The total task assignment value is $29,933.46, with labor comprising $18,529.06, travel costs of $720.00, subcontractor services totaling $5,296.50, laboratory fees of $946.00, field expenses of $2,589.00, and a completion fee of $1,852.90. Deliverables are scheduled for completion on August 8, 2025 (Subtask 2), September 12, 2025 (Subtasks 1 and 3), and October 17, 2025 (Subtask 4), with corresponding invoice amounts of $9,416.83, $17,046.97, and $3,469.66 respectively. The estimated labor allocation is 156.80 hours across multiple staff categories including senior scientists, senior engineers, and CADD specialists. Subcontractor costs include drilling services from Ambient Technologies, Inc. ($3,830.00), IDW disposal through Petrotech Southeast ($985.00), and waste transportation and disposal services from multiple vendors including Clean Earth Environmental Solutions and Republic Services, with pricing contingent on waste profile approval.

Waste Cleanup Contract No. HW687 Terracon Proposal No. PH4257108
Dear Mr. Sostre:
Terracon Consultants, Inc. (Terracon) appreciates the opportunity to submit this proposal to the Florida Department of Environmental Program (FDEP) Waste Cleanup Program (WCP) to conduct monitoring wells installation and combinate volatile organic halogenated (VOHs) and per-and polyfluorinated substances (PFAS) groundwater sampling for Fabricare Cleaners and Laundry (ERIC_4741) site located in Tampa, Florida.
Scope of Services
Field preparation, update health and safety plan for monitoring well installation and combined PFAS and VOH groundwater sampling.
Mobilization to the site for the installation of 3 offsite groundwater monitoring wells located southeast of the Site to delineate the PFAS and VOH groundwater plume at shallow, intermediate, and intermediate-deep intervals.
Mobilization to site, collection of depth to groundwater measurements from 18 monitoring wells and perform groundwater sampling of 11 monitoring wells for VOH analysis (see attached Table 1).
Include 18 PFAS analysis for the groundwater monitoring wells with historical exceedances of the provisional GCTL for PFOA, PFOS and their sum, and expanded well network for horizontal and vertical delineation (see attached Table 1).
Proposal for Offsite Monitoring Well Installation and Groundwater Sampling Fabricare Cleaners and Laundry, Inc. ERIC_4741 Tampa, FL April 16, 2025 Terracon Proposal No. PH4257108
Properly manifest the groundwater drum of investigative-derived waste (IDW).
Submit PFAS samples in cooler preserved on ice by overnight courier to FDEP’s laboratory for sample analysis;
Review data and prepare a Groundwater Monitoring Report including updated tables, groundwater flow, and concentration exhibits;
See attached Groundwater Sampling Summary Table and LOE for details
Schedule Deliverable Annual Groundwater Monitoring Report submitted within 30 business days following receipt of laboratory results.
Compensation See attached CEF for details.
SITE RECOMMENDATIONS
Terracon performed groundwater sampling in August 2024 from 19 groundwater monitoring wells for analysis of VOH and PFAS. Based on the results of the 2024 Groundwater Monitoring, concentrations of tetrachloroethene (PCE) and its degradation (daughter) products, including trichloroethene (TCE) and cis-1,2-dichloroethene (cis-1,2-DCE) were reported above their respective groundwater cleanup target levels (GCTLs) pursuant to Chapter 62-777, Florida Administrative Code (FAC). The VOH plume in the shallow groundwater aquifer is defined in onsite monitoring wells with concentrations below the GCTLs. Terracon recommended installation of offsite monitoring wells to define the VOH groundwater impact in the intermediate and intermediate-deep aquifer zones. Additionally, Terracon recommended reducing the scope of the 2025 annual monitoring event by eliminating groundwater monitoring wells which presented non-detected VOH concentrations in the last 2 groundwater sampling events.
Historically PFAS have been reported at concentrations above the provisional GCTL of 70 nanograms per liter (ng/L) for Perfluorooctanoic acid (PFOA), Perfluorooctane sulfonic acid (PFOS), and their sum (PFOA + PFOS)_in the shallow and intermediate monitoring wells. Furthermore, monitoring well MW001 presented PFOA + PFOS at a concentration above 300 ng/L. Based on the updated Waste Site Cleanup Section (WSCS) PFAS Guidance, dated September 13, 2024, Expanded PFAS Monitoring and/or Additional Assessment would be appropriate for Sites with PFOA+PFOS above 300 ng/L.
Terracon recommended expanding the monitoring well network by installing an offsite monitoring well cluster using Direct Push Technologies (DPT). The well cluster will consist of 3 monitoring wells with the following screen intervals:
4-14 feet below ground surface (bgs) – to assist with the PFAS shallow plume delineation 20-30 ft bgs – to assist with the VOH intermediate plume delineation 36-46 ft bgs – to assist with the VOH intermediate-deep plume delineation
April 16, 2025 Terracon Proposal No. PH4257108
The monitoring wells will be constructed with 10 ft of 1-inch diameter polyvinyl chloride (PVC) pre-packed 0.010-inch slotted screen coupled to a sufficient length of PVC casing riser to the ground surface. The annular space of the boring will be packed with 20/30 silica sand to one foot above the screen, followed 30/65 sand and grouted to the surface. The wells will be completed with an 8-inch diameter traffic-rated manhole set in a 2 foot by 2 foot concrete pad. Each well will be developed until the water is clear and free of sediment. The location of the proposed monitoring well cluster is shown in the attached Exhibit 1.
Based on the historical data, Terracon recommends sampling of 11 monitoring wells (see attached Table 1) to confirm the previously reported VOHs concentrations and evaluate any potential trends of VOH concentrations. Additionally, Terracon recommends PFAS sampling of the 9 monitoring wells included in the 2024 PFAS Sampling, and 9 additional wells, including the newly installed wells, for a total of 18 wells (see attached Table 1), to evaluate and delineate the PFAS plume at the Site.
DELIVERABLES
The Groundwater Monitoring Report will detail and summarize the groundwater monitoring well installation, groundwater sampling activities, field observations, and results of monitoring well analysis and will include boring logs, well construction and development logs, IDW disposal documentation updates to the site’s historical data tables, and updated exhibits of groundwater flow and well concentrations. Conclusions and recommendations will include a summary of results and recommendations for future sampling, closure, or remedial activities. As mentioned earlier, the PFAS evaluation will be provided in an additional section within this report.
SCHEDULE
It is estimated that the wells could be sampled within two weeks from TA authorization and report issued within 30 business days of receipt of analytical data.
CLOSING REMARKS
Terracon services will be conducted in accordance with the terms and conditions, contained within DEP Waste Cleanup Program (WCP) Contract No. HW687, executed January 5, 2022. It is understood that a Task Assignment will be issued to authorize us to perform this work.
