SUBJECT: Negotiation Summary

HW689 DC018B

DATE: May 14, 2025

This TACO authorizes APTIM to perform additional assessment activities offsite, including hand auguring at four additional locations and drilling wells at eleven additional locations. Temporary Traffic Control Plans (TTCPs) will be developed and implemented prior to drilling in the Right of Way (ROW). Drilling will occur within four separate ROW areas. The drilling subcontractor which was previously selected for this task, Groundwater Protection, submitted a new quote with increased rates. APTIM requested new bids from three other drilling vendors. Of the four total vendors, Preferred Drilling Solutions submitted the lowest bid. As a result, Preferred has been selected for this additional DPT work.

Negotiations between The Department and APTIM led to a cost reduction of ($461.04). The total number of labor hours for Subtask 4 has been reduced from 175 to 167.

Total task amount has increased from $105,167.51 to $147,732. 8. This is a cost increase of $42,56 . 7. APTIM’s time, labor, and subcontractor costs for these activities have been reviewed and are appropriate. The level-of-effort and cost estimate are reasonable for the scope of services. Final deliverable due date has been extended from April 25, 2025 to September 26, 2025. Task completion date has been extended from May 30, 2025 to December 3, 2025.

Proposed Scope of Work Deluxe Cleaners & Laundry 1622 West Kennedy Boulevard. Tampa, Hillsborough County, FL 33606 Site Assessment and Reporting Task Assignment DC018B TACO #3 ERIC_4696 (Former Facility No. 299500411) Aptim Environmental & Infrastructure, LLC May 5, 2025 Deluxe Cleaners DC018B Task Assignment Change Order (TACO) #3 was created to shift the focus of the assessment to offsite, as the bulk of the contamination appears to be offsite. The analytical results to date for the onsite soil and groundwater samples indicate the soil and groundwater source extends offsite toward the east into an “alley” used by the businesses and several residents. Since efforts to obtain offsite access to the adjacent properties have been unsuccessful, permits have been obtained to allow for access to several Right of Way (ROW) areas to perform well drilling activities.

Subtask 1-Project Management:

This task includes project management activities associated with the completion of the remaining Subtasks, as calculated in accordance with Contract HW689.

Subtask 2-Preliminary Site Activities and MAGS Testing: No Changes Subtask 3-Soil and Groundwater DPT Sampling Activities: No Changes Subtask 4-Well Installations:

Subtask 4 has been updated to allow for the collection of soil samples at four additional locations, and for the installation of a total of 11 wells, as discussed during the March Project Summary Meeting in Tallahassee. Previously, Subtask 7 was approved for the development of a plan to access several offsite ROW areas for the collection of soil samples and the installation of wells surrounding the small property.

The results of these efforts resulted in the issuance of the following permits, which have not been activated until final approval of the revised scope. Once scope approval is received APTIM will submit a notice to proceed:

1) City of Tampa permit number ROW-25-0000105 has been developed to allow for the installation of wells and the collection of soil samples in the alley between the Starbucks and the Dunkin’.

2) City of Tampa permit ROW-25-000013 was developed to allow for the installation of a shallow and deep well in a parallel parking location on the west side of South Dakota Avenue.

3) City of Tampa permit ROW-25-0000104 was created to allow for the installation of a shallow and deep well within a parallel parking lane on the west side of South Rome Avenue

4) Finally, FDOT permit 2024-K-796-00174 was created to allow for the installation of a shallow well within the northern Right-of-Way of State Road 60 (West Kennedy Boulevard).

Temporary Traffic Control Plans (TTCPs), formerly called Maintenance of Traffic (MOT) were developed for each permit, as follows:

1) The drilling and soil sampling within the alley between the Starbucks and the Dunkin’ will require a TTCP to allow the alley to be shut down to through traffic while the work is being completed. Since the road becomes one-way on the residential portion, flaggers will be required to control traffic into and out of this portion of the alley so the residences can leave and enter the area without incident.

2) The South Dakota Avenue work will require a TTCP to close of the parking lane and barricade the parking spaces from access.

3) The TTCP for on South Rome Avenue will also require closure of the parking lane and the placement of barricades to block access to the parking spaces.

4) The required TTCP for the well proposed in the northern Right-of-Way of State Road 60 will require a sidewalk closure (Standard Index 102-660).

5) Diagrams of the required TTCP are provided in the attached bid documents. Sunbelt Rentals provided the lowest estimates to provide TTCP and signage for each location.

Upon submittal of the notice to proceed for each drilling location, each permit will be activated. At that time, the actual proposed TTCP plans will be submitted for review and approval by the permitting entity.

For the alley closure, the City of Tampa will require the delivery of Work Notice Letters for the residential houses located on the alley, summarizing the proposed scope of work and schedule. In accordance with the City of Tampa permits, requests for utility As-Builts will be submitted for each location. The As-Builts will be utilized to avoid utilities during white lining and will be used by the private utility survey subcontractor to confirm that all known utilities are marked by Sunshine 811. Before drilling, each area will be inspected by the permitting entity (City of Tampa or FDOT) to document the drilling location and proximity to utilities, and compliance with the specific permit conditions and City or State regulations. It is anticipated that the alley will be closed for a total of 5 days (during working hours), and the remaining locations will be closed for one day each to construct the wells. The TTCP signage will be dropped at the end of each day and replaced prior to work each morning.

Following approval of the schedule, the TTCPs will be activated, and soil sampling will be performed on the initial day during the alley closure. Four (4) hand-augered borings will be advanced to an average depth of 6 ft bls to the water table or to the zone of collapse, whichever is first. A lithologic log will be prepared from the notes at each bore. Soil samples will be collected from depths of 6-inches, 2-foot, 4-foot, and 6-foot (dependent on the depth to water) for fixed laboratory analysis. The locations of the proposed soil samples are shown on the attached Proposed Well and Soil Boring Location Map. The laboratory samples will be analyzed by Environmental Protection Agency (EPA) Method 8260 analysis for the presence of VOCs (full list) by a fixed based laboratory. Additional material will be collected from each sample location (Encores) for possible SPLP extraction (EPA Method 1312) and analysis of the leachate for volatile organic halocarbons (VOHs) by EPA Method 8260.

The proposed well locations to be installed are also shown on the attached map. As discussed, five shallow and six deeper monitoring wells will be installed onsite and surrounding the small Dunkin’ Donuts property. Since the work increased significantly and now includes mostly offsite drilling in ROWs, the drilling costs were re-bid and the former drilling costs have been withdrawn from the subtask. All the wells will be installed with a direct push drilling rig. Because DPT refusal was encountered onsite at a depth around 25 ft, a macrocore will be installed at each deep well location to determine the lithology and the construction of the well. The deeper wells will be installed to refusal, or to a total depth of 30 ft bls, if refusal is not encountered.

The deeper wells will be constructed of 1-inch diameter PVC with 5 feet of fine mesh pre-pack 0.006-inch slotted screen with a 30/65 sand pack installed from 25 to 30 ft bls. A two-to-three-foot fine silica sand seal will be installed above the screen and the remaining annulus will be tremie grouted to just below the surface. The adjacent shallow wells will be constructed of 1-inch diameter PVC with 10 feet of fine mesh pre-pack 0.006-inch slotted screen with a 30/65 sand pack and installed to a 15 ft total depth, or less, dependent on the depth to groundwater encountered at the time of drilling.

Upon completion of the drilling activities, all the wells, including the existing piezometers will be sampled and submitted to a State of Florida-certified fixed-base laboratory for analyses by EPA Method 8260 (full list). The top of casing elevations for the wells will be surveyed to the existing benchmarks installed previous on the former drycleaning property.

Subtask 5-Repoting: No Changes Subtask 6-WCD#3: No Changes Subtask 7-Permitting and MOT Development: No Changes

Subtasks 2, 3, 6 and 7 have been completed and the results and deliverables reported. Having completed efforts to initiate access to the offsite ROW locations for drilling and upon approval of TACO #3 the wells will be scheduled for installation by August 30, 2025 and sampled soon thereafter. The results of the sampling will be reported to the FDEP upon receipt to determine if additional assessment activities should be performed, or if reporting (SAR) should be completed to document the work completed to date.

If it is determined that reporting is preferred, the SAR will be completed and submitted by September 26, 2025. The task completion date should be extended to November 28, 2025.

APTIM proposes to perform this scope of work in accordance with the terms and conditions of the contract executed between APTIM Environmental & Infrastructure, LLC and Florida Department of Environmental Protection No. HW689. APTIM will begin the proposed scope of work upon issuance of the Task Assignment Notification.
