About this file

This document is a Task Assignment Change Order (TACO #2) issued by the State of Florida Department of Environmental Protection (FDEP) to Aptim Environmental & Infrastructure, LLC (APTIM) for environmental services at the Crystal Cleaners site in West Palm Beach, Palm Beach County, Florida. The change order amends the original Task Assignment DC017B under Contract HW689 and reflects modifications to work scope and associated costs. The project is identified as ERIC_5171 and involves a site assessment and reporting project with Patrick McGrane serving as the FDEP Project Manager and Eric Kramer as the Contractor's Project Manager.

TACO #2 authorizes a price increase from $105,576.75 to $109,838.49, representing a total increase of $4,261.75 to reconcile Work Change Directive #2 and actual site assessment costs. The increase addresses adjustments in Subtask 4 (Final Well Installations), where only three monitoring wells were installed instead of four and five drums of investigative-derived waste (IDW) were generated instead of eight, resulting in a cost reduction of $1,002.50. Subtask 6 (WCD #2) costs increased by $5,264.25 to cover additional MAGS testing at wells MAG-4, MAG-7, and MAG-8, plus groundwater sampling as requested by FDEP. The deliverables include an Analytical Results and Field Notes Package due January 31, 2024, and a Site Assessment Report with additional sampling and project management due March 29, 2024. The invoicing structure provides $85,180.16 upon FDEP approval of analytical results and $24,658.33 upon FDEP approval of the final Site Assessment Report, with a completion date maintained at May 31, 2024.

TO: Negotiation File

Crystal Cleaners West Palm Beach, Palm Beach County

ERIC_5171

FROM: Patrick McGrane, Project Manager

SUBJECT: Negotiation Summary Task Assignment Change Order DC017B-2 (HW689)

DATE: Feb 1, 2024

This Change Order authorizes a total cost increase due to additional sampling as described WCD #2 and changes in subcontractor costs. Further information is found in the January 24, 2024, change order email and Scope of Work from Eric Kramer of APTIM.

The previous cost total was $105,576.75. Total costs were increased by $4,261.75 resulting in a new final cost total of $109,838.49. There is a $0.01 rounding error associated with the final Cost

Estimate form. APTIM may invoice according to the included schedule.

Specific negotiated subtasks are as follows:

Subtask 1:

Project Management is automatically calculated as a percentage of total labor in accordance with

Contract HW689.

Subtask 4:

Only three monitoring wells were installed based on preliminary site assessment, versus the four originally planned. Additionally, only five drums of IDW were generated, versus the eight originally estimated. These changes reduced costs by $1,002.50.

Subtask 6:

Subtask 6 rectifies WCD #2. APTIM performed additional MAGS (MAG-4, MAG-7, and MAG-8) and groundwater sampling as requested by the FDEP. The costs associated with WCD #2 and Subtask 6 increased total cost by $5,264.25.

Proposed Scope of Work Crystal Cleaners 2316 South Dixie Highway (US-1) West Palm Beach, Palm Beach County, FL 33401 Site Assessment and Reporting Task Assignment DC017B TACO #2 ERIC_5171 (Former Facility No. 509502101) Aptim Environmental & Infrastructure, LLC January 24, 2024

The following is the proposed scope of work for DC017B Task Assignment Change Order (TACO) #2 for the former Crystal Cleaners site located in West Palm Beach, Palm Beach County, Florida. The proposed scope of work under Task Assignment DC017B TACO #2 is to resolve Work Change Directive (WCD) #2 previously approved for the site, and to reconcile the Task Assignment (TA) for invoicing.

Subtask 1-Project Management:

This task includes project management activities associated with the completion of the remaining Subtasks, as calculated in accordance with Contract HW689.

Subtask 2-Preliminary Site Activities and MAGS Testing: No Changes

Subtask 3-Soil and Groundwater Field Activities: No Changes

Subtask 4-Final Well Installations:

Based on the preliminary assessment reporting, only three wells were recommended to complete the groundwater assessment. Therefore, the Subtask 4 drilling costs were slightly less than anticipated and have been reduced to the actual costs in TACO #2. Also, the cost for the laboratory analysis for the well not installed is being returned to the Subtask. Additionally, only 5 drums of IDW were generated during the field assessment, therefore, the transportation and disposal of three drums, which were not generated, are being removed from the Subtask.

Subtask 5-Reporting: No Changes

Subtask 6-WCD #2

WCD #2 was approved for additional MAGS testing and groundwater sampling requested by the FDEP based on the preliminary findings of the field soil and groundwater assessment. Additional MAGS testing was requested from the MAG-4, MAG-7, and MAG-8 locations. In addition, the deep well was resampled.

The total WCD #2 request was approved for $5,740.00, and is being resolved for a total of $5,264.25, including allowable markups and completion fees. A copy of executed WCD #2 is attached.

Currently the site is on track for completion and submittal of the SAR deliverable on or before the March 29, 2024, due dates, and the previous deliverables have been submitted. The current Task Assignment Completion Date is May 31, 2024.

APTIM proposes to perform this scope of work in accordance with the terms and conditions of the contract No. HW689 executed between Florida Department of Environmental Protection and APTIM Environmental & Infrastructure, LLC.

If you have any questions, please contact us for clarification.
