Proposed Scope of Work Crystal Cleaners 2316 South Dixie Highway (US-1) West Palm Beach, Palm Beach County, FL 33401 Site Assessment and Reporting Task Assignment DC017B TACO #4 ERIC_5171 (Former Facility No. 509502101) Aptim Environmental & Infrastructure, LLC March 13, 2025

The following scope of work proposed for Task Assignment Change Order (TACO) #4 at the former Crystal Cleaners site is to remove the IDW transportation and disposal (T&D) costs as the drum was not located onsite by the vendor (Erwin Remediation). In addition, APTIM requests a time extension.

Subtask 1-Project Management:

This task includes project management activities associated with the completion of the remaining Subtasks, as calculated in accordance with Contract HW689.

Subtask 2-Preliminary Site Activities and MAGS Testing: No Changes

Subtask 3-Soil and Groundwater Field Activities: No Changes

Subtask 4-Final Well Installations: No Changes

Subtask 5-Reporting: No Changes

Subtask 6-WCD #2: No Changes

Subtask 7-WCD Quarterly Sampling of Deep Well

Erwin Remediation visited the site on the morning of March 12, 2025 and the poly drum containing the IDW purge water collected during the three quarterly groundwater sampling events was not located onsite. The FDEP was notified of the missing drum and the site operator was contacted to confirm the drum was not located on the property. Therefore, the costs associated with the pickup, transportation and disposal of the drum have been removed from the Subtask.

Three (3) quarterly samples of deep well DW-1 have been completed with favorable results from the last two samples. These results will be incorporated into the previously submitted SAR and amended to include a No Further Action recommendation based on the results. A 30-day extension for the TA Completion Date is requested to allow for the processing of TACO #4, pickup and disposal of the IDW and payment of the vendor. The extension will adjust the TA completion to April 30, 2025.

APTIM proposes to perform this scope of work in accordance with the terms and conditions of the contract No. HW689 executed between Florida Department of Environmental Protection and APTIM Environmental & Infrastructure, LLC.

If you have any questions, please contact us for clarification.
