About this file

This Task Assignment (TA) is a state contract between the State of Florida Department of Environmental Protection (FDEP) and Terracon Consultants, Inc. for environmental services at the Markham Maytag Laundry site located at 15093 Martin Luther King Boulevard in Alachua, Alachua County, Florida. The contract operates under the parent indefinite delivery contract HW687, executed January 5, 2022, with the FDEP Waste Cleanup Program. Gabriel Sostre serves as the Department Project Manager, and Donna Cline is the Contractor's Project Manager. The Task Assignment is designated as DC015D with a completion date of July 17, 2026.

The scope of work includes four subtasks: project management; groundwater sampling and analysis from eight monitoring wells (four shallow zone and four intermediate zone) for volatile organic halogen (VOH) constituents; PFAS (per- and polyfluoroalkyl substances) sampling from four wells and investigative derived waste (IDW) management; and preparation of an Annual Groundwater Monitoring Report. The total Task Assignment value is $15,631.29, allocated as follows: $1,296.88 for project management, $8,538.13 for groundwater sampling (Subtask 2), $2,560.24 for PFAS sampling and IDW management (Subtask 3), and $3,236.04 for report preparation (Subtask 4). Compensation is based on fixed unit rates and cost reimbursement. Deliverables are due on specified dates with FDEP approval required before invoicing. IDW disposal is managed through a subcontractor, with Erwin Remediation selected at a cost of $740.00 for two 55-gallon drums of non-hazardous PFAS-impacted water, including profiling and transportation services.

(Terracon) appreciates the opportunity to submit this proposal to the Florida Department of Environmental Program (FDEP) Waste Cleanup Program to conduct Annual Groundwater Monitoring (GWM) activities and prepare a report documenting the results for the Markham Maytag Laundry facility (Site), located at the intersection of 15093 Martin Luther King Boulevard in Alachua, Alachua County, Florida.

The purpose of this proposal is to collect groundwater samples from four shallow and four intermediate zone monitoring wells for analysis of volatile organic halogens (VOHs) constituents of concern. In addition, groundwater samples will also be collected from one shallow and three intermediate zone monitoring wells and analyzed for per- and polyfluoroalkyl substances (PFAS) constituents of concern to address the Terracon recommendations in the PFAS Groundwater Monitoring Event deliverable, dated November 30, 2023.

SCOPE OF SERVICES

Subtask 1 – Project Management

Ralph F. Mongillo Jr P.G. will serve as the Project Manager during the field investigation. Donna Cline, P.E., will be involved throughout the project, serving as Authorized Project Reviewer.

Terracon obtained quotes from subcontractors for IDW management services. Exhibits of proposed sample locations are provided in Attachment B. A summary of subcontractor quotes and selected contractor is provided for each service in Attachment C and includes a copy of the quotes.

Subtask 2 – Groundwater Monitoring

Pre-Task Planning will be performed to prepare for field investigations. Terracon will update the existing a site-specific health and safety plan. A contact with the property owner/tenant will be completed to inform them of our field activities as the best way to complete the work with minimal interruptions to the business.

Groundwater Well Sampling Groundwater samples will be collected from four (4) shallow zone wells (MW002, MW003, MW006, and MW017) and four (4) intermediate zone wells MW018, MW022, MW023R, and MW024) for VOH analysis.

The monitoring wells will be sampled in accordance with the FDEP SOPs, dated January 2017 (effective April 16, 2018) for chlorinated solvents. Prior to monitoring well sampling, Terracon will collect depth-to-water measurements of the shallow zone monitoring wells presented in the table below and MW004, MW007, MW014 and intermediate zone wells MW010 MW015. A summary table located below includes the monitoring wells to be sampled, screen elevation interval depths, and the parameters to be analyzed.

Terracon will complete an email deliverable for Subtask 2 documenting the field event is completed by providing field notes, groundwater sampling forms, laboratory chain-of-custodies, and nonhazardous waste manifest.

Subtask 3 – PFAS Screening and IDW Management

Terracon proposes to collect groundwater samples during this mobilization from shallow zone well MW003 and intermediate zone wells MW022, MW023R, and MW024 for PFAS analyses. Groundwater samples are being collected from these four monitoring wells for VOH analysis. PFAS constituents exceeded the practical groundwater cleanup target levels (PGCTL) for one or more PFAS constituents of concern. Terracon will complete the PFAS sampling in general accordance with the Terracon State of Florida Sampling Standard Operating Procedure PFAS Version 2.0, dated January 30, 2024. The groundwater samples will be submitted overnight delivery to the FDEP Central Laboratory (DEP SOP LC- 001-3). One PFAS Quality Assurance/Quality Control (QA/QC) and equipment blank during groundwater sampling activities for a total of one (1) sample. The groundwater and QA/QC samples will be packaged under proper chain-of-custody (COC) procedures and sent overnight delivery to the FDEP laboratory in Tallahassee, Florida for analysis.

January 12, 2026 Terracon Proposal No. PEQ257426

IDW Management and Disposal Liquid IDW derived from purge water during the Subtasks 2 and 3 will be contained in DOT approved 55-gallon drums and temporarily stored onsite until the waste characterization sample results for the full list VOCs and PFAS can be evaluated. Assuming the waste is non-hazardous, the drum(s) will be transported to an approved facility for disposal or treatment. Estimate two (2) drums of liquid.

Subtask 4 – Annual Groundwater Monitoring Report

Terracon will prepare an Annual Groundwater Monitoring (GWM) report that includes a summary of the sampling methodology and analytical results. The GWM report will include tables and figures summarizing the groundwater results in comparison to FDEP’s Cleanup Target Levels (groundwater).

Exhibits will be provided to illustrate the site layout, sample locations, and results by matrix.

SCHEDULE

The goal of the report deliverable is to provide any data obtained by the end of December 2025 and is dependent on FDEP laboratory analytical turn around.
