Assessment
This Task Assignment authorizes GHD Services, Inc. to conduct site assessment activities. Specific subtasks are detailed below:
Subtask 1: Project Management Project Management is calculated at 13% of the direct labor.
Subtask 2: Soil and Groundwater Sampling Soil sampling and groundwater profiling will be conducted in order to establish the extent of soil and groundwater impacts both vertically and horizontally. Soil sampling will include advancement of soil borings within and in the immediate vicinity of the former drycleaner with hand operated equipment and through the use of direct-push technology (DPT). Modified Active Gas Sampling (MAGS) will also be conducted as part of the soil assessment. One continuous soil boring will be completed to establish site lithologic conditions. The initial scope of work includes groundwater profiling at eight (8) exterior locations. All groundwater samples will be submitted to a Florida-certified laboratory for volatile organic analysis by EPA Method 8260. The initial soil sampling will involve the advancement of seven (7) shallow soil borings within and near the former drycleaner facility. All soil samples will be submitted to a fixed laboratory for analysis for the presence of volatile organic compounds (VOCs). Following soil sampling, piezometers will be installed at seven (7) locations. The piezometers will be constructed with 2-inch PVC well screens across the vadose zone. Following installation, MAGS will be conducted and soil gas samples will be gathered for field screening and fixed laboratory analysis for the presence of VOCs by Method TO-15. During the same mobilization, permanent monitor wells will be installed. At this time, six (6) shallow wells and one deep well are planned for installation. One of the shallow wells will be installed within the facility using hand operated equipment. Well locations and construction details will be provided to the Department for approval prior to installation. Following installation, the wells will be surveyed and sampled in accordance with FDEP SOPs. Groundwater samples will be submitted to a fixed laboratory for VOC analysis by EPA Method 8260.
Subtask 3: Surveying/IDW Disposal Following all field activities, a surveyor will be subcontracted to develop a site plan and survey elevations of all sample locations. Investigation derived waste (IDW) disposal is anticipated and has been accounted for under this subtask. Subcontractors for IDW disposal were chosen on a lowest-cost basis.
Subtask 4: Site Assessment Report Upon receipt of analytical data, a Site Assessment Report (SAR) will be prepared. The SAR will include a summary of field methodology, field screening data, and laboratory analytical results. The SAR will also include conclusions and recommendations. The recommendations may include the use of active remediation to mitigate identified impacts.
GHD and the FDEP confirmed on June 26, 2023, that an Access Agreement with the current real property owner of the aforementioned site has been executed.
This work will be conducted in line with the scope of work included with GHD’s proposal, received on June 9, 2023. Documentation of the costs is included in the attached CEF, Level-of-Effort sheets, and proposal letter. The final deliverable due date is October 30, 2023, and the completion date for the task assignment is January 26, 2024.
The final cost total for Task Assignment DC039B is $68,729.88.

COST PROPOSAL FOR TASK DC039B
<China Boy Cleaners/FDEP ERIC_5271>
< Site Assessment >
1. INTRODUCTION: <DISCUSS SCOPE OF WORK, DATES >
GHD Services Inc. (GHD) submitted a Site Assessment Workplan for the China Boy Cleaners site located in Largo, Pinellas County, in April 2023. The Site Assessment Workplan detailed an approach to determining the extent of contamination in soil and groundwater both vertically and horizontally. Task DC039B includes initial site assessment activities including soil sampling, modified active gas sampling (MAGS), discrete groundwater profiling using direct push technology (DPT), monitor well installation, and groundwater sampling
Subtask 1: Project Management
Subtask 1 includes all tasks associated with project management of the site assessment activities at the Ideal Cleaners Laundry & Shoe site.
Subtask 2: Soil and Groundwater Sampling
Upon Department approval, GHD will mobilize with the selected direct-push contractor. Soil sampling and groundwater profiling will be conducted in order to establish the extent of soil and groundwater impacts both vertically and horizontally. Soil sampling will include advancement of soil borings within and in the immediate vicinity of the drycleaner with hand operated equipment and through the use of direct-push technology (DPT). Modified Active Gas Sampling (MAGS) will also be conducted as part of the soil assessment. One continuous soil boring will also be completed to establish site lithologic conditions.
The initial scope of work includes groundwater profiling at eight (8) exterior locations. At each location, direct-push tooling will be advanced to the desired depths and discrete groundwater samples will be gathered for fixed laboratory analysis. For planning purposes, it is assumed that discrete groundwater samples will be gathered from the surface of the groundwater, 10, 20 feet, 30 feet, 40 feet, and 50 feet below land surface. All groundwater samples will be submitted to a Florida-certified laboratory for volatile organic analysis by EPA Method 8260.
The initial soil sampling will involve the advancement of seven (7) shallow soil borings within and in close proximity to the former drycleaner facility. Soil borings will be advanced with a stainless steel hand auger and soil samples will be collected from 0-0.5’, 0.5-2.0’, and 2’ intervals until refusal or the water table. All soil samples will be submitted to a fixed laboratory for analysis for the presence of volatile organics (VOCs).
At this time, up to six (6) shallow and one deep well are planned for installation. The 6 shallow wells will be installed to approximate depths of 15 ft bls. One of the shallow wells will be installed within the facility using hand operated equipment. The wells will be screened to straddle the water table. The deep well is proposed for a total depth of 50 feet and will be screened between 45 and 50 feet, depending on site lithology. Well locations and construction details will be provided to the Department for approval prior to installation. Following installation, the wells will be surveyed and sampled in accordance with FDEP SOPs. All of the groundwater samples will be submitted to a fixed laboratory for VOC analysis by EPA Method 8260.
Subtask 3: Surveying/IDW Disposal Following all field activities, a surveyor will be subcontracted to develop a site plan and survey elevations of all sample locations. Investigative Derived Waste (IDW) will be disposed of at an appropriate off-site facility. GHD will coordinate manifest generation and IDW pickup.

Dear Brian Moore, Colliers Engineering & Design, Inc. (DBA Maser Consulting) is pleased to submit this proposal to provide professional services for a Boundary & Partial Topographic Survey in Largo, Pinellas County, Florida. The property in question is located at the Northeast quadrant of Missouri Avenue North and Jasper Street. The overall property consists of approximately 3.2 acres of land and currently an active strip shopping center.
This proposal is divided into four sections as follows:
Section I – Scope of Services Section II – Business Terms and Conditions Section III – Technical Staff Hourly Rate Schedule and Reimbursable Expenses Section IV – Client Contract Authorization

Section I – Scope of Services
Based on our conversations and information noted above, we propose to complete the following:
Boundary Survey
Colliers Engineering & Design will prepare a Boundary Survey of Lots 15 though 20, as per map or plat thereof Marlowe Heights Subdivision as recorded in Plat Book 46, Page 62 of the Public Records of Pinellas County, Florida in accordance with the Standards of Practice as established by the Florida Board of Surveyors and Mappers.
Proposal No. 23007555P May 23, 2023 Page 2 | 14
Included in this task of service are the following tasks:
Public records research and pre-field records review;
Field traverse, location survey and data collection;
Field survey data reduction and computation;
Boundary analysis and survey calculations;
Preparation of legal description of the subject property’s boundary.
Unless provided by the client, the boundary survey will be subject to such facts that a current and accurate title report would disclose.
The client should furnish the surveyor with the following information/documents before we initiate services on the project:
A copy of the property deed(s) of record;
A copy of the property title binder;
A copy of all adjoining deeds and reference filed maps. (Colliers Engineering & Design can obtain this information for you if you are unable to through our subcontractor. This fee will be passed onto you as a reimbursable expense);
A corner marker waiver form (unless corner markers are desired to be set) (form enclosed);
A statement in writing of the person or persons to be named in the certification of survey (form enclosed).
Final Deliverable. The final deliverable will be a maximum of three (3) hardcopies signed and sealed by a Florida Surveyor and Mapper, and an electronic file copy in Adobe pdf format. Draft deliverables will be in form of electronic file Adobe pdf format.
Delivery of hard copy maps will be limited to one (1) package sent to one (1) address, one (1) time.
Additional delivery of hard copy maps will be billed as a reimbursable expense in accordance with Section III of this agreement.
The fee for this task is based upon the assumption that the deed mathematically closes, that there are no title problems, that there are no overlaps or gores with adjoining properties and that extraordinary research or analysis is not required. If any of the items cited in this paragraph do become issues during our survey efforts, we will discuss the necessary additional services and related costs with you prior to proceeding with the additional services.
Lump Sum Fee $ 6,400.00
Partial Location & Topographic Survey
Colliers Engineering & Design will perform a Partial Topographic Survey of applicable state and local jurisdictional requirements and standards of practice.

Proposal No. 23007555P May 23, 2023 Page 3 | 14
SURVEY LIMITS
Our office will prepare a Partial Topographic Survey map that is a graphic pictorial representation of existing site features observed at the time of the field survey such as buildings, curbs, sidewalks, roadways, driveways, retaining walls, fences, and utility hardware. Limits of wooded areas will be depicted based on the approximate dripline, but individual trees within wooded areas will not be surveyed. The topographic map will depict existing spot elevations and contours at 0.50-foot contour intervals. GPS surveying techniques will be used to control the survey with the resulting horizontal datum being Florida West State Plane Coordinate System NAD83 and the vertical datum being North American Vertical Datum NAVD88.
Visible and accessible utilities and/or utility structures within the survey limits as described above will be surveyed and shown on the plan to include rim, grate and invert elevations, and pipe sizes entering and/or exiting the structures. For the purposes of this contract, accessible utilities shall be defined as those utilities that are visible to the naked eye at ground level and are safely accessible by foot by Colliers Engineering & Design field survey personnel without the need for additional safety measures and/or assistance with making pipes visible, open, and clear for inspection and measuring.
We will survey visible evidence of existing utilities within the survey limits, but may not be able to confirm the existence, or actual position of all underground utilities which may be running through or servicing the subject property. The Florida 811 One Call System prohibits the use of its service for surveying and mapping of subsurface utilities for engineering design purposes. If requested, we can enlist the services of our in-house subsurface utility engineers to investigate and mark the
