Document Summary

This is a Task Assignment Change Order (TACO #6) issued by the State of Florida Department of Environmental Protection (FDEP) to APTIM Environmental & Infrastructure, LLC for work at the Imperial Cleaners site (ERIC_4106) located at 1500 E Commercial Boulevard, Fort Lauderdale, Broward County, Florida. The original Task Assignment (DC013B) was issued under Contract HW689, with FDEP Project Manager Nicole Knutson and contractor Project Manager Eric Kramer serving as the primary contacts. The change order reconciles Work Change Directive #5 and increases labor hours to accommodate Interim Source Removal Plan (ISRP) revisions and permitting requirements related to the site assessment and remedial design activities.

The total task value has been increased from $131,826.91 to $142,582.43, representing a cost increase of $10,755.52. The contract operates on a fixed unit rate basis with deliverables structured across eight subtasks: project management, site meeting and MAGS investigation, soil and groundwater field activities, final well installations, site assessment reporting, additional assessment work, WCD #5 evaluation, and ISRP modifications. The final deliverable due date was extended from June 23, 2025, to August 25, 2025, with overall task completion extended from August 25, 2025, to November 24, 2025. APTIM's labor, time, and subcontractor costs have been reviewed and determined to be reasonable for the scope of services. Payment is invoiced upon FDEP approval of deliverables, with the total contract cost of $142,582.43 to be distributed across the approved subtasks and their respective milestones.
