About this file

This document is a Task Assignment (TA) issued by the State of Florida Department of Environmental Protection (FDEP) to Terracon Consultants, Inc. for environmental services at the Miller Machinery & Supply Company #2 site located at 1920 Tampa East Boulevard, Tampa, Hillsborough County, Florida. The TA is issued under Contract Number HW687, a Waste Cleanup Program contract. The project is assigned TA Number DC020D with a completion date of April 24, 2026, and is associated with FDEP Project Number ERIC_4733. Gabriel Sostre serves as the FDEP Project Manager, while Donna Cline represents Terracon Consultants, Inc.

The scope of work consists of three subtasks: Subtask 1 involves project management; Subtask 2 encompasses groundwater sampling, obtaining Initial Noticing of Contamination (INOC) packages, and securing offsite access agreements; and Subtask 3 includes PFAS (per- and polyfluoroalkyl substances) sampling from four monitoring wells. Deliverables are due by March 27, 2026, and include a Groundwater Monitoring Report with updated charts, tables, exhibits, and laboratory results, approval of INOC packages, signed offsite access forms, and FDEP approval of PFAS analytical results. The total task assignment value is $14,442.03, structured as a fixed unit rate with two invoicing components: $11,966.75 for Subtasks 1 and 2 upon FDEP approval of the Groundwater Monitoring Report and INOC package, and $2,475.28 for Subtask 3 upon FDEP approval of PFAS results. The original estimated cost of $15,467.04 was negotiated downward to $14,442.03, representing a cost reduction of $1,025.01 or 6.63%, with a total of 90.70 labor hours allocated to the project.

Waste Cleanup Contract No. HW687 Terracon Proposal No. PH4247265

Dear Mr. Sostre:

Terracon Consultants, Inc. (Terracon) appreciates the opportunity to submit this proposal to the Florida Department of Environmental Program (FDEP) Waste Cleanup Program to conduct annual groundwater sampling for Volatile Organic Halogenated (VOHs) analysis and prepare offsite access agreements and offsite notification for the Miller Machinery & Supply Company #2 (ERIC_4733), located in 1920 Tampa East Blvd, Tampa, Hillsborough County, Florida.

Scope of Services

Attempt to gain offsite access agreement from 2 southwest properties.

Field preparation, update health and safety plan for groundwater sampling.

Mobilization to site, locate and evaluate monitoring well condition, collection of depth to groundwater measurements from 8 monitoring wells, and perform groundwater sampling of 8 monitoring wells for VOH analysis. Include 4 PFAS analysis for the groundwater monitoring wells

Proposal for Annual Groundwater Sampling Majik Touch Cleaners and Laundry, ERIC_4714 Valrico, FL October 23, 2025 Terracon Proposal No. PH4257302

Properly manifest the groundwater drum of investigative-derived waste (IDW).

Submit PFAS samples in cooler preserved on ice by overnight courier to FDEP’s laboratory for sample analysis;

Review data and prepare a Groundwater Monitoring Report including updated tables, groundwater flow, and concentration exhibits.

Submit Initial Noticing of Contamination (INOC) package for 4 adjacent properties.

Schedule Deliverable Annual Groundwater Monitoring Report submitted within 20 business days following receipt of laboratory results.

Compensation See attached CEF for details.

SITE SUMMARY AND SCOPE OF WORK

Groundwater sample data collected during the 2023 assessment activities suggested that the VOH groundwater contaminant plume, likely originating from the Miller Machinery site, has migrated off-site and southwest and southeast of the site. Contaminant levels exceeding FDEP Natural Attenuation Default Concentrations (NADC) exist beneath the former tetrachloroethylene (PCE) aboveground storage tank (AST), railroad line, and loading dock on the Miller site and extend off-site onto the adjacent Road Safe property southwest of the site. The VOH Groundwater Cleanup Target Levels (GCTL) exceedance contaminant plume extends further off-site southwest onto the Massaro Blvd. properties and commercial properties southeast of the Miller site. The VOH impacts to the groundwater are widespread and appear to originate from the soil source on the Miller site.

During the site assessment activities, Terracon attempted to reach out to some offsite properties located southwest and southeast but received no response at that time. Many of the properties are owned by foreign entities with limited contact information readily available.

Terracon recommends identifying and reaching out to the offsite property owner/tenants to obtain offsite access agreements from the downgradient offsite properties E and H (see locations in Exhibit 1). After obtaining access from property E, Terracon will collect groundwater samples from the existing monitoring wells for VOH analysis. Four (4)

October 23, 2025 Terracon Proposal No. PH4257302 monitoring wells are located on site (W-6, MW-1S, MW-2S, IW-2), and four (4) monitoring wells (IW-3, IW-6, MW-3, and MW-7) are located at the adjacent southwest property (property E). Monitoring well analytical results map from the Site Assessment Report (September 2023) is included in Exhibit 2.

Terracon also recommends sampling groundwater in four (4) of the monitoring wells [MW- 2S (source), W-6 (upgradient), MW-7, and IW-6 (downgradient wells)] (see location in Exhibit 2) for PFAS analysis by DEP SOP LC-001-3 method. Terracon will complete the PFAS sampling in general accordance with the Terracon State of Florida Sampling Standard Operating Procedure PFAS, dated November 15, 2024. The groundwater samples will be submitted overnight delivery to the FDEP Central Laboratory. PFAS screening will be conducted along with the VOC groundwater sampling field activity. An electronic mail deliverable will be submitted to the FDEP within 48 hours after Terracon receives the PFAS analytical results. This deliverable will include the monitoring well locations map with PFAS groundwater results, laboratory reports, groundwater sampling logs, and field notes.

In addition, Terracon recommends the preparation and submittal of the FDEP Initial Notice of Contamination Beyond Property Boundaries (INOC) package for the offsite properties - Parcels IDs: C, E, and H, as well as for the southwest Massaro Boulevard. Terracon will review the groundwater sampling results and coordinate with the FDEP to include additional properties in the INOC package, if necessary.

DELIVERABLES

The Groundwater Monitoring Report will detail and summarize the groundwater sampling activities, field observations, and results of monitoring well analysis and will include equipment calibration logs, groundwater sampling logs, IDW disposal documentation, updates to the site’s historical data tables, and updated exhibits of groundwater flow and well concentrations. Conclusions and recommendations will include a summary of results and recommendations for future sampling and/or remedial activities. Additionally, Terracon will submit The FDEP Initial Notice of Contamination Beyond Property Boundaries package including lab reports, summary tables, and site maps identifying the offsite groundwater contamination.

SCHEDULE

It is estimated that the wells could be sampled within two weeks from TA authorization and report issued within 20 business days of receipt of analytical data.

CLOSING REMARKS

Terracon services will be conducted in accordance with the terms and conditions, contained within FDEP Waste Cleanup Program (WCP) Contract No. HW687, executed January 5, 2022. It is understood that a Task Assignment will be issued to authorize us to perform this work.

October 23, 2025 Terracon Proposal No. PH4257302

We appreciate the opportunity to provide this proposal and look forward to working with you on this project. If you should have any questions or comments regarding this proposal, please contact either of the undersigned at 703-606-8538.

Sincerely, Terracon Consultants, Inc.
