About this file

This is a Task Assignment Change Order (TACO #2) issued by the State of Florida Department of Environmental Protection (FDEP) to Aptim Environmental & Infrastructure, LLC for environmental assessment services at the Deluxe Cleaners & Laundry site located at 1622 West Kennedy Boulevard, Tampa, Hillsborough County, Florida. The project is identified as ERIC_4696 under Contract HW689. The FDEP project manager is Nicole Knutson, and the contractor's project manager is Eric Kramer.

TACO #2 authorizes APTIM to prepare Maintenance of Traffic (MOT) plans and locate utilities in preparation for monitoring well installation at multiple offsite locations, including the alley between Dunkin' Donuts and Starbucks on West Kennedy Boulevard, and right-of-way areas on South Dakota Avenue, South Rome Avenue, and North Kennedy Boulevard. The amended total contract value is $105,167.51, representing an increase of $12,363.62 from the prior price of $92,803.89. The task completion date has been extended from September 27, 2024, to May 30, 2025 (245 days total). Deliverables include approved MOT permits, updated MAGS location maps, and associated documentation, with final reporting expected by April 25, 2025. Invoicing is structured upon FDEP approval of specific deliverables for analytical results, site assessment reporting, well construction documentation, and permitting activities. The cost increase reflects additional labor, travel, and subcontractor expenses for utility location services and permitting research required due to unsuccessful offsite property access negotiations.

Memorandum

TO: ERIC_4696

FROM: Nicole Knutson, Project Manager

SUBJECT: Negotiation Summary

HW689 DC018B CO2

DATE: September 3, 2024

APTIM’s efforts to obtain offsite access to properties to the east and south of the source property have been unsuccessful. Therefore, APTIM will attempt to gain access to the easement to the east of the source property and apply for ROW permits for roads to the north, east and west of the source property. APTIM will also submit a ticket to Sunshine811 to locate utilities prior to MAGS well installations.

This Change Order increases the total cost by $12,363.62. The price prior to this Change Order was $92,803.89, and the amended price is $105,167.51. APTIM’s time, labor, and subcontractor costs for these activities have been reviewed and are appropriate. The level-of-effort and cost estimate are reasonable for the scope of services. Final deliverable due date has been extended from July 26, 2024 to April 25, 2025. The task completion date has been extended from September 27, 2024 to May 30, 2025.

Proposed Scope of Work Deluxe Cleaners & Laundry 1622 West Kennedy Boulevard. Tampa, Hillsborough County, FL 33606 Site Assessment and Reporting Task Assignment DC018B TACO #2 ERIC_4696 (Former Facility No. 299500411) Aptim Environmental & Infrastructure, LLC August 28, 2024

Deluxe Cleaners DC018B Task Assignment Change Order (TACO) #2 was created for the preparation of numerous permits to allow the installation of monitoring wells to advance the soil and groundwater site assessment.

The analytical results to date for the onsite soil and groundwater samples, which have been previously reported indicate the soil and groundwater source extends offsite toward the east into an “alley” used by the businesses and several residents. Efforts to obtain offsite access to the adjacent property owners located east and south of the source property have not been received favorable and will likely not be approved and have delayed the assessment. Therefore, access to the east is limited to a single lane alley which, according to the City of Tampa cannot be blocked (even at night).

Based on the preliminary data, the orientation of the former drycleaning facility suggests that the release was likely from a side door of the former cleaners, which opened directly to the access road or alley that is used today by Dunkin’ Donuts (the source property) and Starbucks (to the east). To continue the assessment, access will be required within easement (“alley”) and right-of-way areas to the north, east and west . Most of the proposed work will be within the ”alley” which is located between the two businesses (see attached Proposed Location Figures document). As previously noted, the City of Tampa, has stated that the the “alley”, being one-way, cannot be closed for any reason. When the City was told the “alley” was used as an entry and exit by both businesses, APTIM was instructed to prepare and submit the application so the City of Tampa can evaluate the road use properly. In addition to the “alley”, three other locations will be permitted, including the ROWs of West Kennedy Boulevard, South Rome Avenue, and South Dakota Avenue.

Subtask 1-Project Management:

This task includes project management activities associated with the completion of the remaining Subtasks, as calculated in accordance with Contract HW689.

Subtask 2-Preliminary Site Activities and MAGS Testing: No Changes Subtask 3-Soil and Groundwater DPT Sampling Activities: No Changes Subtask 4-Well Installations: No Changes Subtask 5-Repoting: No Changes Subtask 6-WCD#3: No Changes

Subtask 7-Permitting and MOT Development The Subtask 7 scope of work includes further research into the installation of monitoring wells at several offsite easement and ROW locations. The locations researched for access and for application to obtain special use permits will include:

1) The “alley” located on south side of West Kennedy Boulevard between the Dunkin’ Donuts (1622 W. Kennedy Blvd) and Starbucks (1600 W. Kennedy Blvd) restaurants. This location will require Permitting through the City of Tampa.

2) The west ROW of South Dakota Avenue (see attached maps) near the southwest corner of South Dakota Avenue and West Kennedy Boulevard. This location will also likely require Permitting through the City of Tampa.

3) Tentative location in North ROW of West Kennedy Boulevard (Taco Bell and KFC restaurant, 1605 W. Kennedy Blvd). This location will likely require permitting through the Florida Department of Transportation (FDOT).

4) Tentative Location in west ROW of South Rome Avenue near the southwest corner of South Rome Avenue and West Kennedy Boulevard. This location will likely require permitting through the City of Tampa.

The scope of work includes the preparation of proposed well location figures for each location to allow for submittal of Design Tickets to Sunshine811. The design tickets will be utilized to identify obvious utility conflicts and confirm or determine alternate locations for the proposed well installations.

After review of the design ticket plans, a site visit will be performed to white-line final proposed MAGS sample locations and monitoring well locations. After marking, Sunshine811 will be called for utility locates. A second site visit will document any actual utility conflicts and allow utilities and other features of each proposed location to be measured for development of detailed figures, which will be included in the applications. The updated MAGS sample and well location maps will be updated to include the known utility locations.
