Subtask 1: Project Management Project Management is calculated at 13% of the direct labor.

Subtask 2 and 3: Operation and Maintenance This task includes quarterly O&M services (May through October 2023) for the remedial system operation. The services include system maintenance, field and system data collection, verification of efficient system operation and collection of one quarterly vapor influent sample per quarter (June 2023 and September 2023) from the remediation system for field screening with a PID and fixed base laboratory analysis. One quarterly monitoring report will be submitted as a deliverable each quarter that will present monthly O&M data for each of the three months per quarter.

Subtask 4: Semi-Annual Remedial System O&M Report This task includes preparation of a semi-annual remedial system O&M report (May through October 2023) for two quarters of remedial system operation. The O&M Report will include quarterly influent vapor analytical results, associated forms summarizing field activities performed in support of the remedial system operation, and conclusions and recommendations for continued system operation.

Subtask 5 and 8: Utilities This task includes the estimated cost associated with providing electrical power to the remedial system for the first six (6) month period and the last six (6) month period of operation in this Task Assignment. Administration costs are covered under Handling Fees. Service cost for the cellular communications is paid annually.

Proposed Scope of Work O&M Services & Groundwater Monitoring (12 months) Palm Bay Laundry and Dry Cleaners (former) 5275 Babcock Street, Palm Bay, Florida FAC ID No. 059502190 (ERIC_4028) Aptim Environmental & Infrastructure, Inc. (APTIM) 5/31/22

The following proposed scope of work has been developed for the former Palm Bay Laundry and Dry Cleaners site located in Palm Bay, Florida, and was prepared to provide remedial system operation and maintenance (O&M) services and groundwater monitoring activities for a period of 12 months. Attached are the CEF and LOE proposed for the Palm Bay Laundry and Dry Cleaner as Task Assignment (TA) request DC003B. The proposed level of effort includes four quarters of O&M activities in support of SVE operation through April 2024.

Subtask 2: This task includes quarterly O&M services (May through July 2023) for the remedial system operation. The services include system maintenance, field and system data collection, verification of efficient system operation and collection of one quarterly vapor influent sample (June 2023) from the remediation system for field screening with a PID and fixed base laboratory analysis. One quarterly monitoring report will be submitted as a deliverable that will present monthly O&M data for each of the three months per quarter.

Subtask 3: This task includes quarterly O&M services (August through October 2023) for the remedial system operation. The services include system maintenance, field and system data collection, verification of efficient system operation and collection of one quarterly vapor influent sample (September 2023) from the remediation system for field screening with a PID and fixed base laboratory analysis. One quarterly monitoring report will be submitted as a deliverable that will present monthly O&M data for each of the three months per quarter.

Subtask 4: This task includes preparation of a semi-annual remedial system O&M report (May through October 202) for two quarters of remedial system operation. The O&M Report will include quarterly influent vapor analytical results, associated forms summarizing field activities performed in support of the remedial system operation, and conclusions and recommendations for continued system operation.

Subtask 5: This task includes the estimated cost associated with providing electrical power to the remedial system for the first six (6) month period of operation in this Task Assignment. Service cost for the cellular communications is paid annually.

Subtask 6: This task includes quarterly O&M services (November 2023 through January 2024) for the remedial system operation. The services include system maintenance, field and system data collection, verification of efficient system operation and collection of one quarterly vapor influent sample (December 2023) from the remediation system for field screening with a PID and fixed base laboratory analysis. One quarterly monitoring report will be submitted as a deliverable that will present monthly O&M data for each of the three months per quarter.

Subtask 7: This task includes quarterly O&M services (February through April 2024) for the remedial system operation. The services include system maintenance, field and system data collection, verification of efficient system operation and collection of one quarterly vapor influent sample (April 2024) from the remediation system for field screening with a PID and fixed base laboratory analysis. One quarterly monitoring report will be submitted as a deliverable that will present monthly O&M data for each of the three months per quarter.

In addition, this task includes annual groundwater sampling. The designated monitoring are based on the recommendations APTIM provided in the ISR that identified annual groundwater sampling of monitoring wells MW-2S, MW-8S and PZ001 for groundwater restoration, MW-9D for deep monitoring and MW-3S and MW-7S for downgradient monitoring during SVE system operation. If additional wells are requested, prior to that time, based on data presented in the currently tasked annual report, the additional sampling will be incorporated into the sampling effort under a change order.

Subtask 8: This task includes the estimated cost associated with providing electrical power to the remedial system for the second six (6) month period of operation in this Task Assignment.

Subtask 9: Included in this subtask is preparation of an annual report that includes the second six months of SVE operation and maintenance, and annual groundwater sampling. The Annual Groundwater and O&M Report will be prepared summarizing the O&M data and groundwater monitoring data throughout the year of remedial system operation. The O&M Report will include quarterly influent vapor analytical results, associated forms summarizing field activities performed in support of the remedial system operation, and conclusions and recommendations for continued system operation.

A schedule of the proposed O&M, groundwater sampling, and reporting activities follows:

Qtr. 1 O&M Reporting Due Date 8/18/23

Qtr. 2 O&M Reporting Due date 12/15/23

Semi-Annual O&M Report Due Date 1/9/23

Qtr. 3 O&M Reporting Due date 2/16/24

Qtr. 4 O&M Reporting Due date 5/17/24

Annual O&M Report Due date 6/21/24

TA completion Due Date 8/23/24

This LOE and proposed scope of work does not include extensive system diagnostics and repairs. Required diagnostic and repair services will be estimated, approved and funded individually, as required and based on O&M findings.

APTIM proposes to perform this scope of work in accordance with the terms and conditions of the contract executed December 21, 2021, between APTIM Environmental & Infrastructure, LLC and Florida Department of Environmental Protection No. HW689.

APTIM will begin the proposed scope of work upon written authorization. APTIM has an existing Access Agreement in place from the current site owner.

Craig, From: Roberts, Gregg Sent: Tuesday, October 25, 2022 3:15 PM To: Fischer, Steve M Cc: Kramer, Eric Subject: Request for Quotation for Non-Hazardous IDW T&D – Krause’s Cleaners

Hello Steve:

I need an estimate to include in a proposal to pick-up and dispose of non-hazardous liquid which will be generated during upcoming groundwater sampling activities at the following site:

Here are the site specifics:

Krause’s Coin Laundry and Dry Cleaning 2049 U.S. Highway 1 South Fort Pierce, Saint Lucie County, Florida 34950 ERIC_5411 (Former Facility ID No. 569501010) DEP/EPA Identification Number FLR000231142

Please estimate the IDW transportation and disposal estimate is for non-hazardous waste. Please provide a

Lump Sum Estimate per Drum, as the actual disposal count may vary from the estimate below:

a) 1 drum of purge/decon water (liquid)

b) Please include transportation, disposal and preparation of the waste profiles and manifests in the cost estimate.

The former Krause’s Coin Laundry and Dry Cleaning was accepted into the Drycleaning Solvent Cleanup Program in 1997 under FDEP Facility Identification Number 569501010. The work is being performed for the FDEP Waste Cleanup Section.

There is no analytical available now, but the proposal is for non-hazardous transportation and disposal. Characterization of the liquid IDW composite will be submitted for EPA Method 8260 (VOHs only).

If you need additional analysis for the waste characterization, please indicate so in your response.

I do not need a formal proposal, an email response by Thursday, October 27, 2022, would be greatly appreciated.

Thank you, Gregg
