in groundwater at the site, PFAS was detected, therefore the disposal rate is increased.

Please see the attached IDW bid summary documents.

Three replacement PDBs will be obtained from ALS Environmental (ALS), a licensed manufacturer of passive diffusion samplers. The PDBs will be assembled and placed within each well similar to the previous PDB sampler placement. Each PDB sampler is 24 long and 1 ¼ in diameter and pre-filled with approximately 220 milliliters of certified, laboratory-grade, analyte-free, deionized water. Specific deployment of the PDB bags is as follows.

1. MW009, (2 well) total depth of approx. 84 ft bls (placement from 80.5 to 82.5 ft bls)

2. MW017, (2 well) total depth of approx. 85.5 ft bls (placement from 82 to 84 ft bls)

3. MW019, (2 well) total depth of approx. 85.5 ft bls (placement from 82 to 84 ft bls)

Subtask - 3: Biennial Groundwater Reporting At the conclusion of the groundwater sampling event and upon receipt of the analytical report from the fixed laboratory, a Biennial Groundwater Monitoring Report documenting the sampling and results will be prepared and include conclusions and recommendations for future site activities.

APTIM recommends the following schedule:

Description Due Date

Perform VOH Sampling 2/27/26

Submit Biennial Groundwater Monitoring Report 3/27/26

Task expiration date 4/30/26

APTIM proposes to perform this scope of work in accordance with the terms and conditions of the DEP Contract No. HW689 between the Florida Department of Environmental Protection and APTIM dated December 21, 2021. APTIM will begin the proposed scope of work upon issuance of the Task Assignment Notification. This site has an Access Agreement dated October 2004 providing approved for the proposed scope of work.
