Document Summary

This is a Task Assignment Change Order (TACO No. 1) issued by the State of Florida Department of Environmental Protection (FDEP) under Contract No. HW687 with Terracon Consultants, Inc. The change order amends Task Assignment DC002B for remediation work at the Village Cleaners site (ERIC_5205) located at 2100 45th Street, West Palm Beach, Florida. The work is overseen by FDEP's Dean Cox and Terracon's Donna Cline as project managers.

The change order authorizes $9,715.84 in additional costs for SVE (soil vapor extraction) system blower replacement and telemetry installation, bringing the total task assignment value to $19,541.89, which includes $2,000.00 in cost reimbursement. The fixed unit rate component increases from $7,826.05 to $17,541.89, while cost reimbursement remains at $2,000.00. The task completion date is extended from May 31, 2023, to October 31, 2023, providing an additional 153 days. Deliverables include photos of before and after blower and telemetry installation, monthly O&M checklists and field notes, lab reports for two system sampling events, O&M tables updates, and system evaluation recommendations. Invoice payments are structured in three tranches: $9,351.63 for blower and telemetry installation approval, $8,110.26 for O&M work approval, and $2,080.00 for utility cost reimbursement, totaling $19,541.89. The replacement blower was selected from RSI Inc. at $5,700.00, representing the lowest of three competitive bids evaluated.

TO: HW687 Contract File FROM: Dean Cox, Contract Manager SUBJECT: DC002B-1 Village Cleaners (ERIC_5205) SVE Blower & Telemetry Installation

DATE: 2/24/23

This TACO adds $9,715.84 for SVE system blower replacement and telemetry installation. The total cost is $19,541.89 with $2,000 in cost reimbursement. The cost of this TA was negotiated - the total cost is $321.08 or 3% less than originally proposed.

The purpose of this TA is to repair the SVE so it will operate, and to upgrade its telemetry components to make telemetry fully functional. These repairs are subcontracted by Terracon and Terracon will oversee their completion. The ESE Waste to Water bid is the Optional Blower Swap bid. They also included the cost of a new SVE system for reference. RSI was by far the low bid and their bid includes blower replacement and telemetry upgrade.

This SVE system is not expected to be needed at this site much longer, but it will be needed soon at another Terracon site. This TACO adds this work to an existing SVE system O&M TA. Site access has been confirmed current.

Subtask 3 – Utilities, No proposed change at this time, however the power bills will be followed closely, and a change order may be required if the $2000 budget is exceeded.

Subtask 4 - This is a new subtask to authorize costs for the installation of a replacement SVE Blower, contactor overload, Dwyer low flow switch, blower relief valve, SVE breaker, telemetry upgrade and 60-amp breaker. The costs to perform the additional work are $9,715.84 which includes labor, T/L, PM, subcontractor costs of $5700.00 and handling fees.

See attached LOEs for details

RE: Village Cleaners, ERIC_5205, SVE System

A summary of the SVE system history is below and Terracon’s recommendations are summarized below. As of 6/14/22, the system remains non-operational after several attempts to trouble shoot and repair the system.

04/12/2022 System Startup Visit

Terracon Initially visited the site on 04/12/2022. The skid system 140347 was inherited from Tetra Tech and restarted. The initial startup was conducted, and the system started without incident. Flow and vacuum readings were collected, and analytical samples were also collected. The system was left with all (4) legs running.

05/12/22 System Visit

Terracon visited the site on 05/12/2022. The system was running on arrival. Flow, vacuum measurements were collected. No analytical samples were also collected. SVE-3 and SVE-4 were shut down during this event and SVE-1 and SVE 2 were left open to extract.

06/14/2022 System Visit

Terracon visited the site on June 14, 2022 and observed the system to be shut down. The

SVE main breaker was tripped. The onsite Tech performed routine troubleshooting; attempt to reset/restart failed. Tested voltages and ground. Disconnected motor leads-test SVE breaker. Determined motor issues: check motor rotation, ohm check passed each leg and to ground, earth ground within limits. Next tested and determined that blown capacitors may have caused this problem. The capacitors were ordered/picked up and a TA to replace these capacitors was requested. The TA to purchase and replace the capacitors was received on

10/18/2022.

09/26/2022 System Visit-Hurricane Ian Preparedness

Terracon visited the site on September 26, 2022, as part of the Hurricane Protocol. The

Mains circuit breaker were turned off and the system disconnect was closed. Site inspection revealed enclosure to be secure as possible. The overhead power lines and the service drop to the system have overgrown tree branches surrounding them.

11/1/2022 System Repair Visit

Terracon visited the site and replaced blown motor capacitors. After installation, the SVE

System would not run in Hand or Auto, tripping the system’s 60-amp breaker. The technicians performed electrical troubleshooting and determined that there is correct voltage to the motor. However, the motor is drawing overrange amps and tripping the system’s 60-amp breaker. The motor overload and SVE circuit breaker are not tripping.

Rechecked capacitors-passed, motor leads proper voltage and open to ground, but motor earth ground is anomalous unlike previous site visit in June. Indicating an occurrence of possible power surge and/or possible storm damage. Additionally, a visual reinspection at the blower electrical connections, it was determined corrosion and overheating likely contributed to blower failure.

Terracon recommends

1.Replacing the Blower AMETEK DR555K58 81098

2. Replacing the SVE Breaker on/off

3. Replacing the Contactor and Overload

4. Replacing the wiring from the Contactor Overload block to the Blower Motor.

5. Replace 60-amp circuit breaker

6. Replace fail safes; vacuum and pressure switches, tubing.
