TO: HW687 Contract File FROM: Dean Cox, Contract Manager SUBJECT: DC027A Carriage Trade Dry Cleaners (ERIC_4799) Site Assessment
DATE: 1/6/2023
This TA authorizes costs associated with site assessment & reporting, based on the revised Terracon TA proposal dated 12/16/22 and cost estimate agreed upon 12/16/22.
The total cost of this TA is $117,861.25 and all costs are fixed unit rate. This TA was negotiated - the total cost is $4,106.17 or 3.4% less than originally proposed.
CEF task elements are accurately enumerated in each LOE subtask, with associated descriptions in the Deliverables & Invoicing Schedule. Deliverable due dates are farther out than usual to account for availability of subcontractors and a simultaneous assessment at another Tampa Bay area site (using the same office/personnel). Site access was recently acquired.

Proposal for Site Assessment Workplan Investigation
Carriage Trade Dry Cleaners, ERIC_4799 ■ Tampa, FL
October 31, 2022, Revised December 16, 2022 ■ Terracon Proposal No. PH4227412
SCOPE OF SERVICES
Subtask 1 – Project Management
Ricker Lamphier, E.I. will serve as the Project Manager during the field investigation. Donna Cline, P.E., and Dan Nedvidek, P.G., will be involved throughout the project, serving as Authorized Project
Reviewers.
Subtask 2 - Soil Sampling and Modified Active Gas Sampling (MAGS)
Pre-Task Planning will also be performed to prepare for field investigations. Terracon will prepare a site specific health and safety plan.
Utility Locate
Prior to conducting the soil sampling, monitoring well installations, and DPT groundwater profile assessment, Terracon will subcontract the services of a subsurface geophysical survey company to identify potential subsurface utilities or obstructions. GeoView was the low bid and estimates that the work will take one day to complete. The geophysical survey bids are attached. As a cost saving measure, no additional mobilization costs will be incurred as the subsurface utility survey will begin the same day as soil sampling activities.
Soil Investigation
Terracon will collect soil samples via hand-auger from 6 MAGS locations as described in SAWP
(Terracon 2022). Terracon will collect soil samples from 0.5, 2, 4, and 6, feet (ft) below ground surface (BGS), or to the top of the water table, via hand auger at each soil boring location. Terracon has contracted with Action Environmental to perform the concrete coring and complete the soil borings. Terracon personnel will also field screen the soils with an OVA-PID, document the lithology observed at each of these soil boring locations, and will record the Global Positioning System (GPS) coordinates of completed borings. Based on the number of proposed boring locations and depth intervals, Terracon will collect up to 24 soil samples for “full list” Volatile Organic Compounds via EPA
Method 8260 and Synthetic Precipitation Leaching Procedure (SPLP) via EPA 1311. The SPLP samples will remain on hold until the results from the full list VOC results are received. SPLP extraction and
VOC analysis will be performed with FDEPs discretion on those samples that report concentrations exceeding the respective Soil Cleanup Target Level. This additional analysis will be considered a Work
Change Directive/Task Assignment Change Order. The approximate sampling locations, depth intervals, matrices, analytes and laboratory methods, rationale, and screening criteria are summarized in the SAWP (Terracon, 2022). As detailed in the SAWP, all (6) soil borings will be completed as
MAGS wells. The MAGS wells are proposed to a depth of 6 ft BGS, with 5-ft of 2-inch diameter 10 slot screen. Each well will be completed flush to ground surface. The interior MAGS wells will be temporarily covered with a neat cement grout at surface to allow for their later use in a Soil Vapor
Extraction system. The exterior MAGS wells will be completed within an 8-inch manhole set within a
2x2 ft pad.
Modified Active Gas Survey (MAGS)
The MAGS test will be performed over a period of 2 days.
Subtask 3 - Piezometer and Interior Well Installation, Lithological Profiling, Groundwater
Profile Assessment
Piezometer and Interior Well Installation, Lithological Profile
Four piezometers will be installed at the site, screened 4 to 14 ft BGS (based on an estimated depth to water of 6-7 ft BGS), the piezometers will be surveyed to assess groundwater flow direction, and the lithological boring will be installed within an upgradient hydraulic location. The lithological profile boring will be advanced to an estimated depth of 50 ft BGS, and Terracon’s geologist will be on-site to describe the lithology.
The interior monitoring well will be hand-installed with a 5 ft screen to a depth of 10 ft bgs.
DPT Groundwater Profile Assessment
The objective of the DPT groundwater profile assessment is to evaluate the extent and magnitude of drycleaning contaminants of interest within the on-site property boundary. Terracon anticipates advancing up to 7 DPT groundwater borings and collecting groundwater samples from the following intervals: 6 to10, 16 to 20, 26 to 30, 36 to 40, 46 to 50, or until refusal at each location. The groundwater samples will be submitted to the laboratory for analysis of full list VOCs. As detailed in the SAWP, the VOH (short list) will be requested for 24-hour turnaround time, for screening purposes, to allow for the horizontal and vertical assessment of groundwater. This assessment data will be used to evaluate the location and the screen interval of the proposed monitoring wells. Daily updates of activities will be uploaded within 24 hours on the Terracon website Compass, to allow FDEP access to the site activities, and groundwater analytical results.
Based on the number of proposed boring locations and depth intervals, Terracon will collect up to 35 groundwater samples. Additional details are summarized in the SAWP (Terracon, 2022).
Subtask 4 -Monitoring Well Installation and Monitoring Well Sampling
Monitoring Well Installation
Terracon will oversee the installation of six intermediate and one deep monitoring wells. The proposed locations for the monitoring wells are shown in the SAWP (Terracon, 2022). Terracon will advise with
FDEP on the proposed locations, and the monitoring wells will not be installed unless the locations are agreed on by FDEP.
It is anticipated that the proposed intermediate monitoring wells will be installed to a depth of approximately 30 ft BGS, with 10 ft pre-packed screen. The deep monitoring well is proposed to a depth of 50 ft BGS, with 5-ft of pre-packed screen. Each well will be completed flush to ground surface in an 8-inch diameter manhole set within a 2-ft by 2-ft by 4-inch concrete pad. Monitoring wells will be developed until the water is visibly free of sediment.
Monitoring Well Sampling
Groundwater samples will be collected from the 4 piezometers, and the 6 intermediate, 1 deep, and 1 shallow interior newly installed monitoring wells, in accordance with the FDEP SOPs. Prior to monitoring well sampling, Terracon will collect depth-to-water measurements. The analytes and laboratory methods are summarized in the SAWP (Terracon, 2022), and will include full list VOCs and
PFAS analysis.
As a cost saving measure, no additional mobilization costs will be incurred as the specific purpose survey by SurvTech Solutions, Inc. will begin the same day as groundwater sampling activities.
Subtask 5 –Investigation Derived Waste
Decontamination activities will take place at a designated staging and laydown area. The soil and water IDW will be containerized in 55-gallon drums and staged on-Site at a designated location. It is estimated that 10 drums of IDW will be generated in Subtasks 2, 3 and 4, and the transportation and disposal costs for the drums are included under Subtask 5.
Terracon recommends subcontracting Erwin Remediation, Inc. (Erwin) for IDW transportation and disposal. Terracon will provide oversight of the IDW pickup, and sign waste manifests as an agent of
FDEP.
Subtask 6 –Site Assessment Report
Terracon will prepare a Site Assessment Report that will detail and summarize the groundwater sampling activities, field observations, and results of soil and groundwater analysis and will include data tables and exhibits of groundwater flow and soil and groundwater concentrations. Conclusions and recommendations will include a summary of results and recommendations for future sampling or remedial activities.
SCHEDULE
It is estimated that the drilling activities may be completed within a 10-day period. Due to the expedited schedule while performing work at an active commercial drycleaning facility, Terracon recommends having three Terracon personnel on-site. Daily updates of activities will be uploaded within 24 hours on the Terracon website Compass, to allow FDEP access to the site activities, and groundwater and soil analytical and investigative results. Terracon anticipates the deliverable of site assessment activities within 30 business days of receipt of analytical results.
CLOSING REMARKS
Terracon services will be conducted in accordance with the terms and conditions, contained within DEP
Waste Cleanup Program (WCP) Contract No. HW687, executed January 5, 2022. It is understood that a Task Assignment will be issued to authorize us to perform this work.
We appreciate the opportunity to provide this proposal and look forward to working with you on this project. If you should have any questions or comments regarding this proposal, please contact either of the undersigned at 813-221-0050.
Sincerely, Terracon Consultants, Inc.

"PFAS-Free" water source: Samples of the driller's shop water will be required to be analyzed for PFAS by the FDEP Laboratory prior to use on-site. If water has been previously tested and approved for use please provide letter or copy of lab data with this solicitation.
General Information: Terracon is soliciting quotes from three (3) Florida licensed well contractors, on behalf of the FDEP Waste Cleanup Program, for the following scope of services. The driller will be responsible for all necessary permits, public utility locates, equipment and personnel to complete this scope of work. The driller should be aware that increases in scope may occur once the work has begun.
Preconstruction Meeting
Drillers Signature: Carl Leonhardt
License No.: 9521

Decontamination: All downhole tools will be decontaminated prior to and between each sample interval location (i.e. every foot during hand auger advancement and every five feet during macrocore advancement). Specific decontamination procedures include alconox scrub and steam wash and apply for all drilling equipment and downhole tools (e.g., tremie pipe, hoses).
Decontamination fluids and solids will be containerized separetly. Offeror’s equipment shall arrive clean and free of soil or dirt from its previous work location.
Investigation Derived Waste (IDW) and Trash: Soil cuttings, decontamination water and solids, and development water (IDW) generated during well installation will be drummed. The
Driller will supply a sufficient number of FDOT-approved 55-gallon steel drums to contain all IDW generated during site activities. The Offeror will be required to consolidate the IDW drums into one central location.
Drilling Conditions: The site is an active facility, operating as a nail salon and shopping plaza. The site has limited space. It should be assumed that potable water and electricity are not available. The depth to water is estimated at 7 feet bgs.
Health and Safety: Work is expected to be perfomed in Level D personal protective equipment. The primary contaminats of concern at the site are chlorinated solvents, tetrachloroethene. Proper safey procedures to minimize contact with contaminated site media is required. Site personnel are required to be 40-hour OSHA trained.
