Task Assignment Change Order Summary

This is a Task Assignment Change Order (TACO #1) issued by the State of Florida Department of Environmental Protection (FDEP) to Aptim Environmental & Infrastructure, LLC, dated February 9, 2024. The change order amends Task Assignment DC018B under Contract HW689 for environmental assessment work at the Deluxe Cleaners & Laundry site located at 1622 West Kennedy Boulevard, Tampa, Hillsborough County, Florida (Site ID: ERIC_4696). The FDEP project manager is Patrick McGrane, and the contractor's project manager is Eric Kramer.

The change order authorizes a cost increase from $88,766.53 to $92,803.89 (an increase of $4,037.37) and extends the task completion date from May 31, 2024 to September 27, 2024 (a 119-day extension). The cost adjustment reconciles Work Change Directives (WCDs) #1 through #5 and adjusts for expected versus final subcontractor costs related to soil and groundwater sampling, SPLP extraction analysis, and additional labor costs for night work activities. The deliverables include an Analytical Results and Field Notes Package due March 22, 2024; Well Installations deliverable due May 31, 2024; and a Site Assessment Report & Project Management deliverable due July 26, 2024. Invoice payments are structured by FDEP approval of each deliverable, with total amounts of $56,778.89 for analytical results, $18,938.39 for well installations, and $17,086.61 for the site assessment report.

SUBJECT: Negotiation Summary Task Assignment Change Order DC018B-1 (HW689)

DATE: Feb 1, 2024

This Change Order authorizes a total cost increase and time extension that reconciles WCD #1-5. Further information is found in the January 22, 2024, change order email and Scope of Work from Eric Kramer of

APTIM.

The previous cost total was $88,766.53. Total costs were increased by $4,037.37 resulting in a new final cost total of $92,803.89. There is a $0.01 rounding error associated with the final Cost Estimate form.

Subtasks 2, 3, 4, and 6 deliverable due date was moved from 1/31/2024 to 5/31/2024. Subtasks 1 & 5 deliverable due date was moved from 3/29/2024 to 7/26/2024. The Task Assignment completion date was extended from 5/31/2024 to 9/27/2024, a 119-day extension. APTIM may invoice according to the included schedule.

Specific negotiated subtasks are as follows:

Subtask 1:

Project Management is automatically calculated as a percentage of total labor in accordance with Contract HW689.

Subtask 2:

The depth to groundwater was higher than estimated, meaning deeper soil and MAGS samples were not needed. This change was reflected in fewer laboratory analysis needed. WCD #4 (SPLP extraction and analysis for four samples above PCE leachability SCTL) is resolved in this Subtask.

Subtask 3:

Costs were reduced in this Subtask as well due to depth to groundwater occurring higher than expected.

WCD #1 (SPLP extraction and analysis for two samples above PCE leachability SCTL), WCD #2 (SPLP extraction and analysis for five samples about PCE leachability SCTL), and WCD #5 (additional costs associated with subcontractor prices for night drilling) are resolved in this Subtask.

Subtask 6:

Due to some work having to be completed at night, additional labor hours and PPE were required, increasing costs by $6,937.37. This change reconciles WCD #6.

Proposed Scope of Work Deluxe Cleaners & Laundry 1622 West Kennedy Boulevard. Tampa, Hillsborough County, FL 33606 Site Assessment and Reporting Task Assignment DC018B TACO #1 ERIC_4696 (Former Facility No. 299500411) Aptim Environmental & Infrastructure, LLC January 22, 2024

Deluxe Cleaners DC018B Task Assignment Change Order (TACO) #1 was prepared to reconcile the initial field operations during site assessment of the former cleaners. Specifically, TACO #1 was prepared to reconcile Subtasks 2 and 3 of the Task Assignment (TA) for invoicing. TACO #1 also resolves Work Change Directive (WCD) numbers WCD#1, WCD#2, WCD#3, WCD#4, and WCD#5, which were approved during the performance of Subtasks 2 and 3. Copies of the executed WCDs are attached for reference.

Subtask 1-Project Management:

This task includes project management activities associated with the completion of the remaining Subtasks, as calculated in accordance with Contract HW689.

Subtask 2-Preliminary Site Activities and MAGS Testing:

During the MAGS sampling event, the depth to groundwater was anticipated to be observed at or below the depth of 6 ft, however, the depth to groundwater at the time of sampling was approximately 5 ft bls.

Therefore, the deeper samples (proposed at 6 ft bls) were not collected during the MAGS testing and soil sampling. For that reason, the analytical costs have been reduced to include only the samples collected and analyzed. Subtask 2 also resolves WCD #4, which was approved on October 12, 2023, to allow for SPLP extraction and VOHs analysis for four (4) samples displaying results above the PCE leachability SCTL. WCD#4 was approved and is being resolved for a total of $512.00.

Subtask 3-Soil and Groundwater DPT Sampling Activities:

During the soil sampling event, the depth to groundwater was anticipated to be observed at or below the depth of 6 ft, however, the depth to groundwater at the time of sampling was approximately 5 ft bls.

Therefore, the deeper proposed samples (6 ft bls) were not collected during the soil assessment.

Similarly, due to refusal during DPT groundwater sampling at 24 ft bls, the proposed 30 ft bls samples could not be collected. Therefore, the costs associated with soil and groundwater samples not collected have been removed from the Subtask.

Subtask 3 also resolves WCD #1, which was approved on September 26, 2023, to allow for SPLP extraction and VOHs analysis for two (2) samples displaying results above the PCE leachability SCTL.

WCD#2 was approved and is being resolved for a total of $256.00.
