About this file
This document is a Task Assignment Change Order (TACO) No. 4 executed between the State of Florida Department of Environmental Protection (FDEP) and APTIM Environmental & Infrastructure, LLC. The change order amends Task Assignment DC029A under Contract HW689 for site assessment and reporting services at Spring Lake Cleaners Inc. (Former), located at 1120 Havendale Boulevard NW, Winter Haven, Polk County, Florida. The project is identified as ERIC_5381 (Former Facility No. 539502466). FDEP's project manager is Alyssa Lamb, and the contractor's project manager is Eric Kramer. The change order was executed on March 13, 2026, with digital signatures from both parties.
TACO No. 4 provides additional funding of $961.08 to increase the total task assignment value from $106,551.26 to $107,512.34 for the collection of PFAS (per- and polyfluoroalkyl substances) groundwater samples that could not be collected during the January 2026 sampling event due to Federal Express shipping delays caused by winter storms. The scope includes purging and sampling from wells MW001, MW002, PZ002, and PZ003, plus collection of a PFAS IDW (investigation-derived waste) sample. The completion date was extended 60 days from April 17, 2026, to June 16, 2026. Deliverables include field notes packages, DPT field notes, a site assessment report, and a PFAS email deliverable with invoicing upon FDEP approval of each subtask. The cost breakdown allocates $455.00 for equipment and $120.00 for vehicle rental, with the remainder covering labor and compliance with FDEP methodology DEP SOP: LC-001-3 for PFAS analysis.

Proposed Scope of Work Spring Lake Cleaners Inc. (Former) Spring Lake Plaza 1120 Havendale Boulevard NW Winter Haven, Polk County, FL 33881 Site Assessment and Reporting Task Assignment DC029A TACO #4 ERIC_5381 (Former Facility No. 539502466) Aptim Environmental & Infrastructure, LLC March 2, 2026
The following Task Assignment Change Order (TACO) #4 has been prepared to provide additional funding for the collection of the PFAS groundwater samples, which could not be collected during the January scheduled groundwater sampling event due to Federal Express shipping delays caused by the winter storms across much of the United States at the time.
Subtask 1-Project Management Project management hours are calculated based on the labor hours in the scope of work.
Subtask 2-Meeting & MAGS/Soil Sampling: No Changes Subtask 3-DPT & GW Sampling Field Activities: No Changes Subtask 4-Well Installations: No Changes Subtask 5-SAR Reporting: No Changes
Subtask 6-Additional PFAS Sampling:
As noted above, the final site assessment groundwater sampling event was scheduled and performed on January 26, 2026. At that time, PFAS samples were also scheduled for collection, however, winter weather caused shipping delays and the sampling kit from the FDEP Laboratory in Tallahassee was not delivered by Federal Express in time to allow for the collection of the samples. Therefore, the site will need to be revisited for the collection of PFAS samples. The TACO includes the necessary additional labor, equipment and travel to revisit the site and purge wells MW001, MW002, PZ002 and PZ003 for the collection of the PFAS samples, which will also include the collection of a PFAS IDW sample for characterization of the waste.
At this time, to allow time for processing of the TACO #4 request, rescheduling the PFAS sampling, analysis of the samples at the FDEP Laboratory, scheduling the IDW pickup and reporting, a 60-day extension is requested for the submittal of the Subtask 5 and 6 deliverables. The extension will extend the due date for both Subtasks to April 14, 2026. The Task Assignment Completion Date would be extended to June 16, 2026.
APTIM proposes to perform this scope of work in accordance with the terms and conditions of the contract executed between APTIM Environmental & Infrastructure, LLC and Florida Department of Environmental Protection No. HW689. APTIM will begin the proposed scope of work upon issuance of the Task Assignment Notification.
