About this file
This Task Assignment (TA) is a contract modification executed between the State of Florida Department of Environmental Protection (FDEP) and GHD Services, Inc., under the indefinite delivery vehicle Contract Number HW684. The task assignment authorizes work at the Garland Supply Co. site located at 7800 W 25th Avenue, Miami-Dade County, Hialeah, Florida. The relevant FDEP project manager is Mackenzie Childs, and the contractor's project manager is Brian Moore. The work is identified as ERIC_4464 and is scheduled for completion by June 12, 2026.
The scope of work consists of two subtasks: Subtask 1 involves project management calculated at 13 percent of direct labor costs, and Subtask 2 encompasses supplemental site assessment activities including the installation and development of eleven replacement monitoring wells (four shallow wells at 4-14 feet below land surface, four intermediate wells at 23-28 feet below land surface, and three deep wells at 43-48 feet below land surface), groundwater sampling at all newly installed wells, and preparation of a supplemental site assessment report due April 9, 2026. The contract is structured as a fixed unit rate with a total task assignment value of $29,897.57. This amount covers all labor (including project management, engineering, and technical staff), overhead, general and administrative costs, fringe benefits, travel, subcontractor services for drilling and well installation, laboratory analytical costs for volatile organic compound analysis, and field expenses. The single invoice milestone requires FDEP review and approval of the supplemental site assessment report plus project management deliverables before the contractor can invoice the full authorized amount.

COST PROPOSAL FOR TASK DC004
< Garland Supply , FDEP ERIC_4464>
< Supplemental Site Assessment >
1. INTRODUCTION: <DISCUSS SCOPE OF WORK, DATES >
GHD previously completed well abandonment and source removal activities at the Garland Supply site located in Hialeah. Following source removal, the property was redeveloped. Task DC004F includes installation of replacement monitoring wells and groundwater sampling.
The wells will be installed on-site to determine current groundwater quality conditions.
Subtask 1 – Project Management Subtask 1 includes all those activities associated with project management for the implementation of supplemental site assessment activities.
Subtask 2 – Supplemental Assessment Subtask 2 includes supplemental assessment activities. Specifically, GHD will oversee the installation of eleven (11) replacement monitoring wells. The wells will include four (4) shallow, 4 intermediate, and three (3) deep wells. The proposed well locations are depicted on the attached site plan. The well screens/depths are as follows
Shallow – 4-14 ft bls Intermediate – 23-28 ft bls Deep – 43-48 ft bls
The wells will be installed and then developed. Following installation, the top of casings of the newly installed wells will be surveyed by GHD personnel.
Following installation and surveying groundwater sampling will be conducted at the eleven newly installed monitoring wells. Prior to sampling, depth to water measurements will be gathered at all available site monitor wells. Groundwater sampling will be conducted in accordance with FDEP standard operating procedures. Groundwater samples will be analyzed for the presence of volatile organic halocarbons.
Upon receipt of analytical data, a supplemental site assessment report (SSAR) will be prepared and submitted to the Department. The SSAR will include a summary of well installation and groundwater sampling.

Breck, Per our discussion, the plan is to install the well clusters shown on the attached map to determine if residual material is located near the former source areas and immediately downgradient.
If residual contaminant is confirmed we can discuss delineation in the future
3 shallow/intermediate/deep clusters - including a cluster in SE corner of property if feasible with overhead lines 1 shallow/intermediate cluster

Re: Non-Hazardous Drum Disposal – State of Florida
Mr. Moore:
The price for the transportation and disposal (T&D) of drums of non-hazardous soil and water located in the State of Florida is listed below.
Non-Hazardous Drums:
Description Rate Unit False Mobilization (No drum found) $ 85.00 Per Stop T&D - Non-Haz Soil $ 110.00 Per Drum T&D - Non-Haz Water $ 125.00 Per Drum
The prices listed are all inclusive without any additional fees or charges (fuel surcharge, low volume stop fee etc.) and are valid for all regions in the State of Florida except for the Florida Keys. Pricing for sites located in the Florida Keys will be bid on a case-by-case basis dependent on the volume of drums and the location.
The lab data we require for the disposal of drums of petroleum impacted soils are totals for BTEX and the 4 RCRA metals (arsenic, cadmium, chromium, and lead), with analyses no older than 1 year from the estimated disposal date.
We do not require analyticals for the disposal of Petroleum Contact Water (PCW) as long as it has only been impacted with petroleum - not pesticides, herbicides or dry-cleaning solvents etc.
Please ensure that all drum lids are properly secured and that drums are on a level surface like the surrounding area (i.e., not on a pallet, etc.). A single False Mobilization fee will apply if our driver is not able to locate a drum on site or if we are unable to access the property due to a locked gate, etc.
This quote is valid for 90 days.
