About this file

This document comprises a Task Assignment (TA) and associated Task Assignment Change Orders (TACOs) between the State of Florida Department of Environmental Protection (FDEP) and APTIM Environmental & Infrastructure, LLC for environmental remediation services at Carmon's Cleaners (formerly) located at 7980 North Tamiami Trail, Sarasota, Florida. The original Task Assignment DC005C was issued under Master Contract HW689 with an initial completion date of July 16, 2024, and has been subsequently modified through two change orders (TACO #2 and TACO #3) signed in September and December 2024.

The contract authorizes APTIM to install and operate a Soil Vapor Extraction (SVE) remediation system at the site, with deliverables including SVE system mobilization and installation, project management, system startup and one-month operation, utilities, and groundwater sampling. The original task value was $66,476.98, which was extended through TACO #2 to $79,720.77 with a new completion date of March 26, 2025, and further amended through TACO #3 to reduce costs to $76,084.54 due to reduced equipment usage, removal of undelivered portable toilet services, and lower construction debris disposal costs, with a final completion date extended to May 23, 2025. Cost reimbursement for utilities is capped at $650.00 plus a $245.00 handling fee, and the contract includes detailed level-of-effort worksheets documenting labor categories, equipment rentals, laboratory services, subcontractor costs, and per diem expenses across five subtasks.

Carmon’s Cleaners

FROM: Nicole Knutson, Project Manager

SUBJECT: Negotiation Summary

HW689 DC005C-3

DATE: December 2, 2024

This Change Order reduces equipment and subcontractor costs for SVE installation at Carmon's Cleaners. Some pieces of construction equipment were not utilized, so the prices were adjusted accordingly. Additionally, the portable toilet was not delivered, so this subcontractor cost was removed. APTIM also generated less construction debris than anticipated. Consequently, the C&D subcontractor costs and container costs have been reduced.

Total task amount has reduced by $3,636.23. The original price was $79,720.77 and the amended price is $76,084.54. APTIM’s time, labor, and subcontractor costs for these activities have been reviewed and are appropriate. The level-of-effort and cost estimate are reasonable for the scope of services. Final deliverable due date has been extended from February 17, 2025 to March 17, 2025. Task completion date has also been extended from March 26, 2025 to May 23, 2025.

Proposed Scope of Work Carmon’s Cleaners (Former) 7980 North Tamiami Trail Sarasota, Manatee County, Florida 34243 Continuing SVE Installation and Startup Proposal Task Assignment DC005C TACO #3 ERIC_4903 (Former FAC ID No. 419700833) APTIM Environmental & Infrastructure, LLC (APTIM) November 21, 2024

The following proposed Task Assignment Change Order (TACO) #3 scope of work for the Carmon’s Cleaners TA DC005C has been prepared to reconcile Subtask 2, which includes the installation of the SVE remediation system.

Subtask - 1: Project Management Subtask 1 includes project management activities associated with the completion of Subtasks, as calculated in accordance with Contract HW689.

Subtask 2: SVE System Installation The SVE system has been installed and is waiting for the groundwater elevation to lower before remedial system startup can be performed. Although the scope of approved work did not change there are cost adjustments to Subtask 2 that include the cost removal of construction debris. The quantity of construction debris was less than anticipated so the material was containerized and transported to the Winter Garden for disposal. Therefore, the disposal cost is not required. Although a portable toilet was ordered, it was not delivered so this cost is removed from the Subtask. In addition, the costs associated with the construction equipment rentals has been adjusted to the actuals. Some equipment was not used due to the limited space onsite requiring portions of the work to be performed by hand. Although a crew of 4 was used, some of the onsite technicians did not require hotel and per diem, as they mobilized from Tampa. Lastly, the carbon unit was slightly less than the proposed costs and only one T&D bag for waste was used (as opposed to two proposed), therefore the associated cost adjustments were made to Subtask 2.

Subtask 3: System Startup & Operation: No Changes

Subtask 4: Utilities One Month: No Changes

Subtask 5: Groundwater Sampling: No Changes

The current Task Assignment (TA) Completion Date is March 26, 2025, however, a slight adjustment to the completion date is warranted due to the uncertainty of the timing for the groundwater elevation to drop below the design operational conditions. As you know, the recent storms have raised the groundwater elevation which prevents SVE operation. Therefore, remedial system startup has been delayed until the groundwater recedes to normal conditions. To allow for the startup, subsequent operation for one month and payment of the initial utility bills prior to the TA ending, a 60-day extension is requested which will adjust the TA Completion date to May 23, 2025.

APTIM proposes to perform this scope of work in accordance with the terms and conditions of the DEP

Contract No. HW689 between the Florida Department of Environmental Protection and APTIM dated December 21, 2021. APTIM will begin the proposed scope of work upon issuance of the Task Assignment Notification.
