Task Assignment Change Order Summary
This document represents a Task Assignment Change Order (TACO #1) between the State of Florida Department of Environmental Protection (FDEP) and APTIM Environmental & Infrastructure, LLC (Contractor). The change order amends the original Task Assignment DC003C under Contract HW689, executed on December 21, 2021. The project site is the former One Stop Cleaners facility located at 3303 University Drive, Coral Springs, Broward County, Florida (ERIC_4123). Department Project Manager Nicole Knutson and Contractor Project Manager Eric Kramer serve as the primary contacts for this engagement.
The change order reconciles Work Change Directive #1, which authorized APTIM to install rotometer flow meters at the existing vapor extraction wells to improve vapor flow measurement accuracy. The total task amount increased from $49,200.52 to $51,985.02, representing an increase of $2,784.49. Compensation consists of a fixed unit rate of $48,385.02 and cost reimbursement of $3,600.00. The task completion date remains July 26, 2025, with final deliverables due May 23, 2025. Services include quarterly operations and maintenance reports, semi-annual and annual groundwater sampling reports, utility cost reimbursement at $300 per month with a $20 monthly handling fee, and analytical laboratory costs. Invoice schedules are structured upon FDEP approval of deliverables, with amounts ranging from $6,374.62 to $24,745.34 per invoice milestone. APTIM's time, labor, and subcontractor costs have been reviewed and determined appropriate for the scope of services.
