Subtask 1: Project Management An automatic calculation based upon total direct labor hours determines the labor hours within this subtask.
Subtask 2: Annual Groundwater Monitoring and IDW Treatment Golder will install two new monitoring wells and collect groundwater samples from eight monitoring wells (MW005, MW006, MW011, MW017, MW024, MW025, and the two new wells). Prior to well installation, Golder will obtain the required permits and oversee a utility locate. Golder requested bids for the utility locate from Blood Hound, GPRS, and GeoTek.
Blood Hound was selected as the lowest responsive and responsible bidder.
Prior to sampling, depth-to-water will be measured. The wells will then be purged, and field parameters will be recorded. The groundwater samples will be collected using low flow sampling methods and analyzed for chlorinated volatile organic compounds (VOCs). Investigation-derived waste (IDW) will be temporarily stored in a 55-gallon drum. Golder requested bids for IDW drum treatment and disposal from Clark, Erwin, Whatever Waste, and M & D Industrial Services. Erwin was selected as the lowest responsive and responsible bidder.
Subtask 3: Reporting Golder will submit the Annual Groundwater Monitoring Report and ADaPT and GIS deliverables to FDEP.
Golder and the FDEP confirmed on May 20, 2022, that an Access Agreement with the current real property owner of the Jet Cleaners (ERIC_4540) site has been executed.
This groundwater sampling event is a part of long-term monitoring to determine when the site will be eligible for closure.

RE: COST PROPOSAL FOR THE FORMER JET CLEANERS
ANNUAL GROUNDWATER MONITORING
1346 GANDY AVENUE
JACKSONVILLE, DUVAL COUNTY, FLORIDA
DEP FACILITY ID NUMBER: ERIC_4583
PROPOSED TASK ASSIGNMENT: HW685_DC021B
Dear Mr. Grubbs:
Golder Associates USA Inc. (Golder) is pleased to provide the Florida Department of Environmental Protection (DEP) with this cost proposal for annual groundwater monitoring at the above-referenced site. This proposal includes a description of the proposed scope of services, level of effort sheets for each subtask, and a cost estimate form that summarizes the costs of the proposed services. The scope of work is based on the recommendations presented in Soil, Soil Vapor, and Groundwater Sampling Letter Report DEP approval electronic mail correspondence, dated April 6, 2021, to Golder. The proposed scope of services is described below.
SUBTASK 1 – PROJECT MANAGEMENT
To facilitate project quality and satisfactory completion of the proposed scope of work, all aspects of this project will be overseen by an experienced project manager. The project manager will be responsible for the schedule, budget, invoicing, project activities, and communications with DEP and the field team. Project management will include coordination of field activities; resource allocation to confirm the work is conducted by qualified professionals in accordance with the project schedule; cost control and tracking of the budget status; and submittal of project invoices in accordance with DEP Drycleaning Solvent Cleanup Program requirements. The project manager will oversee the work so that it is conducted in a manner that is consistent with the requirements of the DEP-approved task assignment.
SUBTASK 2 – ANNUAL GROUNDWATER MONITORING AND IDW TREATMENT
Golder will collect groundwater samples from 6 monitoring wells (MW005, MW006, MW011, MW017, MW024, and MW025) for chlorinated volatile organic compounds (VOCs) by EPA Method 8260B. In addition to collecting
Mr. Ben Grubbs Proposal No. 202208346
Florida Department of Environmental Protection May 20, 2022 groundwater samples from six existing wells, two additional monitoring wells will be installed manually using a hand auger boring device as follows and sampled:
A monitoring well will be installed in the shallow zone of the surficial aquifer adjacent to MW011, so the bottom of the well is no deeper than the top of the clay (at 10 feet bgs).
A shallow zone surficial aquifer monitoring well will be inside the building adjacent to MAGS well MAG006.
A private utility locator subcontractor will be contacted to locate buried utilities at the two proposed boring locations.
Figures 3, 4, 5, and 6 from the previous sampling event completed in 2021 showing groundwater flow and chlorinated VOC results for the two zones of the surficial aquifer, are attached.
Prior to groundwater sampling, depth-to-water measurements will be measured at 15 accessible monitoring well locations (MW001, MW002, MW003, MW004, MW009, MW010, MW012, MW014, MW015, MW016, MW018, MW019, MW022, MW023, MW099) using an electronic water level indicator in addition to the eight wells that will be sampled to determine groundwater depth. During the sampling event, the locks and well caps will be inspected for damage or degradation and repaired or replaced, as necessary.
Groundwater samples will be collected from the wells in accordance with DEP Standard Operating Procedures (SOPs) for Field Activities (DEP SOP-001/01). Prior to collecting the groundwater sample, each well will be purged and field parameters including temperature, pH, specific conductance, dissolved oxygen, turbidity, and oxidation reduction potential (ORP) will be monitored. Purge volumes and field parameters will be recorded on field data sheets, and the field monitoring equipment will be calibrated in accordance with the manufacturer’s recommended procedures.
Groundwater samples will be collected using low flow sampling methods. The groundwater samples will be placed in laboratory-supplied containers, labeled with a unique identification number attached to the outside of the sample container, preserved on ice in a laboratory-provided cooler at a maximum temperature of four degrees Celsius, and delivered to a Florida Department of Health and National Environmental Laboratory Accreditation Program certified fixed-base laboratory along with the completed chain-of-custody documentation to be analyzed for chlorinated VOCs using EPA Method 8260B and MEE at selected wells. Upon receipt of the laboratory analytical results, Golder will evaluate the constituent concentrations to determine if off-site notification is required.
Golder will temporarily store the investigation derived waste (IDW) purge water generated from the groundwater sampling event in a 55-gallon steel drum(s). A waste characterization sample will be collected from the IDW drum(s) for analysis of total VOCs using EPA Method 8260. If the waste characterization sample is acceptable to the disposal subcontractor, the IDW drum(s) will be picked up and transported to a facility for proper disposal. A non-hazardous waste manifest will be provided by the disposal subcontractor upon delivery to the disposal facility.
Upon completion of Subtask 2, an e-mail deliverable will be submitted and include copies of the field data sheets, equipment calibration forms, field notes, laboratory analytical report, and laboratory chain-of-custody documentation.
SUBTASK 3 – REPORTING
The annual groundwater monitoring report will include a description of the sampling procedures, evaluation of the analytical results, comparison of current to historical data, tabulated summary of the current and historical analytical data, a site map showing the groundwater analytical results, water level elevation information in tables and figures, laboratory analytical data, conclusions, and recommendations. Golder will also update the DEP GIS tables with the new water level data, field parameter measurements, and VOC data; run the DEP data checker on the completed tables; and submit the updated tables and error reports under this subtask. Additionally, Golder will forward the Automated Data Processing Tool (ADaPT) data deliverables for the groundwater sampling event provided by the laboratory.
HEALTH AND SAFETY
Golder will update the existing site-specific Health and Safety Plan for the field work. The work will be completed in accordance with Golder’s safety policies and procedures. At a minimum, Golder will wear safety glasses, high visibility safety vests, and steel-toed boots. A safety job briefing will be conducted prior to the start of work activities, and anytime the scope of work or site conditions change.
COST AND SCHEDULE
The estimated cost to complete the proposed scope of work is $13,052.65. Golder anticipates the monitoring event will take place late July or early August 2022. The LOE/CEF sheets and Disposal subcontractor IDW quotes are included in Attachment A. The following table summarizes the proposed costs and deliverable schedule.
