◼ Include laboratory analysis for the biostimulation remedial design parameters: total iron, dissolved iron, manganese, sulfate, sulfide, total organic carbon, methane, ethane, ethene, and alkalinity to monitoring wells MW007D-2, MW003D-2, MW003S, MW003I, MW013D and MW011D.
◼ Collect water levels from 21 existing wells, 3 existing piezometers, 8 newly installed wells, and 2 from Former Imperial Cleaners.
Proposal for MW Install, GW Sampling and SSA Report
Village Laundry, ERIC_4743 ■ Tampa, FL
July 6, 2022 ■ Terracon Proposal No. PH4227262
◼ Review data and prepare a Supplemental Site Assessment Report including updated tables and groundwater flow and concentration exhibits.
See attached LOE for details
Schedule
Based on driller availability, Terracon is proposing to install the new wells from August 1-7, 2022. Groundwater sampling is proposed for August 10-
12, 2022. The Supplemental Site Assessment report will be submitted within 20 business days of receipt of analytical results.
Compensation See attached CEF for details.
SITE RECOMMENDATIONS
Recent site assessment activities are summarized in a Tetra Tech Groundwater Monitoring
Report dated April 4, 2022. The report documented the third sampling event for the site and the first event to occur following the installation and startup of an SVE system in April 2021.
The report concluded that additional horizontal and vertical delineation was required. Based on this report, a review of historical data, and discussions with FDEP, Terracon recommends installing three (3) vertical delineation wells along a northwest to southeast axis through the site. The proposed vertical delineation wells will be installed using a sonic drill rig with temporary override casings set at various depths based on previous assessment data.
Continuous coring and lithological logging data will be collected to address data gaps identified in previous reports. These wells will be terminated at 70 feet below ground surface
(bgs) with a screen interval from 65-70 bgs. Two wells will be installed to provide additional source zone data. One well will be located adjacent to the existing MW007 cluster and one near the existing MW003 cluster. Both will be terminated at a maximum depth of approximately 55’ bgs (for driller quote purposes). Conversations with Breck Dalton suggested a screen interval of 45-50 ft for MW007D2 and 40-45 ft for MW003D2. Final depths and screen intervals will be determined on-site using lithological data collected during drilling operations. Three additional wells will be installed off-site to the northeast, southwest, and northwest to provide confirmation of delineation assumptions. These wells will be terminated at 40’ bgs with a screen interval of 30-40’ bgs.
Terracon will collect site-wide groundwater samples following the installation of the new monitoring wells. Groundwater samples will be collected from twenty-one (21) existing wells, 8 newly installed wells. At the recommendation of FDEP samples will be collected from 2 wells in the MW006 cluster associated with the Imperial Cleaners site during this monitoring event. Additionally, the monitoring wells MW007D-2, MW003D-2, MW003S, MW003I, MW013D and MW011D will be analyzed for the biostimulation remedial design parameters:
total iron, dissolved iron, manganese, sulfate, sulfide, total organic carbon, methane, ethane, ethene, and alkalinity.
DELIVERABLES
Terracon will prepare a Supplemental Site Assessment Report encompassing both the monitoring well installation and sampling event at the conclusion of field activities. The report will re-evaluate the conceptual site model with regards to horizontal and vertical delineation
Proposal for MW Install, GW Sampling and SSA Report
Village Laundry, ERIC_4743 ■ Tampa, FL
July 6, 2022 ■ Terracon Proposal No. PH4227262 and provide recommendations for additional services. Field notes, monitoring well construction diagrams, soil boring logs, lithologic and contaminant cross sections, and updated analytical tables and exhibits will be included in this report.
SCHEDULE
It is estimated that installation of the new monitoring wells will take seven (7) business days to complete with an additional three (3) days required for groundwater sampling. Terracon proposes to submit the Supplemental Site Assessment Report within 20 days from the receipt of laboratory analytical data. Based on driller availability, Terracon is proposing to install the new wells from August
1-7, 2022. Groundwater sampling is proposed for August 10-12, 2022.
CLOSING REMARKS
Terracon services will be conducted in accordance with the terms and conditions, contained within DEP
Waste Cleanup Program (WCP) Contract No. HW687, executed January 5, 2022. It is understood that a Task Assignment will be issued to authorize us to perform this work.
We appreciate the opportunity to provide this proposal and look forward to working with you on this project. If you should have any questions or comments, please contact either of the undersigned at

Oliver, Thanks for the quick turn around on this. I have a few questions that I’m sending to each respondent as we start comparing bids since there was a lot of variation in the responses and we don’t want to mess around with trying to get a change order approved during the site work. Please take a look at your quote and make sure you’re good to go on the following:
Confirm your number of days on site. Make sure it’s adequate to complete the scope without a change order.
Cascade believes the scope of work can be completed in 10 days on site Confirm worst case number of drums.
Worst case number of drums would be 50 Drum staging. Please include costs for a skidsteer if you haven’t already. Space for drums is limited and we’re going to have trouble controlling a large area of the parking lot for staging.
Cascade has included a BOBCAT skid steer in our original quote.

Please indicate if (if you haven’t already) if you’re planning on tapping a hydrant or bringing a full tank with you. Do not rely on site water being available.
Cascade will contact the local utilities and arrange a water source close to the site if awarded Confirm the number of well permit applications. Everyone came back with a different number.
From looking at the map, it looks like (4) addresses would need to be used for pulling permits. 4 permits in total would be needed.
Please note (if not already detailed in a line item) if decon equipment – steam cleaner, etc.
are built into your daily rates. Please add costs if not.
Cascade has included a steam cleaner in our original quote.
Thanks, Dan

Dan, Attached is the proposal for the 8 wells at the laundromat in Tampa. I noticed the depths of the wells in the email were just a few feet different than the depths on the map provided. I went with the deeper info. If they go shallower, you will be billed only what we drill. If you have any questions about the bid, feel free to call me anytime.
Thank you for allowing Cascade to bid on your project.

Proposal
This document shall serve as a proposal for work to be performed by
GeoView, Inc. (GeoView) for Terracon.
Description of Site
The project site is located at 10049 North Dale Mabry Highway in Tampa, Florida. The purpose of the geophysical survey will be to determine the location of underground utilities within a 10 feet radius of 8 proposed borings.
Scope of Work
The geophysical survey will be conducted using ground penetrating radar
(GPR) and conventional electromagnetic (EM) pipe and cable locators. The GPR survey will be conducted using either a Mala and/or a GSSI GPR system. The appropriate GPR antenna configuration and time range and processing parameters will be determined in the field by the project manager.
The EM survey will be performed using a Subsite 95R and/or a
Radiodetection RD (8000, 8100 or 8200) system. As appropriate, the EM survey will be conducted using both passive and active modes. The passive mode is used to find cables and conduits carrying live power (50-60 Hz) and communication
(29-33Hz) signals. For the active mode survey, a current is transmitted directly or broadcast into using electromagnetic induction a metal pipe or conduit and the path of the pipe/conduit is identified by following the signal with a portable receiver.
The active mode is used to trace out cast iron pipes, metal water and sewer lines, utilities that have trace wires, and inactive power and communication lines.
Presentation of Results
The location and estimated depths of underground utilities or any other identified structures shall be indicated on the ground surface using spray paint or other methods as specified by Terracon. No report shall be prepared.
Compensation
Cost to complete the survey will be $800. If an scaled AutoCAD site map is required to document the study results, an additional fee of $200 will be added.
This price is inclusive of all charges associated with the project. Unless otherwise agreed upon, GeoView shall be compensated for all services within 30 days of invoice date. GeoView shall be paid in full regardless of whether the results of the geophysical survey are what Terracon anticipated. If it is determined during the survey that the geophysical survey will not achieve the objectives of the project, Terracon will immediately be notified. If a decision is made to discontinue the survey, only charges for time and materials costs to that point will be submitted.
Requirements of Client
Terracon will provide a scaled map and digital file, if available, of the project site showing the boundaries of the project site, areas of specific concern and pertinent landmarks. Terracon will also supply all available information regarding the presence and history of underground utilities at the project site (see limitations for further discussion). Unless otherwise requested, Terracon will be responsible for the coordination of site access, traffic control, clearing of onsite obstructions or any other logistical consideration necessary to conduct the survey.
General Description of Each of The Geophysical Methods
Ground Penetrating Radar (GPR)
The GPR survey will be conducted using a Mala or GSSI GPR system. The survey will be conducted with either a 500, 400 or 250 mega-Hertz antenna. The appropriate antenna will be determined in the field by the project manager. GPR will be able to provide both the position and estimated depth of both metallic and non-metallic underground utilities.
The ability of GPR to resolve a particular utility will be controlled by the following factors: 1) Depth of penetration of the GPR signal, 2) Depth of burial and diameter of the underground utility and 3) Electrical contract between the underground utility and the surrounding soils and 4) The complexity of the utility layout at the site. Due to one or a combination of these factors the GPR may not be able to identify all the utilities at the site. The diameter or composition of utilities cannot be accurately determined using GPR, though can be estimated. Estimates of burial depth are typically accurate to within +/- 20 percent, more accurate depths are possible if invert elevations for some on-site utilities are known.
Conventional Electromagnetic Pipe and Cable Locators
A Radiodetection System is used to locate underground utilities using conventional means. The system consists of a dual-function receiver and transmitter. The receiver can be operated in two modes: active and passive. In the passive mode the receiver detects the presence of underground utilities that are energized by a 60-Hertz or other known frequency. These utilities can either be those actually carrying electrical power or those utilities that are both metallic and sufficiently close to the electrical lines to have an electrical field induced within them.
In the active mode, the transmitter deliberately induces an electrical current into the utility. The frequency of this field can be varied from 512 Hertz to 200 kilohertz. The electrical field is induced using a transmitter which is either directly attached to the utility, placed on or above the utility or attached to an induction coil which is placed around the utility. Depths of underground utilities can be determined when the EM equipment is being used in the active mode. Depths are typically accurate to within 10-15 percent of the total utility depth.
Limitations
GeoView shall only be liable for the successful locating of underground utilities or any other under ground structures whose presence is both known and indicated on site maps to be provided prior to the performance of the geophysical survey. Every attempt will be made to locate both known and unknown utilities within the survey area. However, GeoView will only be liable for utilities whose presence is known prior to the survey.
The objective of the geophysical survey is to determine the presence and configuration of underground utilities. However, the geophysical response of these features may be highly complex depending upon on the physical characteristics of the underground utilities, the electrical properties of the surrounding soils, the depth of burial of the utilities, and the proximity of the utilities to each other.
GeoView shall conduct the geophysical survey using the most “up-to-date” geophysical equipment in a manner consistent with the level of care and skill ordinarily exercised by members of the geophysical profession practicing in the same locality under similar conditions. GeoView’s utility locating personnel are all certified GPR and conventional pipe and cable locator operators with extensive in-house training.
When Terracon is working around indicated utilities, a minimum of 2 feet clearance from either side of the marked utility will be maintained.
