Task Assignment Change Order Summary

This document is a Task Assignment Change Order (TACO #1) between the State of Florida Department of Environmental Protection (DEP) and APTIM Environmental & Infrastructure, LLC (Contractor), modifying Task Assignment DC009C under Contract HW689. The project involves groundwater monitoring services at the Blue Ribbon Cleaners (former) site located at 160 South U.S. Highway 17-92 in Longwood, Seminole County, Florida. The DEP's Project Manager is Nicole Knutson, and the Contractor's Project Manager is Eric Kramer. This change order specifically addresses reconciliation of Subtask 2 and adjustment of deliverable dates for the overall task.

The change order reduces the total task value from $29,651.03 to $29,361.53—a decrease of $289.50—by eliminating costs for investigative derived waste (IDW) transportation and disposal and removing a poly drum from Subtask 2, as the IDW subcontractor (Erwin Remediation) was unable to locate the IDW container during the site visit. The scope encompasses four quarters of groundwater monitoring for volatile organic hydrocarbons (VOHs), annual monitoring for per- and polyfluoroalkyl substances (PFAS), preparation of an Annual Groundwater Monitoring Report, and preparation of a Site Rehabilitation Completion Report. The task completion date was extended from July 17, 2026 to August 28, 2026 (42 days). Invoice payments are contingent upon DEP approval of deliverables: $10,595.08 upon Q1 approval, $3,202.62 each for Q2, Q3, and Q4 approvals, and $9,158.59 upon final SRCR and Project Management report approval, totaling $29,361.53.

HW689 DC009C-1

DATE: September 10, 2025

This Change Order removes the cost of IDW T&D from Subtask 2. The IDW T&D subcontractor, Erwin Remediation, was unable to locate the IDW container during their site visit. The line items for the poly drum and Erwin Remediation have been eliminated from the Q1 subtask.

Total task amount has decreased from $29,651.03 to $29,361.53. The difference between the revised cost and the original cost is $289.50. APTIM’s time, labor, and subcontractor costs for these activities have been reviewed and are appropriate. The level-of-effort and cost estimate are reasonable for the scope of services. Final deliverable due date has been extended from April 30, 202 to June 29, 202 . Task completion date has been extended from July 17, 202 to August 28, 202 .
